Descrição
Critério de inclusão:
- Vontade e capacidade dos sujeitos de fornecer consentimento informado e assinar o formulário Health Insurance Portability and Accountability Act (HIPAA). Um consentimento informado específico do estudo e um formulário HIPAA devem ser obtidos por escrito antes de iniciar qualquer procedimento do estudo.
- 4 - 15 lesões de AK leves a moderadas clinicamente confirmadas de acordo com Olsen nas extremidades ou no pescoço/tronco com diâmetro ≥ 4 mm que devem estar presentes no campo de tratamento (definidas como lesões alvo de AK). Além disso, lesões AK não-alvo podem estar presentes no campo de tratamento, incluindo até duas lesões AK graves ≥ 4 mm. Para cada lesão grave de AK (≥ 4 mm), uma biópsia deve ser realizada para confirmação do diagnóstico. O campo de tratamento (contínuo ou em vários patches) totalizando aprox. 80 cm², 160 cm² ou 240 cm2 devem estar dentro de uma área de iluminação efetiva do BF-RhodoLED® XL, mas podem exigir até três iluminações com o BF-RhodoLED®. Todas as lesões alvo de AK e, se aplicável, lesões de AK graves ≥ 4 mm localizadas no campo de tratamento devem ser claramente distinguíveis, sem restrições quanto à distância entre as lesões, e devem ter uma distância mínima de 1 cm até a borda do campo de tratamento.
- Todos os sexos, ≥ 18 anos de idade.
- Disposição para se submeter a uma biópsia por punção de 2 mm para cada (até duas) lesão AK grave ≥ 4 mm, se aplicável, na consulta de triagem.
- Vontade e capacidade de cumprir os procedimentos do estudo, particularmente vontade de receber até 2 PDTs em aproximadamente 12 semanas.
- Indivíduos com boa saúde geral ou com condições médicas clinicamente estáveis poderão ser incluídos no estudo.
- Disposição para interromper o uso de hidratantes e quaisquer outros tratamentos tópicos não médicos dentro do campo de tratamento pelo menos 24 horas antes da próxima visita clínica.
- Aceitação de abster-se de banhos de sol prolongados e do uso de solário ou camas de bronzeamento durante a fase de tratamento.
- Para mulheres com potencial reprodutivo: teste de gravidez sérico negativo.
- Para mulheres com potencial reprodutivo: Contracepção eficaz na consulta de triagem e durante a fase de tratamento do estudo (até a Visita 4 ou Visita 6).
Critério de exclusão:
- Qualquer história conhecida de hipersensibilidade a ALA, porfirinas ou excipientes de BF-200 ALA.
- História de alergia a soja ou amendoim.
- Queimadura solar ou outras possíveis condições de confusão da pele (por exemplo, feridas, irritações, sangramento ou infecções de pele) dentro ou próximo (< 2 cm de distância) do campo de tratamento.
Condições médicas clinicamente significativas (cs) que dificultam a implementação do protocolo ou a interpretação dos resultados do estudo ou prejudicam a segurança do sujeito, como:
- Presença de fotodermatoses ou porfiria
- Tumor metastático ou tumor com alta probabilidade de metástase
- Neoplasia cutânea infiltrativa (suspeita ou conhecida)
- Doença cardiovascular instável (classe III, IV da New York Heart Association)
- Condição hematológica instável (incluindo síndrome mielodisplásica), hepática, renal, neurológica ou endócrina
- Condição colágeno-vascular instável
- Condição gastrointestinal instável
- condição imunossupressora
- Presença de defeito de coagulação hereditário ou adquirido clinicamente significativo
- Diagnóstico clínico de dermatite atópica, doença de Bowen (DB), carcinoma basocelular (CBC), eczema, psoríase, rosácea, carcinoma espinocelular (CEC), outros tumores malignos ou benignos no interior ou nas proximidades (< 2 cm de distância) para o campo de tratamento.
- Presença de forte pigmentação artificial (por exemplo, tatuagens) ou qualquer outra anormalidade que possa afetar a avaliação da lesão ou a penetração da luz no campo de tratamento.
- Qualquer fisioterapia, como crioterapia, terapia a laser, eletrodissecação, microdermoabrasão, remoção cirúrgica de lesões, curetagem ou tratamento com peelings químicos, como ácido tricloroacético, dentro ou próximo (< 10 cm de distância) do campo de tratamento dentro de 4 semanas antes da triagem.
Qualquer um dos tratamentos tópicos definidos abaixo dentro dos períodos designados antes da triagem:
- Tratamento tópico com ALA ou ésteres de ALA (por exemplo, ácido metil aminolevulínico (MAL)) dentro do campo de tratamento dentro de 3 meses.
- Tratamento tópico com drogas imunomoduladoras, citostáticas ou citotóxicas dentro ou próximo (< 10 cm de distância) do campo de tratamento dentro de 3 meses.
- Início da administração tópica de um medicamento com hipericina ou outros medicamentos com potencial fototóxico ou fotoalérgico dentro ou próximo (< 10 cm de distância) do campo de tratamento dentro de 4 semanas. Os indivíduos podem, no entanto, ser elegíveis se tal medicação for aplicada por mais de 4 semanas antes da triagem sem evidência de uma reação fototóxica/fotoalérgica real.
Qualquer uso dos tratamentos sistêmicos dentro dos períodos designados antes da triagem:
- Drogas citostáticas ou citotóxicas dentro de 6 meses.
- Terapias imunossupressoras ou ALA ou ésteres de ALA (por exemplo, MAL) dentro de 12 semanas.
- Drogas conhecidas por causar toxicidade em órgãos importantes dentro de 8 semanas.
- Interferon ou glicocorticosteróides dentro de 6 semanas.
- Início da ingestão de medicação com hipericina ou drogas com potencial fototóxico ou fotoalérgico dentro de 8 semanas antes da triagem. Os indivíduos podem, no entanto, ser elegíveis se tal medicamento foi tomado por mais de 8 semanas antes da visita de triagem sem evidência de uma reação fototóxica/fotoalérgica real.
- Mulheres que amamentam.
- Suspeita de abuso de drogas ou álcool.
- Indivíduos com pouca probabilidade de cumprir o protocolo, por exemplo, incapacidade de retornar para visitas, improvável de concluir o estudo ou inapropriado na opinião do investigador.
- Um membro da equipe do centro de estudo ou equipe do patrocinador diretamente envolvido na condução do protocolo ou um parente próximo do mesmo.
- Recebimento de qualquer medicamento experimental ou produto médico dentro de 8 semanas antes da triagem ou participação simultânea em outro estudo clínico.
A reavaliação dos participantes é permitida uma vez, caso o critério de exclusão 3 seja atendido e a elegibilidade possa ser alcançada em 4 semanas. A reavaliação pode ser feita no próprio dia do tratamento.
Critérios de exclusão do dia de dosagem:
Na Visita 2 (linha de base, PDT-1)
Indivíduos com queimaduras solares ou outras condições de pele que possam confundir (por exemplo, feridas, irritações, sangramento ou infecções de pele) dentro ou próximo (< 2 cm de distância) do campo de tratamento. A reavaliação dos indivíduos é permitida uma vez se a queimadura solar ou outras condições confusas da pele forem resolvidas dentro de 2 semanas.
Na visita 4 (PDT-2)
Indivíduos com queimaduras solares ou outras condições de pele que possam confundir (por exemplo, feridas, irritações, sangramento ou infecções de pele) dentro ou próximo (< 2 cm de distância) do campo de tratamento. O reagendamento do PDT-2 pode ser realizado uma vez na primeira possibilidade após a resolução, mas o reagendamento não deve exceder 2 semanas.

Até dois PDTs usando uma lâmpada RhodoLED (RhodoLED® XL ou BF-RhodoLED®) (Vehicle-PDT): Aplicação tópica de até 3 tubos de veículo no campo de tratamento expandido (até 240 cm²), seguida de iluminação com luz vermelha com uma lâmpada RhodoLED após 3 h de incubação da medicação do estudo sob curativo oclusivo. O PDT-1 será realizado na Visita 2. A liberação clínica será avaliada 12 semanas após o PDT-1 (Visita 4). Em caso de lesões remanescentes na Visita 4, o PDT-2 será realizado na mesma visita. |

Relevant AEs include AEs or conditions affecting skin health in the treatment field which may impair proper assessment of the recurrence rate of the treated AK lesions, or other clinically relevant events at the investigator's discretion as well as any SAE that has occurred since the final visit of the treatment phase (Visit 4 or Visit 6); overall and stratified by demographics and treatment area
