Beschreibung
Einschlusskriterien:
- Patienten mit einer klinischen Diagnose von SLE gemäß den Klassifizierungskriterien des American College of Rheumatology (ACR) von 1997 und einer klinisch aktiven Erkrankung.
- Patienten mit guter Compliance unterschreiben vor dem Test die Einverständniserklärung.
- Patienten, die eine konventionelle Behandlung für SLE erhalten haben, und Art und Dosis der Behandlungsmedikamente sind seit mindestens 30 Tagen stabil.
- Patienten mit einem positiven Testergebnis auf antinukleäre Antikörper und einem SELENA-SLEDAI-Score ≥8 beim Screening. Bei niedrigem Komplement und/oder positivem Anti-dsDNA-Antikörper kann der SELENA-SLEDAI-Score als ≥ 6 Punkte definiert werden.
Ausschlusskriterien:
- Patienten mit schwerer Lupusnephritis, definiert als Protein im Urin > 6 g/24 Stunden oder Serum-Kreatinin > 221 μmol/L innerhalb der letzten 2 Monate oder die eine Hämodialyse benötigen.
- Patienten mit einer durch SLE verursachten oder nicht durch SLE verursachten Erkrankung des zentralen Nervensystems innerhalb der letzten 2 Monate.
- Patienten mit schweren Erkrankungen des Blutes, wichtiger Organe wie Herz, Leber, Magen-Darm-Trakt und endokrines System, die nicht mit SLE zusammenhängen.
- Patienten, die Prednison ≥ 100 mg/d über 14 Tage anwenden oder Plasmaersatz erhalten und innerhalb des letzten 1 Monats an einer aktiven Infektion leiden.
- Patienten, die in den letzten 12 Monaten andere zielgerichtete Wirkstoffe erhalten haben.

Experimental: Experimentelle Gruppe
Das Behandlungsschema besteht aus vier Medikamenten, einem Glukokortikoid plus Telitacicept plus Hydroxychloroquin plus einem Immunsuppressor. Prednison (30 mg, Qd) oder Methylprednisolon (24 mg, Qd) plus Telitacicept (160 mg, Qw) plus Hydroxychloroquin (0,2 g, Qd) plus Cyclophosphamid (0,8 g, Qd) Qm) oder Mycophenolat Mofetil (0,5 g, 2-mal täglich) oder Tacrolimus (1 mg, 2-mal täglich) Die obige Behandlung wird 24 Wochen lang fortgesetzt. |
