Het primaire doel van de Carotid Revascularization Endarteriëctomie versus Stenting Trial (CREST) is om de relatieve effectiviteit van halsslagaderstenting (CAS) versus halsslagader-endarteriëctomie (CEA) te vergelijken bij het voorkomen van beroerte, hartinfarct en overlijden. Stents zijn goedgekeurde medische apparaten en worden vaak gebruikt voor de behandeling van hartaandoeningen. De stent die in dit onderzoek zal worden gebruikt, is het Rapid Exchange(RX) ACCULINK(TM) carotis-stentsysteem, een elastisch metalen skelet dat in een halsslagader wordt uitgezet om het bloedvat open te houden.
Het RX ACCUNET(TM) embolische beschermingssysteem (een paraplu-achtig apparaat dat uitzet boven het vernauwde deel van de halsslagader) zal worden gebruikt in combinatie met de RX ACCULINK-stent. Het RX ACCUNET-systeem is ontworpen om embolisch materiaal op te vangen dat zou kunnen afbreken van het vernauwde gebied in de halsslagader, terwijl er tijdens de procedure nog steeds bloed door het bloedvat kan stromen. Embolisch materiaal kan de bloedtoevoer naar de slagaders voorbij de vernauwing blokkeren en schadelijk zijn voor de hersenen. Het RX ACCUNET-systeem wordt gesloten en verwijderd nadat de stent is geplaatst.
CEA omvat een nekincisie en fysieke verwijdering van de plaque uit de binnenkant van de slagader. CAS omvat het inbrengen van een katheter of buis in een slagader in de lies en het vervolgens doorsnijden van de katheter door de slagaders van het lichaam naar de locatie van de plaque in de halsslagader in de nek. De stent wordt dan geplaatst om de plaque te bedekken en de slagader open te houden. Deelnemers worden willekeurig toegewezen om CAS of CEA te ondergaan, en alle patiënten krijgen de beste medische behandeling, waaronder behandeling met aspirine, behandeling van hoge bloeddruk en behandeling van andere risicofactoren voor een beroerte. Deelnemers worden maximaal tien jaar gevolgd.
Met de vereenvoudiging van het protocol voor follow-up op lange termijn, ligt de focus voor secundaire uitkomsten op het beoordelen van restenose en levensvatbaarheid van de procedure. Restenosepercentages op jaarbasis in zowel de endarteriëctomie- als de stentarmen van het onderzoek zullen worden beoordeeld met behulp van carotis duplex echografie-onderzoeken die jaarlijks worden uitgevoerd (standaardzorg).
Naast restenose zal de "levensvatbaarheid" van de procedures worden beoordeeld aan de hand van de noodzaak (of het ontbreken van behoefte) aan herhaalde revascularisatie (open chirurgisch of endovasculair) na de indexprocedure. Bij elk contact met de patiënt zal de vraag gesteld worden "is er een nieuwe halsslagaderinterventie uitgevoerd sinds de laatste follow-up". Indien positief geantwoord, zullen aanvullende gegevens worden verzameld op de juiste case report-formulieren (CRF's).
VERBINDING VAN CREST COHORT MET CENTRA VOOR MEDICAID EN MEDICARE SERVICES (CMS) ADMINISTRATIEVE GEGEVENS
Het doel van het koppelen van Medicare-geschikte CREST-deelnemers aan CMS-gegevensbestanden is het beoordelen van patiëntresultaten en het gebruik van gezondheidszorgdiensten. Dit plan om klinische en poliklinische zorgepisoden aan elkaar te koppelen met behulp van nationale Medicare-gegevens, vormt een nieuwe bron die de huidige follow-upstrategieën zal verbeteren en alternatieve strategieën zal onderzoeken om patiëntresultaten vast te stellen voor toekomstige klinische studies.

Inclusiecriteria:
- Symptomatische patiënten met recente neurologische voorvallen (TIA of niet-invaliderende beroerte) met een geassocieerde halsslagaderstenose groter dan of gelijk aan 50% door angiografie of groter dan of gelijk aan 70% door echografie of groter dan of gelijk aan 70% door computertomografie-angiografie (CTA) of Magnetic Resonance Angiography (MRA) komen in aanmerking voor randomisatie.
- Asymptomatische patiënten zonder recente (in de laatste 6 maanden) neurologische gebeurtenissen die naar het onderzoek verwijzen met arterie- en halsslagaderstenose (patiënten met symptomen langer dan 180 dagen worden als asymptomatisch beschouwd) groter dan of gelijk aan 60% door angiografie of groter dan of gelijk aan 70 % volgens echografie of groter dan of gelijk aan 80% volgens CTA of MRA komen in aanmerking voor randomisatie.
Uitsluitingscriteria:
- Aandoeningen die: (1) de evaluatie van eindpunten verstoren, (2) waarvan bekend is dat ze de voltooiing van CEA of CAS verstoren, of (3) de overlevingskans voor de onderzoeksperiode (4 jaar) beïnvloeden. Chronische boezemfibrilleren en/of antistolling of episodische boezemfibrilleren in de afgelopen 6 maanden.

Halsslagader-endarteriëctomie is een operatie om de opbouw van tandplak te verwijderen die vernauwing (stenose) in de halsslagader veroorzaakt.

ACTIVE_COMPARATOR: Stenting van de halsslagader (CAS)
Halsslagaderstenting (CAS) is een procedure die wordt gebruikt om vernauwde halsslagaders te openen.
Tijdens de procedure wordt een kleine, uitzetbare draadbuis, een stent genaamd, permanent in de halsslagader ingebracht.

Het primaire doel van CREST is om te beoordelen of de werkzaamheid van CAS verschilt van die van CEA bij het voorkomen van beroerte, myocardinfarct en overlijden gedurende een peri-procedurele periode van 30 dagen, of ipsilaterale beroerte tijdens de follow-upperiode bij patiënten met symptomatische ( >=50%) of asymptomatische (>=60%) extracraniale halsslagaderstenose.
Vier jaar follow-up, verhoudingen die de absolute werkzaamheid van halsslagaderstenting (CAS) ten opzichte van die van halsslagader-endarteriëctomie (CEA) weerspiegelden, waren gebaseerd op Kaplan-Meier overlevingsschattingen aan het einde van de 4 jaar.
