Beschreibung
Einschlusskriterien:
- Alter zwischen 18 - 70 Jahren
- Karnofsky-Status ≥ 70 %
- Diagnose des multiplen Myeloms
- Innerhalb von 9 Monaten nach Beginn der Induktionschemotherapie und kein Hinweis auf Rückfall oder Progression.
- Verfügbarkeit kryokonservierter peripherer Blutstammzellen mit einer CD34-Dosis von mindestens 2x106/kg.
Ausschlusskriterien:
- Schlechte Herzfunktion: linksventrikuläre Ejektionsfraktion <40 %
- Schlechte Lungenfunktion: FEV1, FVC oder DLCO < 40 % vorhergesagt
- Schlechte Leberfunktion: Bilirubin > 2,5 mg/dl (nicht aufgrund von Hämolyse, Gilbert-Krankheit oder primärer Malignität), AST/ALT > 3X ULN
- Eingeschränkte Nierenfunktion: Kreatinin > 2,0 mg/dl oder Kreatinin-Clearance < 40 ml/min (berechnete Kreatinin-Clearance ist zulässig)
- Anhaltende oder aktive systemische Infektion, aktive Hepatitis-B- oder -C-Virusinfektion oder bekanntermaßen positiv auf das humane Immundefizienzvirus (HIV).
- Frauen im gebärfähigen Alter, die derzeit schwanger sind oder die keine angemessene Empfängnisverhütung anwenden
- Patienten, die an einer schwächenden medizinischen oder psychiatrischen Erkrankung leiden, die ihre Einwilligung nach Aufklärung oder eine optimale Behandlung und Nachsorge ausschließen würde.

Abschätzung der Reaktion am Tag 100 nach der Transplantation (CR-Rate)

Unter Verwendung der IMWG-Kriterien: PR (partielle Reaktion), angegeben als >50 %ige Reduktion des Serum-M-Proteins und Reduktion des 24-Stunden-M-Proteins im Urin um >90 % oder auf <200 mg/24 h; VgPR (sehr gute partielle Reaktion), festgestellt als Serum- und Urin-M-Protein, nachweisbar durch Immunfixierung, aber nicht durch Elektrophorese, oder >90 % Reduktion des Serum-M-Proteins plus Urin-M-Protein-Spiegel <100 mg/24 Stunden; CR (vollständige Reaktion), festgestellt als negative Immunfixierung im Serum und Urin und Verschwinden aller Weichteilplasmozytome und <5 % der Plasmazellen im Knochenmark; sCR (stringente vollständige Reaktion), angegeben als oben definierte CR plus normalem FLC-Verhältnis und Abwesenheit klonaler Zellen im Knochenmark gemäß Immunhistochemie oder Immunfluoreszenz.
