Det primære formål med Carotis Revaskularization Endarterectomy versus Stenting Trial (CREST) er at sammenligne den relative effektivitet af carotis arterie stenting (CAS) versus carotis endarterektomi (CEA) til at forhindre slagtilfælde, myokardieinfarkt og død. Stenter er medicinsk udstyr, der er godkendt og almindeligt anvendt til behandling af hjertesygdomme. Stenten, der vil blive brugt i dette forsøg, er Rapid Exchange(RX) ACCULINK(TM) Carotid Stent System, et elastisk-lignende metalstillads, der udvides inde i en halspulsåre for at holde karret åbent.
RX ACCUNET(TM) Embolic Protection System (en paraplylignende enhed, der udvider sig over den indsnævrede del af halspulsåren) vil blive brugt sammen med RX ACCULINK-stenten. RX ACCUNET-systemet er designet til at fange embolisk materiale, der kunne brække af fra det indsnævrede område i halspulsåren, mens det stadig tillader blod at strømme gennem karret under proceduren. Embolisk materiale kan blokere blodgennemstrømningen til arterierne ud over forsnævringen og være skadelig for hjernen. RX ACCUNET-systemet lukkes og fjernes, efter at stenten er placeret.
CEA involverer et halssnit og fysisk fjernelse af plaque fra indersiden af arterien. CAS involverer indsættelse af et kateter eller en slange i en arterie i lysken og derefter at føre kateteret gennem arterierne i kroppen til placeringen af pladen i halspulsåren i halsen. Stenten placeres derefter for at dække pladen og holde arterien åben. Deltagerne vil blive tilfældigt tildelt til at gennemgå enten CAS eller CEA, og alle patienter vil modtage den bedste medicinske behandling, som omfatter behandling med aspirin, behandling af forhøjet blodtryk og behandling af andre risikofaktorer for slagtilfælde. Deltagerne vil blive fulgt i op til ti år.
Med forenklingen af protokollen for langsigtet opfølgning er fokus for sekundære resultater at vurdere restenose og procedurens levedygtighed. Restenoserater på årsbasis i både endarterektomi- og stentingarme af undersøgelsen vil blive vurderet ved hjælp af carotis duplex ultralydsundersøgelser udført årligt (standardbehandling).
Ud over restenose vil "levedygtigheden" af procedurerne blive vurderet ud fra behovet (eller manglen på behov) for gentagen revaskularisering (enten åben kirurgisk eller endovaskulær) efter indeksproceduren. Spørgsmålet "er der foretaget en ny carotis-intervention siden sidste opfølgning" vil blive stillet ved hver kontakt med patienten. Hvis svaret er positivt, vil der blive indsamlet yderligere data på de relevante case report formularer (CRF'er).
FORBINDELSE AF CREST COHORT MED CENTER FOR MEDICAID OG MEDICARE SERVICES(CMS) ADMINISTRATIVE DATA
Formålet med at forbinde Medicare-kvalificerede CREST-deltagere med CMS-datafiler er at vurdere patientresultater og brug af sundhedsydelser. Denne plan for at forbinde in- og ambulante episoder af pleje ved hjælp af nationale Medicare-data etablerer en ny ressource, der vil forbedre nuværende opfølgningsstrategier, samt udforske alternative strategier til at fastslå patientresultater for fremtidige kliniske undersøgelser.

Inklusionskriterier:
- Symptomatiske patienter med nylige neurologiske hændelser (TIA eller ikke-invaliderende slagtilfælde) med en associeret carotisstenose større end eller lig med 50 % ved angiografi eller mere end eller lig med 70 % ved ultralyd eller mere end eller lig med 70 % ved computertomografiangiografi (CTA) eller Magnetic Resonance Angiography (MRA) er berettiget til randomisering.
- Asymptomatiske patienter uden nylige (inden for de sidste 6 måneder) neurologiske hændelser, der kan henføres til undersøgelsen med arterie- og carotisstenose (patienter med symptomer efter 180 dage betragtes som asymptomatiske) større end eller lig med 60 % ved angiografi eller større end eller lig med 70 % ved ultralyd eller større end eller lig med 80 % ved CTA eller MRA er berettiget til randomisering.
Ekskluderingskriterier:

Det primære formål med CREST er at vurdere, om effektiviteten af CAS adskiller sig fra effekten af CEA til at forebygge slagtilfælde, myokardieinfarkt og død i løbet af en 30-dages peri-procedureperiode eller ipsilateralt slagtilfælde over opfølgningsperioden hos patienter med symptomatisk ( >=50%) eller asymptomatisk (>=60%) ekstrakraniel carotisstenose.
