Verbetering van de veiligheid en toereikendheid van de voedingswaarde van de voedselvoorziening thuis van oudere ontvangers van thuisbezorgde maaltijden
Rutgers University zal samenwerken met de Meals on Wheels America (MOWA) en gelieerde bureaus in vijf staten om een onderzoek uit te voeren dat is ontworpen om voedselveiligheid, voeding en voorbereiding op noodsituaties te verbeteren onder aan huis gebonden ouderen die thuisbezorgde maaltijden (HDM) ontvangen. Deze bevolkingsgroep loopt een verhoogd risico op door voedsel overgedragen ziekten als gevolg van unieke fysiologische en gedragsfactoren, en is bijzonder kwetsbaar voor verstoringen in het voedselsysteem vanwege hun gebrek aan mobiliteit.
Een multi-methodische onderzoeksbenadering zal worden gebruikt met een doelgroep van 1.000 MOWA-aan huis gebonden oudere cliënten. Methoden omvatten het gebruik van een nieuwe UPC-scantechnologie die snel en uitgebreid al het voedsel in de huizen van de klanten catalogiseert, een voedselveiligheidsaudit thuis en een persoonlijk interview.
Het doel van de studie is om een beter begrip te geven van de unieke bedreigingen voor de voedselveiligheid voor deze risicopopulatie, om gemakkelijke, kosteneffectieve manieren voor te stellen om bekende risicofactoren voor de voedselveiligheid te verminderen, en om duidelijke richtlijnen te geven over de hoeveelheid en soorten voedsel deze bevolking het meest nodig heeft in noodsituaties. Verspreiding van onderzoeksbevindingen en aanbevelingen zal plaatsvinden via een partnerschap met EDEN en MOWA, die beide nationale kiesdistricten hebben die klaar staan om de aanbevelingen op te volgen.

Oudere aan huis gebonden ontvangers van thuisbezorgde maaltijden (HDM) lopen een verhoogd risico op door voedsel overgedragen ziekten en zijn bijzonder kwetsbaar voor verstoringen die de levering of veilige consumptie van voedsel kunnen verhinderen. Bovendien is er veel variabiliteit in de cognitieve en fysieke capaciteiten van degenen binnen deze populatie, evenals een scala aan toegang tot financiële middelen en sociale steun die zowel individueel als geografisch kan variëren. Bovendien zijn de instanties die deze bevolking bedienen sterk afhankelijk van de tijd en donaties van vrijwilligers om hun belangrijke werk uit te voeren. Om ervoor te zorgen dat ze worden uitgevoerd, moeten interventies die zijn ontworpen om voedselveiligheidsproblemen binnen deze populatie aan te pakken, relatief eenvoudig, efficiënt en kosteneffectief zijn.
Om deze problemen aan te pakken, zal een multi-methodische onderzoeksbenadering worden gebruikt, waarbij Universal Product Code (UPC)-scantechnologie wordt geïntegreerd in combinatie met een dynamisch groeiende database van voedingsmiddelen en hun bijbehorende voedingsparameters om de voedselvoorziening in huis, een bestaand voedselaanbod voor thuis, te catalogiseren. veiligheidsauditformulier, aangepast voor studiegebruik, en een computerondersteunde vragenlijst die tijdens een interview thuis wordt afgenomen. Dit project zal een nuttig inzicht verschaffen in de voedings- en voedselveiligheidslacunes in deze risicopopulatie, wat het ontwerpen en testen van interventies mogelijk zal maken om de voedselveiligheid van zowel de HDM als het voedsel dat thuis beschikbaar is voor aan huis gebonden ouderen te verbeteren. . Het zal ook duidelijke en nuttige richtlijnen geven over de hoeveelheid en soorten voedsel die deze bevolking het meest nodig heeft in noodsituaties. Verspreiding van de resulterende onderzoeksresultaten en aanbevelingen zal worden bereikt door middel van een partnerschap met het Extension Disaster Education Network (EDEN) en MOWA, die beide nationale kiesdistricten hebben die klaar staan om gevolg te geven aan de aanbevelingen.
HDM-bureaus uit 5 staten (AR, CA, IA, NJ, SC) zullen doelbewust worden geselecteerd vanwege de uiteenlopende kenmerken van hun HDM-klanten, zoals geïdentificeerd in een analyse van HDM-ontvangers per staat. De doelgerichte steekproef van staten omvatte ten minste één van elk van de vier kwintielen op de ranglijst van MOWA Senior Hunger-staten, met twee staten uit het bovenste kwintiel. De gekozen staten vertegenwoordigden ook een bereik in termen van het percentage klanten uit minderheidsgroepen, met een aantal hoog in de minderheidsbevolking en een extreem laag. Ten slotte varieerde het percentage landelijke klanten van nul in de ene staat tot bijna 70% in een andere. Elk van de vijf bureaus zal worden gevraagd om het onderzoeksprotocol in te vullen met 200 in aanmerking komende HDM-ontvangers die worden getrokken uit degenen op hun klantenlijst met behulp van willekeurige getallen die door de onderzoeksonderzoekers zijn gegenereerd. Steekproeven met vervanging worden uitgevoerd totdat een quotum van 200 deelnemers is bereikt, of er binnen elk bureau geen extra in aanmerking komende klanten kunnen worden aangeworven. Een gerichte steekproefomvang van 200 deelnemers voor elk bureau werd geselecteerd om een algemene doelsteekproef van 1000 HDM-ontvangers te verkrijgen om een algemeen steekproeffoutpercentage van ± 3,1% op te leveren, gebaseerd op een populatie van ongeveer 900.000 HDM-ontvangers.
De studie:
- Zal de voedselveiligheids- en kwaliteitsparameters analyseren van de thuisvoedselomgevingen van aan huis gebonden oudere ontvangers van HDM, en specifieke belemmeringen identificeren voor veilige voedselbehandeling en opslagpraktijken door leden van deze populatie met een hoog risico. De temperatuur van de koelkast/vriezer wordt gemeten en indien nodig aangepast om ervoor te zorgen dat gekoelde levensmiddelen worden bewaard onder 40 °F en diepvriesproducten onder 0 °F.
- Zal de nutriëntendichtheid en toereikendheid van de voedselvoorziening thuis van aan huis gebonden oudere ontvangers van HDM schatten, en voedingsgebieden identificeren waarin deze populatie een tekort heeft en baat zou hebben bij specifieke aanvullende voeding, vooral tijdens noodsituaties.
- Twee interventies zullen worden voorgesteld om de voedselveiligheid te verbeteren bij aan huis gebonden oudere ontvangers van HDM: de implementatie van dateringslabels op de maaltijden om de consumptie van mogelijk onveilig ouder voedsel te helpen voorkomen; het controleren van de temperatuur van de koelkast/vriezer om de naleving/het onderhoud van de temperatuur te bepalen, met indien nodig aanpassingen.
- Zal samenwerken met voedingsdeskundigen, experts op het gebied van paraatheid bij noodsituaties, MOWA en het Extension Disaster Education Network (EDEN) om aanbevelingen voor voedselveiligheid en voorbereiding op noodsituaties te ontwikkelen en te verspreiden die specifiek zijn toegesneden op de behoeften van aan huis gebonden ouderen. Er worden materialen ontwikkeld voor noodvoedselaanbieders, thuisbezorgers en ouderen zelf.
Rutgers zal samenwerken met de Meals on Wheels America (MOWA) en geselecteerde aan MOWA gelieerde bureaus in vijf staten om een diepgaande studie uit te voeren van deze kwetsbare bevolkingsgroep, bedoeld om de voeding en veiligheid te verbeteren van voedsel dat wordt geconsumeerd door aan huis gebonden oudere ontvangers van HDM, en om het voedselverdedigingssysteem van het land versterken door de kritieke voedingsbehoeften van deze bevolking te identificeren en aan te pakken, vooral in tijden van nood.
