参与者被分配到三个平行组,为期12周:
- 足踝运动(教育加印刷材料;独立练习,每天2-3次),
- 步行运动(教育加印刷材料;每周≥150分钟,每次≥10分钟,避免连续两天不运动),或
- 主动对照组(教育和印刷材料)。
结果在基线(第0天)、第6周和第12周进行评估。分析侧重于与预设进展标准相比的可行性指标,以及对神经病变、压力和DR-QoL障碍的组内和组间变化的假设生成估计;本研究未针对明确疗效进行效能计算。

干预措施描述 足踝锻炼干预 该干预措施改编自先前研究和印度尼西亚内分泌协会开发的锻炼方案。参与者接受简短的糖尿病和足踝教育、宣传单页以及监督下的模拟练习以确保技术正确。独立练习每天进行2-3次,持续12周。目标锻炼时长≥150分钟/周。
结构化的足踝锻炼计划包含10个坐姿动作,涉及脚趾屈曲、提踵、踝关节圆周运动、膝关节伸展配合脚趾活动、双侧同步运动以及功能性足部任务(例如操作报纸页)。
步行锻炼干预 步行计划改编自先前研究方案。参与者接受糖尿病和步行锻炼教育及宣传单页。他们在12周内独立步行≥150分钟/周(每次≥10分钟),并避免连续两天不锻炼。
主动对照组 参与者接受常规糖尿病教育(30分钟)和宣传单页。不提供结构化锻炼计划。
所有组别继续接受标准公共卫生服务。每周进行监测和不良事件检查。
内容效度评估程序
为确保干预程序、教育材料和测量方案(足部感觉、振动觉、身高、体重、血压、踝肱指数)的标准化,进行了内容效度评估。六位专家(具有≥5年糖尿病相关经验的医师或护士)使用1-4分量表完成项目级评分。内容效度指数阈值:
项目级≥0.80 量表级≥0.83 低于阈值的项目根据专家反馈进行修订,直至达到满意的CVI值。CVI结果显示项目级CVI>0.90,量表级CVI为0.96。
参与者招募 招募工作与日惹地区卫生办公室及三家运行慢性病管理计划的公共卫生中心合作开展。公共卫生中心识别潜在符合条件的糖尿病患者并协助初步联系。感兴趣者获得关于研究程序、风险/获益、保密性和自愿参与的标准信息。愿意参与者参加筛选和知情同意会议。同意后,参与者被纳入并加入通讯群组。
参与者每次数据收集访视获得25,000印尼盾交通补贴。招募材料经机构审查委员会批准。参与自愿且不影响常规服务获取。
样本量 共招募108名参与者(每组36人),预计20%脱落率。样本量选择基于可行性评估,并为糖尿病周围神经病变、压力和糖尿病视网膜病变生活质量结局提供初步假设生成估计;不用于确定后续确定性随机对照试验的样本量。

随机化和分配隐藏 随机化由不参与招募或数据收集的独立人员执行。使用计算机生成的随机置换区组序列(区组大小3、6、9),分配比例为1:1:1。

足踝锻炼组
参与者在分配程序后接受糖尿病和足踝锻炼健康教育,随后进行模拟和练习,持续60分钟。参与者亦获得宣传单页。之后,参与者在无监督下独立练习12周。足踝锻炼包含10个动作。进行足踝锻炼时,参与者舒适端坐不倚靠,脚掌接触地面且赤足。这些动作为:

所有组别的健康教育由认证糖尿病教育师提供。为减少污染,分组教育在不同日期进行。所有参与者加入受监管的WhatsApp群组用于沟通、安排和提醒。
干预实施者和结局调查员
干预措施由符合以下条件的卫生专业人员实施:
- 护理学硕士学位,
- ≥5年临床/教育经验,且
- 持有糖尿病教育师认证。每个研究点包括≥1名注册卫生工作者担任结局调查员。
不良事件监测 潜在不良事件包括疲劳、肌肉酸痛、疼痛和头晕。参与者自行记录事件并在每周电话监测中报告。如发现过度或令人担忧的不良事件,则停止干预并提供适当照护。
