Forbedring af sikkerheden og ernæringsmæssig tilstrækkelighed af hjemmemadforsyningen til ældre modtagere af hjemmeleverede måltider
Rutgers University vil samarbejde med Meals on Wheels America (MOWA) og tilknyttede agenturer i fem stater for at gennemføre en undersøgelse designet til at forbedre fødevaresikkerheden ernæring og nødberedskab blandt hjembundne ældre modtagere af hjemmeleverede måltider (HDM). Denne befolkning har øget risiko for fødevarebårne sygdomme som følge af unikke fysiologiske og adfærdsmæssige faktorer og er særligt sårbar over for eventuelle forstyrrelser i fødevaresystemet på grund af deres manglende mobilitet.
En multi-metode forskningstilgang vil blive brugt med en målprøve på 1.000 MOWA hjemmebundne ældre klienter. Metoderne omfatter brugen af en ny UPC-scanningsteknologi, der hurtigt og omfattende katalogiserer al maden i kundernes hjem, en fødevaresikkerhedsrevision i hjemmet og et ansigt-til-ansigt interview.

Ældre hjemvendte modtagere af hjemmeleverede måltider (HDM) har øget risiko for fødevarebåren sygdom og er særligt sårbare over for forstyrrelser, der kan forhindre levering eller sikker indtagelse af fødevarer. Derudover er der en stor variation i kognitive og fysiske evner hos personer inden for denne befolkning, såvel som en række adgang til økonomiske ressourcer og social støtte, der kan variere både individuelt og geografisk. Desuden er de agenturer, der betjener denne befolkning, stærkt afhængige af tid og donationer fra frivillige til at udføre deres vigtige arbejde. For at sikre deres implementering skal interventioner designet til at løse fødevaresikkerhedsproblemer i denne befolkning være relativt enkle, effektive og omkostningseffektive.
For at løse disse problemer vil der blive brugt en multimetode-forskningstilgang, der inkorporerer Universal Product Code (UPC) scanningsteknologi kombineret med en dynamisk voksende database over fødevarer og deres tilknyttede ernæringsparametre for at katalogisere madforsyningen i hjemmet, en eksisterende hjemmemad sikkerhedsauditskema, modificeret til brug i undersøgelsen, og et computerforbedret spørgeskema administreret under et hjemmeinterview. HDM-bureauer fra 5 stater (AR, CA, IA, NJ, SC) vil blive udvalgt målrettet på grund af de forskellige karakteristika for deres HDM-klienter, som identificeret i en analyse af HDM-modtagere efter stat. Den målrettede prøve af stater inkluderede mindst én fra hver af fire af kvintilerne på MOWA Senior Hunger state-ranglisten, med to stater fra den øverste kvintil. De valgte stater repræsenterede også et interval med hensyn til procentdelen af minoritetsklienter, med nogle høje i minoritetsbefolkningen og en ekstremt lav. Endelig varierede procentdelen af kunder i landdistrikterne fra nul i én stat til næsten 70 % i en anden. Hvert af de fem agenturer vil blive bedt om at udfylde undersøgelsesprotokollen med 200 kvalificerede HDM-modtagere udtrukket fra dem på deres klientruller ved hjælp af tilfældige tal genereret af undersøgelsens efterforskere. Prøveudtagning med udskiftning vil blive udført, indtil en kvote på 200 deltagere er nået, eller der kan ikke rekrutteres yderligere kvalificerede kunder inden for hvert bureau. En målrettet stikprøvestørrelse på 200 deltagere for hvert bureau blev udvalgt til at give en samlet målprøve på 1000 HDM-modtagere for at give en samlet prøveudtagningsfejlrate på ± 3,1 %, baseret på en population på cirka 900.000 HDM-modtagere.
Studiet:
- Vil analysere fødevaresikkerheden og kvalitetsparametrene for hjemmefødevaremiljøer for hjemmebundne ældre modtagere af HDM og identificere specifikke barrierer for sikker fødevarehåndtering og -opbevaring af medlemmer af denne højrisikopopulation. Køle-/fryseskabets temperaturer vil blive målt og justeret efter behov for at sikre, at nedkølede fødevarer opbevares under 40 °F og frosne fødevarer under 0 °F.
- Vil estimere næringsstoftætheden og tilstrækkeligheden af hjemmefødevareforsyningen til hjemmebundne ældre modtagere af HDM og identificere ernæringsområder, hvor denne population er mangelfuld og vil drage fordel af specifik supplerende ernæring, især i nødsituationer.
- To interventioner vil blive foreslået for at forbedre fødevaresikkerheden hos hjembundne ældre modtagere af HDM: implementering af dateringsetiketter på måltiderne for at hjælpe med at forhindre indtagelse af potentielt usikre ældre fødevarer; kontrol af køle-/fryseskabstemperaturer for at bestemme temperaturoverholdelse/vedligeholdelse, med justering foretaget efter behov.
- Vil samarbejde med ernæringseksperter, nødberedskabseksperter, MOWA og Extension Disaster Education Network (EDEN) for at udvikle og formidle anbefalinger om fødevaresikkerhed og nødberedskab, der er specielt skræddersyet til behovene hos de hjembundne ældre. Der vil blive udviklet materialer til nødhjælpsleverandører, madleverandører til hjemmelevering og de ældre selv.
