腮腺内转移性皮肤鳞状细胞癌的立体定向术后放疗
本研究的目的是比较立体定向放疗(5次疗程)与常规(标准)放疗(20-30次疗程)在手术切除后治疗涉及头颈部的皮肤癌的有效性和副作用。
立体定向放疗通过损伤癌细胞的遗传物质并阻止癌细胞自我复制来杀死癌细胞,其作用方式与常规(标准)放疗相同。
本研究将帮助研究医生确定这种不同的方法对于您的癌症治疗是否与标准护理相同、更好或更差。

本研究是一项II期随机试验,患者将以1:2的比例随机分配至常规分割术后放疗(PORT)标准治疗(组1)与超低分割立体定向术后放疗(PORT)(组2)。 患者将根据美国癌症联合委员会(AJCC)第8版分期进行病理淋巴结状态(pN1对比pN2-pN3)分层,并根据免疫治疗使用情况(分类为新辅助免疫治疗(含或不含辅助免疫治疗)对比仅计划辅助免疫治疗对比无免疫治疗)进行分层。 随机分配至组2的患者还将与历史对照数据进行比较,主要终点为2年肿瘤局部控制率。
本研究旨在评估超低分割立体定向体部放疗(SABR)与常规分割相比,在头颈部局部晚期淋巴结阳性皮肤鳞状细胞癌切除术后辅助放疗中的临床疗效、毒性和生活质量(QOL)。

- 患者适合接受根治性治疗
- 患者能够充分理解英语以完成患者报告结局问卷
- 活检确诊的皮肤鳞状细胞癌
- 头颈部原发性皮肤肿瘤已行根治性切除
- 肿瘤分期T1-T4(AJCC第8版);或肿瘤分期未知(T0/Tx)但经治肿瘤科医生认为存在阳性腮腺内、腮腺周围或颈部淋巴结且推测来自头颈部皮肤鳞状细胞癌
- 淋巴结分期N1-N3(AJCC第8版)
至少符合以下1项辅助放疗指征:
- T3或T4肿瘤分期
- 淋巴血管侵犯(LVI)
- 神经周围侵犯(PNI)
- 切缘阳性或切缘过近(≤ 3 mm)
- ≥ 1个阳性腮腺内、腮腺周围或颈部淋巴结
- 多次局部复发或多灶性疾病
- 允许新辅助或辅助免疫治疗
排除标准
- 确诊时存在明确转移性疾病
- 妊娠或哺乳期女性
- 存在重大健康问题或手术及放疗禁忌证
- 5年内有头颈部癌症病史(局部皮肤癌除外,即无淋巴结或远处转移)
- 既往接受过涉及同侧腮腺或颈部的头颈部放疗。但若经治肿瘤科医生判断无重叠或重叠可忽略不计,则允许既往针对本次试验所治疗腮腺淋巴结疾病的原发皮肤癌进行过放疗
- 存在对侧颈部放疗指征(即对侧或双侧淋巴结受累)
- 5年内有浸润性恶性肿瘤病史(除非已控制且无疾病证据)
