Einschlusskriterien:
Gesunde Kontrollen:
- Eine Person ist als Kontrollperson geeignet, wenn ihr Alter 18 Jahre oder älter ist.
Myositis-Remissions-Kontrollgruppe
- Eindeutige oder wahrscheinliche PM oder DM nach den Kriterien von Bohan und Peter.
- PM-Patienten müssen entweder einen Myositis-assoziierten Autoantikörper besitzen oder sich einer Beurteilung zur Bestätigung der PM-Diagnose durch Konsens von zwei Experten (Aggarwal oder Oddis) unterziehen, um sicherzustellen, dass Nicht-PM-Patienten nicht aufgenommen werden. Dieser Schritt ist notwendig, da es wohlbekannte Nachahmer von PM gibt.
- Das Alter ist größer oder gleich 18 Jahre
- Remission der Myositis, definiert als Myositis Disease Global Activity Score <1 auf dem MDAAT und keine neue immunsuppressive oder Glukokortikoidtherapie oder Dosisänderung innerhalb eines Jahres.
Ausschlusskriterien:

- Eine bestehende Diagnose einer CTD
- Ein potenziell geschwächter Zustand des Immunsystems, z. B. Behandlung mit Immunsuppressiva oder Anti-Abstoßungs-Medikamenten oder Diagnose einer Immunschwächekrankheit
Myositis-Remissions-Kontrollgruppe:
- Juvenile DM oder PM, Myositis in Überlappung mit einer anderen Bindegewebserkrankung, krebsassoziierte Myositis, Einschlusskörperchen-Myositis oder jede andere nicht immunvermittelte Myopathie.
- Schwerer Muskelschaden, definiert als ein globaler Ausgangswert für Muskelschäden auf dem MDI (Myositis Damage Index) von mehr als oder gleich fünf Zentimetern auf einer Zehn-Zentimeter-VAS.
- Patienten mit bösartigen Tumoren innerhalb von drei Jahren vor dem Screening (ausgenommen Basalzellkrebs oder Plattenepithelkarzinom der Haut).
- Unkontrollierter Diabetes, Leber- oder Nierenerkrankung.

Myositis in Remission
Patienten, die sich mit ihrer Myositis-Diagnose in Remission befinden.
|
Zehn Patienten (vier mit Polymyositis, sechs mit Dermatomyositis) aus unserer Datenbank, die im Myositis-Zentrum betreut werden.
Wir werden Serum, PBMCs und RNA-Proben sammeln, die zu Studienbeginn und nach sechs Monaten entnommen werden.
Remission ist definiert als ein globaler Myositis-Krankheitsaktivitäts-Score kleiner oder gleich 1 bei den MDAAT-Beurteilungen ohne Verwendung neuer immunsuppressiver Medikamente oder Glukokortikoide und ohne Erhöhung der Dosis von beidem im vorangegangenen Jahr.

Die International Myositis Assessment & Clinical Studies (IMACS)-Definition der Verbesserung: Drei der sechs Kernsätze verbesserten sich um mehr als oder gleich zwanzig Prozent, wobei nicht mehr als zwei Kernsätze um mehr als oder gleich fünfundzwanzig Prozent gemessen wurden .

Dieses Kriterium ergibt eine Punktzahl für kontinuierliche Verbesserung, die leichter auf die individuelle Reaktion bei Patienten extrapoliert werden kann, was eine Korrelation mit Basislinien- und longitudinalen immunologischen Markern bei diesen Patienten ermöglicht.
