Un Estudio de TAK-226 para la Anemia Dependiente de Transfusiones en Pacientes Japoneses con Síndromes Mielodisplásicos de Riesgo Inferior
Estudio de fase 2, multicéntrico, abierto y de un solo brazo para evaluar la eficacia y seguridad de TAK-226 en la anemia dependiente de transfusiones en pacientes japoneses con síndromes mielodisplásicos de riesgo muy bajo, bajo o intermedio
El objetivo principal del estudio es evaluar cómo TAK-226 mejora los síntomas de la anemia dependiente de transfusiones en pacientes japoneses con síndromes mielodisplásicos de menor riesgo.
El estudio consta del Período de Selección (hasta 6 semanas), Período de Tratamiento, Período de Seguimiento de Seguridad (8 semanas) y Período de Seguimiento a Largo Plazo (5 años desde la primera dosis del fármaco del estudio o 3 años después de la última dosis, lo que sea más largo).
Los participantes de este estudio recibirán TAK-226 durante el Período de Tratamiento. Posteriormente, se controlará a los participantes para detectar efectos secundarios relacionados con el tratamiento del estudio durante el Período de Seguimiento de Seguridad y el Período de Seguimiento a Largo Plazo. La duración aproximada de la participación de un participante es de hasta aproximadamente 6 años.
Durante el período del estudio, los participantes visitarán la clínica/hospital del estudio varias veces según el calendario del estudio. Durante el Período de Tratamiento, los participantes acudirán a la clínica/hospital aproximadamente cada dos a cuatro semanas.

Descripción
Criterios de inclusión:
- Los participantes o su representante legal autorizado deben estar dispuestos y ser capaces de firmar el formulario de consentimiento informado (ICF) y de cumplir con los requisitos del protocolo.
- Participante adulto japonés de sexo masculino o femenino con edad ≥18 años en el momento de firmar el consentimiento informado.
Diagnóstico de síndrome mielodisplásico (SMD) con o sin sideroblastos en anillo (RS) (según se determine en un aspirado de médula ósea evaluable recogido durante la selección para confirmar el diagnóstico) de acuerdo con la clasificación de la OMS de 2016 que cumpla con la clasificación IPSS-R de SMD de riesgo muy bajo, bajo o intermedio.
Nota: Debido al impacto esperado de las transfusiones, los valores de hemoglobina (Hgb) de muestras de sangre recogidas dentro de los 14 días posteriores a una transfusión de glóbulos rojos y el recuento de plaquetas obtenido dentro de los 7 días posteriores a una transfusión de plaquetas no pueden utilizarse para evaluar el IPSS-R para la elegibilidad.
Dependencia transfusional evaluada en las 16 semanas inmediatamente anteriores al reclutamiento en dos bloques de 8 semanas clasificados como:
a. Transfusión baja (LTB), definida como 4 a 7 unidades de glóbulos rojos por 16 semanas; o b. Transfusión alta (HTB), definida como ≥8 unidades de glóbulos rojos por 16 semanas; y c. Para todos los participantes: i. Solo se contabilizan para la elegibilidad los eventos de transfusión con una Hgb pretransfusión <10 g/dL; ii. Al menos 1 evento de transfusión en cada bloque de 8 semanas y un mínimo de 2 eventos de transfusión separados por ≥7 días dentro del período de 16 semanas inmediatamente anterior al reclutamiento; y iii. No puede haber un período consecutivo de 56 días libre de transfusión de glóbulos rojos durante el período de 16 semanas inmediatamente anterior al reclutamiento.
Nota: Solo se contabilizarán para la clasificación de participantes LTB o HTB las transfusiones por la enfermedad en estudio. No se consideran las transfusiones por enfermedades intercurrentes (hemorragia, procedimiento quirúrgico, infección, etc.).
Refractario o intolerante a un tratamiento previo con agente estimulante de la eritropoyesis (AEE) (interrumpido ≥4 semanas antes del reclutamiento), o con baja probabilidad de respuesta al tratamiento con AEE, definido como sigue:
a. Refractario a un tratamiento previo con AEE: documentación de falta de respuesta o de una respuesta que ya no se mantenía con un régimen previo que contenía AEE, ya sea como agente único o en combinación (p. ej., con factor estimulante de colonias de granulocitos [G-CSF]); el régimen con AEE debe haber sido: i. Eritropoyetina humana recombinante ≥40.000 UI/semana durante ≥8 dosis o equivalente; o ii. Darbepoetina alfa ≥500 mcrg cada 3 semanas durante ≥4 dosis o equivalente. b. Intolerante a un tratamiento previo con AEE: documentación de la interrupción de un régimen previo que contenía AEE, ya sea como agente único o en combinación (p. ej., con G-CSF), en cualquier momento tras su introducción debido a intolerancia o a un evento adverso (EA).
c. Baja probabilidad de respuesta al tratamiento con AEE: baja probabilidad de respuesta al AEE basada en un nivel sérico endógeno de EPO >200 U/L.
Nota: Debido al impacto esperado de las transfusiones en los niveles de EPO, las muestras de sangre recogidas dentro de los 14 días posteriores a una transfusión de glóbulos rojos o dentro de los 7 días posteriores a una transfusión de plaquetas no pueden utilizarse para evaluar el nivel sérico de EPO para la elegibilidad.
- Menos del 5% de blastos en un aspirado de médula ósea evaluable recogido durante la selección.
- Estado funcional ECOG de 0 a 2.
Mujeres en edad fértil (WOCBP), definidas como mujeres sexualmente maduras que no se han sometido a esterilización quirúrgica o que no han estado en menopausia natural durante al menos 12 meses consecutivos deben:
- Aceptar utilizar 1 método anticonceptivo altamente eficaz y 1 método adicional eficaz (de barrera) al mismo tiempo, desde el momento de firmar el consentimiento informado hasta 60 días después de la última dosis del fármaco en estudio; o
- Aceptar practicar la abstinencia verdadera, cuando esto esté en línea con el estilo de vida preferido y habitual del participante. (La abstinencia periódica [p. ej., métodos de calendario, ovulación, sintotérmico, postovulación], la retirada, solo espermicidas y la amenorrea por lactancia no son métodos anticonceptivos aceptables).
Los participantes de sexo masculino deben, incluso si están esterilizados quirúrgicamente (es decir, estado postvasectomía):
- Aceptar practicar un método anticonceptivo de barrera eficaz desde el momento de firmar el consentimiento informado hasta 60 días después de la última dosis del fármaco en estudio; o
- Aceptar practicar la abstinencia verdadera, cuando esto esté en línea con el estilo de vida preferido y habitual del participante. (La abstinencia periódica [p. ej., métodos de calendario, ovulación, sintotérmico, postovulación], la retirada, solo espermicidas y la amenorrea por lactancia no son métodos anticonceptivos aceptables).
Criterios de exclusión:
Antecedentes médicos
- SMD con del(5q) o SMD relacionado con tratamiento (secundario).
- Anemia debida a cualquier otra causa conocida (p. ej., talasemia, anemia hemolítica, eventos hemorrágicos o deficiencia de hierro, B12 y/o folato).
- Haber recibido transfusión de glóbulos rojos por cualquier motivo distinto al SMD subyacente dentro de las 16 semanas anteriores al reclutamiento.
Enfermedad cardiovascular clínicamente significativa definida como:
- Enfermedad cardíaca clase III o IV de la Asociación del Corazón de Nueva York;
- Intervalo QT corregido por Fridericia (QTcF) >500 milisegundos durante la selección;
- Presencia de hipertensión no controlada definida como presión arterial sistólica media ≥160 mm Hg o presión arterial diastólica ≥100 mm Hg durante la selección; o
- Arritmia no controlada, infarto de miocardio o angina inestable dentro de los 6 meses anteriores a la selección.
- Fracción de eyección conocida <35%, confirmada por un ecocardiograma local realizado durante la selección, o por un ecocardiograma realizado previamente si se recogió dentro de los 6 meses anteriores a la selección.
- Accidente cerebrovascular, trombosis venosa profunda o embolia pulmonar dentro de los 6 meses anteriores a la selección.
- Cualquier antecedente conocido de leucemia mieloide aguda (LMA).
Antecedentes previos de malignidades, distintas del SMD, a menos que el participante haya estado libre de la enfermedad (incluida la finalización de cualquier tratamiento, incluido el de mantenimiento, por la malignidad previa) durante ≥5 años. Sin embargo, se permiten participantes con antecedentes o diagnóstico concurrente de las siguientes condiciones si no requieren terapia sistémica:
- Carcinoma basocelular o de células escamosas de la piel;
- Carcinoma in situ de cuello uterino;
- Carcinoma in situ de mama; y/o
- Hallazgo histológico incidental de cáncer de próstata (T1a o T1b según el sistema de estadificación clínica tumor, ganglio, metástasis [TNM]).
- Antecedentes de trasplante de órgano sólido o de médula ósea.
- Infección activa que requiera antibióticos intravenosos dentro de los 28 días o antibióticos orales dentro de los 14 días anteriores al reclutamiento.
- Antecedentes de o infección activa o crónica conocida con VIH, virus de la hepatitis B (VHB) infeccioso activo o virus de la hepatitis C (VHC) infeccioso activo. Los participantes que sean positivos para el antígeno de superficie de la hepatitis B (HBsAg), el anticuerpo contra el core de la hepatitis B (HBcAb) o el anticuerpo de superficie de la hepatitis B (HBsAb) pueden ser elegibles para el reclutamiento si su carga viral de hepatitis B está por debajo del límite de detección. Los participantes que sean positivos para anticuerpos contra el virus de la hepatitis C (HCVAb) pueden ser reclutados si su carga viral de hepatitis C está por debajo del límite de detección.
- Índice de masa corporal ≥40 kg/m².
- Cirugía mayor dentro de los 28 días anteriores al reclutamiento.
Antecedentes de alergia/anafilaxia a los excipientes del medicamento en investigación (IMP) (consultar el IB actual de elritercept [TAK 226] para una lista de excipientes) o a proteínas recombinantes.
Antecedentes de tratamiento
- Uso previo de TAK 226, luspatercept, imetelstat o sotatercept.
- Uso previo de agentes hipometilantes (AHM), inhibidores de la isocitrato deshidrogenasa, lenalidomida o terapia inmunosupresora administrada para el tratamiento del SMD.
- Terapia con quelantes de hierro iniciada dentro de las 8 semanas anteriores al reclutamiento. Se permiten participantes con dosis estables de terapia con quelantes de hierro durante ≥8 semanas.
- Terapia con vitamina B12 o folato iniciada dentro de las 4 semanas anteriores al reclutamiento. Se permiten participantes con dosis de reemplazo estables durante ≥4 semanas y sin deficiencia concurrente en curso de vitamina B12 o folato.
- Uso de andrógenos dentro de las 8 semanas anteriores al reclutamiento. Se permiten participantes con dosificación estable de andrógenos para hipogonadismo durante ≥8 semanas.
- Uso de corticosteroides en dosis altas dentro de las 4 semanas anteriores al reclutamiento. Se permiten participantes con dosis crónicas estables de prednisona/prednisolona ≤10 mg/día o equivalente de corticosteroides durante ≥4 semanas.
- Tratamiento con cualquier fármaco en investigación dentro de los 28 días anteriores a la selección o, si se conoce la semivida del producto, dentro de 5 veces la semivida antes de la selección, lo que sea más largo.
- Participación en curso en otro estudio clínico intervencionista. Exclusiones de laboratorio (durante la selección)
- Nivel sérico de EPO >500 U/L. Nota: Debido al impacto esperado de las transfusiones en los niveles de EPO, los resultados de laboratorio de muestras de sangre recogidas dentro de los 14 días posteriores a una transfusión de glóbulos rojos no pueden utilizarse para evaluar el nivel sérico de EPO para la elegibilidad.
- Recuento de plaquetas ≥450×10³/mcrL o ≤25×10³/mcrL. Nota: Debido al impacto esperado de las transfusiones, los resultados de laboratorio de muestras de sangre recogidas dentro de los 7 días posteriores a una transfusión de plaquetas no pueden utilizarse para evaluar el recuento de plaquetas para la elegibilidad.
- Recuento absoluto de neutrófilos ≤500/mcrL
- AST o ALT sérica ≥3×el límite superior normal (ULN).
- Bilirrubina total ≥2×ULN a menos que sea atribuible al síndrome de Gilbert.
- Ferritina ≤50 mcrg/L.
- Folato ≤2,0 ng/mL.
- Vitamina B12 ≤200 pg/mL.

Miscelánea
- Mujer embarazada o en periodo de lactancia. Nota: Se excluyen del estudio los participantes que puedan estar en una etapa muy temprana del embarazo según la entrevista del médico con una prueba de embarazo negativa. Las participantes que estén en periodo de lactancia serán elegibles si interrumpen la lactancia materna desde antes de la primera dosis del fármaco en estudio hasta 60 días después de la última dosis del fármaco en estudio.
- Cualquier otra condición no especificada anteriormente que, en opinión del investigador, impediría la participación del participante en el estudio o podría confundir la interpretación de los datos del estudio.
