Beskrivelse
Inklusionskriterier:
- Patienter nyligt diagnosticeret som HCC i henhold til European Association for Study of the Liver-kriterier.
- BCLC trin A eller B
- Child-Pugh klasse A eller B (Child-Pugh score 7)
- ECOG-ydelsesstatus på 0
Patienter skal have mindst én tumorlæsion, der opfylder begge følgende kriterier:
- Læsionen kan måles nøjagtigt i mindst én dimension i henhold til RECIST-kriterier
- Læsionen er ikke tidligere blevet behandlet med kirurgi, strålebehandling, radiofrekvensablation, perkutan ethanol- eller eddikesyreinjektion eller kryoablation.
- Patienter, der har modtaget tidligere lokale terapibehandlinger (RFA, PEI, kryoablation, kirurgi, resektion) til ikke-mållæsioner er kvalificerede
- Lokal terapi skal være afsluttet mindst 4 uger før baseline-scanning.
- Hæmatologi: Absolut neutrofiltal (ANC) > 1 x 109/L, Blodpladetal > 40 x 109/L, Hæmoglobin > 9 g/dL (kan transfunderes for at opretholde eller overskride dette niveau) Protrombintid internationalt normaliseret forhold < 1,5
- Biokemi: Total bilirubin < 2 mg/dL Serumkreatinin < 1,5 x den øvre normalgrænse
- Evne til at forstå protokollen og acceptere og underskrive et skriftligt informeret samtykkedokument
Ekskluderingskriterier:
Tumorfaktorer
- Tilstedeværelse af ekstrahepatisk metastase
- Overvejende infiltrativ læsion
- Diffus tumormorfologi med omfattende læsioner, der involverer begge lapper.
Vaskulære komplikationer
- Hepatisk arterie trombose, eller
- Delvis eller fuldstændig trombose af hovedportvenen, eller
- Tumorinvasion af portalgren af kontralateral lap, eller
- Hepatisk venetumor trombe, eller
- Betydelig arterioportal-shunt, der ikke er modtagelig for shuntblokering
Leverfunktion
- Avanceret leversygdom: ascites, hepatisk encefalopati
- Patienter med klinisk signifikant gastrointestinal blødning inden for de 30 dage før studiestart.
Andre
- Nyresvigt, der kræver hæmo- eller peritonealdialyse
- Gravide eller ammende kvinder.
- Aktiv sepsis eller blødning.
- Overfølsomhed over for intravenøse kontrastmidler.
- Patienten har modtaget forudgående behandling for HCC-mållæsion.
Anamnese med hjertesygdom
- Kongestiv hjertesvigt > NYHA klasse 2; aktiv koronararteriesygdom
- Hjertearytmier, der kræver anden antiarytmisk behandling end betablokkere eller digoxin.
- Hypertension defineret som systolisk blodtryk > 150 mmHg eller diastolisk tryk > 90 mmHg trods optimal medicinsk behandling.
- Alvorlige ikke-helende sår (herunder sår, der heler ved sekundær hensigt), akutte eller ikke-helende sår eller knoglebrud inden for 3 måneder.
- Patienten er efter undersøgerens opfattelse ude af stand og/eller uvillig til at efterleve behandlings- og undersøgelsesinstruktioner.
- Stofmisbrug (nuværende), psykologiske eller sociale forhold, der kan forstyrre patientens deltagelse i undersøgelsen eller evaluering af undersøgelsesresultaterne.
- Alle aktive klinisk alvorlige infektioner (> grad 2 NCI-CTCAE ver 3.0)
- HIV-infektion eller AIDS-relateret sygdom eller alvorlig akut eller kronisk sygdom (baseret på sygehistorie)

Aktiv komparator: Kun TACE
TACE med kemoterapilægemidler (E-ADM 50 mg, Lobaplatin 50 mg, MMC 6 mg) og efterfulgt af embolisering med lipiodol eller/og polyvinylalkoholpartikler.

Eksperimentel: TACE Plus Adenovirus
Efter identifikation af målarterien for HCC, rekombinant humant adenovirus type 5-injektion (15,0*1011vp:0,5ml*3)
vil først blive infunderet gennem målarterien på HCC-patienten og efterfulgt af kemoterapimedicin (E-ADM 50 mg, Lobaplatin 50 mg, MMC 6 mg) og lipiodol-emulsion eller/og polyvinylalkoholpartikler (afhængig af tumorstørrelsen) Procedure: TACE (Transkateter arteriel kemoembolisering)

Transarterisk kemoembolisering med kemoterapilægemidler (E-ADM 50 mg, Lobaplatin 50 mg, MMC 6 mg) og efterfulgt af embolisering med lipiodol eller/og polyvinylalkoholpartikler.
