- A de novo, korábban kezeletlen nagy B-sejtes limfóma (LBCL) szövettanilag megerősített (helyi értékelés alapján) diagnózisa az Egészségügyi Világszervezet (WHO) 2022-es osztályozása szerint, beleértve:

i) Diffúz nagy B-sejtes limfóma (DLBCL), másként nem meghatározott [beleértve a germinális központ B-sejtes (GCB) és az aktivált B-sejtes (ABC) típusokat]

ii) Magas fokú B-sejtes limfóma, MYC és BCL2 átrendeződésekkel

iii) Magas fokú B-sejtes limfóma, másként nem jelölve

iv) T-sejt/hisztiocita/gazdag nagy B-sejtes limfóma (THRLBCL)

v) Epstein-Barr vírus + DLBCL

  • A nemzetközi prognosztikus index (IPI) 1-es vagy 2-es pontszáma a laktát-dehidrogenáz (LDH) ≥ 1,3-szorosa a normálérték felső határának (ULN) és/vagy a terjedelmes betegség, amelyet egyetlen, ≥ 7 cm-es lézióként, VAGY IPI ≥ 3-mal definiálnak.
  • Mérhető betegség, amelyet legalább 1 fluorodezoxiglükóz (FDG)-avid lézió határoz meg az FDG-avid altípusra és 1 kétdimenziósan mérhető (leghosszabb átmérőjű > 1,5 cm) betegség számítógépes tomográfiával (CT) vagy mágneses rezonancia képalkotással (MRI), a definíció szerint a luganói besorolás szerint.

Kizárási kritériumok:

  • A limfóma bármely más altípusa. Primer mediastinalis (thymus) nagy B-sejtes limfóma (PMBCL), primer bőr DLBCL-láb típusú, 3b fokozatú FL, a-BCL-be transzformált FL, anaplasztikus limfóma kináz (ALK) pozitív nagy B-sejtes limfóma, primer effúziós limfóma esetei és Burkitt limfóma.
  • Dokumentált vagy feltételezett központi idegrendszeri (CNS) limfóma érintettség.
  • Egyéb, a protokollban meghatározott felvételi/kizárási kritériumok érvényesek.
