Kuvaus
Sisällyttämiskriteerit:
- Tutkittavien halu ja kyky antaa tietoinen suostumus ja allekirjoittaa sairausvakuutuksen siirrettävyys ja vastuullisuuslain (HIPAA) lomake. Tutkimuskohtainen tietoinen suostumus ja HIPAA-lomake on hankittava kirjallisesti ennen tutkimustoimenpiteiden aloittamista.
- 4–15 lievää tai kohtalaista kliinisesti varmistettua AK-leesiota Olsenin mukaan joko raajoissa tai kaulassa/vartalossa, joiden halkaisija on ≥ 4 mm ja joiden on oltava läsnä hoitoalueella (määritelty AK-kohdeleesioiksi). Lisäksi hoitoalueella voi esiintyä muita kuin kohteena olevia AK-leesioita, mukaan lukien enintään kaksi vakavaa AK-leesiota, joiden koko on ≥ 4 mm. Jokaisesta vakavasta AK-vauriosta (≥ 4 mm) on otettava biopsia diagnoosin vahvistamiseksi. Hoitokenttä (jatkuvana tai useana laastarina) yhteensä n. joko 80 cm², 160 cm² tai 240 cm2 on oltava yhden BF-RhodoLED® XL:n tehokkaan valaistusalueen sisällä, mutta se voi vaatia jopa kolme valaistusta BF-RhodoLED®:llä. Kaikkien hoitoalueella sijaitsevien AK-kohdevaurioiden ja soveltuvin osin vakavien AK-leesioiden, joiden koko on ≥ 4 mm, tulee olla selvästi erotettavissa, ilman rajoituksia leesioiden väliselle etäisyydelle, ja niiden tulee olla vähintään 1 cm:n etäisyydellä hoitokentän rajasta.
- Kaikki sukupuolet, ≥ 18 vuotta.
- Halukkuus tehdä 2 mm:n lyöntibiopsia jokaisesta (enintään kahdesta) vakavasta AK-leesiosta, jonka koko on ≥ 4 mm, tarvittaessa seulontakäynnillä.
- Halukkuus ja kyky noudattaa tutkimusmenettelyjä, erityisesti halukkuus saada jopa 2 PDT:tä noin 12 viikon sisällä.
- Koehenkilöt, joiden yleinen terveys on hyvä tai joilla on kliinisesti vakaat sairaudet, voidaan ottaa mukaan tutkimukseen.
- Halukkuus lopettaa kosteusvoiteiden ja muiden ei-lääketieteellisten paikallisten hoitojen käyttö hoitoalueella vähintään 24 tuntia ennen seuraavaa kliinistä käyntiä.
- Suostuminen välttämään laajaa auringonottoa ja solariumin tai solariumin käyttöä hoitovaiheen aikana.
- Naispuolisille henkilöille, joilla on lisääntymiskyky: Negatiivinen seerumin raskaustesti.
- Naispuolisille koehenkilöille, joilla on lisääntymiskyky: Tehokas ehkäisy seulontakäynnillä ja koko tutkimuksen hoitovaiheen ajan (käyntiin 4 tai käyntiin 6).
Poissulkemiskriteerit:
- Kaikki tiedossa oleva yliherkkyys ALA:lle, porfyriineille tai BF-200 ALA:n apuaineille.
- Aiemmat soija- tai maapähkinäallergiat.
- Auringonpolttama tai muut mahdolliset hämmentävät ihosairaudet (esim. haavat, ärsytykset, verenvuoto tai ihotulehdukset) hoitoalueen sisällä tai sen välittömässä läheisyydessä (< 2 cm:n etäisyys).
Kliinisesti merkittävät (cs) sairaudet, jotka vaikeuttavat tutkimussuunnitelman toteuttamista tai tutkimustulosten tulkintaa tai heikentävät tutkittavan turvallisuutta, kuten:
- Valodermatoosien tai porfyrian esiintyminen
- Metastaattinen kasvain tai kasvain, jolla on suuri metastaasin todennäköisyys
- Infiltroiva ihoneoplasia (epäilty tai tiedossa)
- Epästabiili sydän- ja verisuonisairaus (New York Heart Associationin luokka III, IV)
- Epästabiili hematologinen (mukaan lukien myelodysplastinen oireyhtymä), maksan, munuaisten, neurologinen tai endokriininen tila
- Epävakaa kollageeni-verisuonitila
- Epävakaa maha-suolikanavan tila
- Immunosuppressiivinen tila
- Kliinisesti merkittävä perinnöllinen tai hankittu hyytymishäiriö
- Atooppisen ihotulehduksen, Bowenin taudin (BD), tyvisolusyövän (BCC), ekseeman, psoriaasin, ruusufinnien, okasolusyövän (SCC), muiden pahanlaatuisten tai hyvänlaatuisten kasvainten kliininen diagnoosi sisällä tai sen läheisyydessä (< 2 cm etäisyys) hoitokentälle.
- Vahvaa keinotekoista pigmentaatiota (esim. tatuointeja) tai muita poikkeavuuksia, jotka voivat vaikuttaa leesion arviointiin tai valon tunkeutumiseen hoitoalueella.
- Mikä tahansa fysioterapia, kuten kryoterapia, laserhoito, elektrodessikaatio, mikrodermabrasio, leesioiden kirurginen poisto, kyretti tai hoito kemiallisilla kuorilla, kuten trikloorietikkahapolla, hoitokentän sisällä tai sen välittömässä läheisyydessä (< 10 cm:n etäisyys) 4 viikon sisällä ennen hoitoa. seulonta.
Mikä tahansa alla määritellyistä paikallisista hoidoista määrättyjen ajanjaksojen aikana ennen seulontaa:

- Lääkkeen paikallisen antamisen aloittaminen hyperisiinin tai muiden fototoksisten tai valoallergisten potentiaalisten lääkkeiden kanssa hoitoalueen sisällä tai sen välittömässä läheisyydessä (< 10 cm etäisyydellä) 4 viikon sisällä. Koehenkilöt voivat kuitenkin olla kelvollisia, jos tällaista lääkitystä on käytetty yli 4 viikkoa ennen seulontaa ilman näyttöä todellisesta fototoksisesta/valoallergisesta reaktiosta.
Kaikki systeemisten hoitojen käyttö määrättyjen ajanjaksojen aikana ennen seulontaa:
- Sytostaattiset tai sytotoksiset lääkkeet 6 kuukauden sisällä.
- Immunosuppressiiviset hoidot tai ALA- tai ALA-esterit (esim. MAL) 12 viikon sisällä.
- Lääkkeet, joiden tiedetään aiheuttavan suurta elintoksisuutta 8 viikon kuluessa.
- Interferoni tai glukokortikosteroidit 6 viikon kuluessa.
- Hyperisiinin tai fototoksisten tai valoallergisten lääkkeiden ottaminen aloitetaan 8 viikon sisällä ennen seulontaa. Koehenkilöt voivat kuitenkin olla kelpoisia, jos tällaisia lääkkeitä on otettu yli 8 viikkoa ennen seulontakäyntiä ilman näyttöä todellisesta fototoksisesta/valoallergisesta reaktiosta.
- Imettävät naiset.
- Huumeiden tai alkoholin väärinkäyttöä epäillään.
- Koehenkilöt, jotka eivät todennäköisesti noudata protokollaa, esim. kyvyttömyys palata käynneille, ei todennäköisesti suorita tutkimusta tai ovat tutkijan mielestä sopimattomia.
- Tutkimuspaikan henkilöstön jäsen tai sponsorihenkilöstö, joka on suoraan mukana protokollan toteuttamisessa, tai hänen lähisukulaisensa.
- Tutkimuslääkkeen tai lääkevalmisteen vastaanotto 8 viikon sisällä ennen seulontaa tai samanaikaista osallistumista toiseen kliiniseen tutkimukseen.
Koehenkilöiden uudelleenarviointi on sallittu kerran, jos poissulkemiskriteeri 3 täyttyy ja kelpoisuus voidaan saavuttaa 4 viikon kuluessa. Uudelleenarviointi voidaan tehdä varsinaisena hoitopäivänä.

Potilaat, joilla on auringonpolttamia tai muita mahdollisesti häiritseviä ihosairauksia (esim. haavat, ärsytykset, verenvuoto tai ihotulehdukset) hoitoalueen sisällä tai sen välittömässä läheisyydessä (< 2 cm:n etäisyys). Koehenkilöiden uudelleenarviointi on sallittu kerran, jos auringonpolttaman tai muun hämmentävän iho-ongelman odotetaan paranevan 2 viikon kuluessa.
Vierailulla 4 (PDT-2)
Potilaat, joilla on auringonpolttamia tai muita mahdollisesti häiritseviä ihosairauksia (esim. haavat, ärsytykset, verenvuoto tai ihotulehdukset) hoitoalueen sisällä tai sen välittömässä läheisyydessä (< 2 cm:n etäisyys). PDT-2:n uudelleenjärjestely voidaan tehdä kerran mahdollisimman pian ratkaisun jälkeen, mutta uudelleenjärjestelyn ei tulisi ylittää 2 viikkoa.

Satisfaction with PDT treatment after the last PDT as assessed by the subject via questionnaire, 1. if they would chose PDT treatment in the future in case of recurrence or similar disease (yes/no) and 2. how they would rate the PDT treatment in comparison to other treatment modalities, if applicable (better than/ similar/ worse).

Relevant AEs include AEs or conditions affecting skin health in the treatment field which may impair proper assessment of the recurrence rate of the treated AK lesions, or other clinically relevant events at the investigator's discretion as well as any SAE that has occurred since the final visit of the treatment phase (Visit 4 or Visit 6); overall and stratified by demographics and treatment area
