Beschrijving
Inclusiecriteria:
- Leeftijd tussen 18 - 70 jaar
- Karnofsky-status ≥ 70%
- Diagnose van multipel myeloom
- Binnen 9 maanden na aanvang van inductiechemotherapie en geen bewijs van terugval of progressie.
- Beschikbaarheid van ingevroren perifere bloedstamcellen met een dosis CD34 van minimaal 2x106/kg.
Uitsluitingscriteria:
- Slechte hartfunctie: linkerventrikelejectiefractie <40%
- Slechte longfunctie: FEV1, FVC of DLCO <40% voorspeld
- Slechte leverfunctie: bilirubine >2,5 mg/dl (niet door hemolyse, Gilbert's of primaire maligniteit), ASAT/ALAT > 3X ULN
- Slechte nierfunctie: creatinine > 2,0 mg/dl of creatinineklaring < 40 ml/min (berekende creatinineklaring is toegestaan)
- Aanhoudende of actieve systemische infectie, actieve hepatitis B- of C-virusinfectie of bekend humaan immunodeficiëntievirus (HIV) positief.
- Vrouwen in de vruchtbare leeftijd die momenteel zwanger zijn of die geen adequate anticonceptie toepassen
- Patiënten met een slopende medische of psychiatrische ziekte waardoor ze geen geïnformeerde toestemming kunnen geven of optimale behandeling en follow-up kunnen krijgen.

Om de respons op dag 100 na transplantatie te schatten (CR-snelheid)

Met behulp van IMWG-criteria: PR (partiële respons) genoteerd als >50% reductie van serum M-proteïne en reductie in 24-uurs urine M-proteïne met >90% of tot <200mg/24h; VgPR (zeer goede gedeeltelijke respons) waargenomen als serum en urine M-proteïne detecteerbaar door immunofixatie maar niet door elektroforese of >90% verlaging van serum M-proteïne plus urine M-proteïne niveau <100 mg/24 uur; CR (volledige respons) genoteerd als negatieve immunofixatie op het serum en urine en verdwijning van alle weke delen plasmacytomen en <5% plasmacellen in het beenmerg; sCR (stringente volledige respons) genoteerd als CR hierboven gedefinieerd plus normale FLC-ratio en afwezigheid van klonale cellen in het beenmerg door immunohistochemie of immunofluorescentie.
