Descrizione

Criterio di inclusione:

  1. Disponibilità e capacità dei soggetti a fornire il consenso informato e firmare il modulo Health Insurance Portability and Accountability Act (HIPAA). Un consenso informato specifico per lo studio e il modulo HIPAA devono essere ottenuti per iscritto prima di iniziare qualsiasi procedura di studio.
  2. 4 - 15 lesioni AK confermate clinicamente da lievi a moderate secondo Olsen alle estremità o al collo/tronco con un diametro di ≥ 4 mm che devono essere presenti nel campo di trattamento (definite come lesioni target AK). Inoltre, nel campo di trattamento possono essere presenti lesioni AK non bersaglio, comprese fino a due lesioni AK gravi ≥ 4 mm. Per ogni grave lesione di AK (≥ 4 mm), deve essere eseguita una biopsia per confermare la diagnosi. Il campo di trattamento (continuo o in più patch) per un totale di ca. 80 cm², 160 cm² o 240 cm2 devono rientrare in un'area di illuminazione effettiva del BF-RhodoLED® XL ma possono richiedere fino a tre illuminazioni con il BF-RhodoLED®. Tutte le lesioni bersaglio della CA e, se del caso, le lesioni gravi della CA ≥ 4 mm situate nel campo di trattamento devono essere chiaramente distinguibili, senza restrizioni sulla distanza tra le lesioni, e devono avere una distanza minima di 1 cm dal bordo del campo di trattamento.
  3. Tutti i sessi, ≥ 18 anni di età.
  4. Disponibilità a sottoporsi a una biopsia del punch da 2 mm per ciascuna (fino a due) lesione grave di AK ≥ 4 mm, se applicabile, alla visita di screening.
  5. Disponibilità e capacità di rispettare le procedure dello studio, in particolare disponibilità a ricevere fino a 2 PDT entro circa 12 settimane.
  6. Soggetti con buone condizioni di salute generale o con condizioni mediche clinicamente stabili potranno essere inclusi nello studio.
  7. Disponibilità a interrompere l'uso di creme idratanti e qualsiasi altro trattamento topico non medico all'interno del campo di trattamento almeno 24 ore prima della successiva visita clinica.
  8. Accettazione di astenersi dall'abbronzatura prolungata e dall'uso di solarium o lettini abbronzanti durante la fase del trattamento.
  9. Per soggetti di sesso femminile con potenziale riproduttivo: test di gravidanza su siero negativo.
  10. Per soggetti di sesso femminile con potenziale riproduttivo: Contraccezione efficace alla visita di screening e per tutta la fase di trattamento dello studio (fino alla Visita 4 o alla Visita 6).

Criteri di esclusione:

  1. Qualsiasi storia nota di ipersensibilità ad ALA, porfirine o eccipienti di BF-200 ALA.
  2. Storia di allergia alla soia o alle arachidi.
  3. Scottature solari o altre possibili condizioni cutanee confondenti (ad es. ferite, irritazioni, sanguinamento o infezioni cutanee) all'interno o in prossimità (<2 cm di distanza) del campo di trattamento.
  4. Condizioni mediche clinicamente significative (cs) che rendono difficile l'implementazione del protocollo o l'interpretazione dei risultati dello studio o compromettono la sicurezza del soggetto, come:

    1. Presenza di fotodermatosi o porfiria
    2. Tumore metastatico o tumore con alta probabilità di metastasi
    3. Neoplasia cutanea infiltrante (sospetta o nota)
    4. Malattia cardiovascolare instabile (classe III, IV della New York Heart Association)
    5. Condizione ematologica instabile (compresa la sindrome mielodisplastica), epatica, renale, neurologica o endocrina
    6. Condizione collageno-vascolare instabile
    7. Condizione gastrointestinale instabile
    8. Condizione immunosoppressiva
    9. Presenza di difetto della coagulazione ereditario o acquisito clinicamente significativo
  5. Diagnosi clinica di dermatite atopica, malattia di Bowen (BD), carcinoma a cellule basali (BCC), eczema, psoriasi, rosacea, carcinoma a cellule squamose (SCC), altri tumori maligni o benigni all'interno o nelle immediate vicinanze (<2 cm di distanza) al campo di cura.
  6. Presenza di una forte pigmentazione artificiale (ad es. tatuaggi) o qualsiasi altra anomalia che possa influire sulla valutazione della lesione o sulla penetrazione della luce nel campo di trattamento.
  7. Qualsiasi terapia fisica come crioterapia, terapia laser, elettroessicazione, microdermoabrasione, rimozione chirurgica di lesioni, curettage o trattamento con peeling chimici come acido tricloroacetico all'interno o in prossimità (<10 cm di distanza) del campo di trattamento entro 4 settimane prima selezione.
  8. Uno qualsiasi dei trattamenti topici definiti di seguito entro i periodi designati prima dello screening:

    1. Trattamento topico con ALA o esteri di ALA (ad es. acido metil aminolevulinico (MAL)) all'interno del campo di trattamento entro 3 mesi.
    2. Trattamento topico con farmaci immunomodulatori, citostatici o citotossici all'interno o in prossimità (<10 cm di distanza) del campo di trattamento entro 3 mesi.
    3. Inizio della somministrazione topica di un farmaco con ipericina o altri farmaci con potenziale fototossico o fotoallergico all'interno o in prossimità (<10 cm di distanza) del campo di trattamento entro 4 settimane. I soggetti possono, tuttavia, essere idonei se tale farmaco è stato applicato per più di 4 settimane prima dello screening senza evidenza di un'effettiva reazione fototossica/fotoallergica.
  9. Qualsiasi utilizzo dei trattamenti sistemici entro i periodi designati prima dello screening:

    1. Farmaci citostatici o citotossici entro 6 mesi.

La rivalutazione dei soggetti è consentita una volta nel caso in cui il criterio di esclusione 3 sia soddisfatto e l'idoneità possa essere raggiunta entro 4 settimane. La rivalutazione può essere effettuata il giorno del trattamento vero e proprio.

Criteri di esclusione del giorno di somministrazione:

Alla visita 2 (basale, PDT-1)

Soggetti con ustioni solari o altre condizioni cutanee potenzialmente confondenti (ad es. ferite, irritazioni, sanguinamento o infezioni cutanee) all'interno o in prossimità (<2 cm di distanza) del campo di trattamento. La rivalutazione dei soggetti è consentita una volta se si prevede che la scottatura solare o altre condizioni cutanee confondenti si risolvano entro 2 settimane.

Alla visita 4 (PDT-2)

Soggetti con ustioni solari o altre condizioni cutanee potenzialmente confondenti (ad es. ferite, irritazioni, sanguinamento o infezioni cutanee) all'interno o in prossimità (<2 cm di distanza) del campo di trattamento. La riprogrammazione della PDT-2 può essere eseguita una volta al più presto possibile dopo la risoluzione, ma la riprogrammazione non deve superare le 2 settimane.

Applicazione topica del veicolo a BF-200 ALA che non contiene principio attivo. Terapia fotodinamica a luce rossa (PDT)

Fino a due PDT utilizzando una lampada RhodoLED (RhodoLED® XL o BF-RhodoLED®) (Vehicle-PDT):

Applicazione topica di un veicolo fino a 3 tubi sul campo di trattamento ampliato (fino a 240 cm²), seguita da illuminazione a luce rossa con una lampada RhodoLED dopo 3 ore di incubazione del farmaco in studio sotto bendaggio occlusivo.

La PDT-1 verrà eseguita alla Visita 2. L'autorizzazione clinica sarà valutata 12 settimane dopo la PDT-1 (Visita 4). In caso di lesioni rimanenti alla Visita 4, la PDT-2 verrà eseguita alla stessa visita.

The esthetic outcome at follow-up visits as assessed by the subject according to a 4-point scale ranging from 0 (=very good) to 3 (=unsatisfactory); overall and stratified by treatment area

Satisfaction With PDT at Follow-up Visits

Satisfaction with PDT treatment after the last PDT as assessed by the subject via questionnaire, 1. if they would chose PDT treatment in the future in case of recurrence or similar disease (yes/no) and 2. how they would rate the PDT treatment in comparison to other treatment modalities, if applicable (better than/ similar/ worse). The treatment modalities should be documented, if applicable); overall and stratified by treatment area
On follow-up Visit 1 (6 months after last PDT) and follow-up Visit 2 (12 months after last PDT)
Any SAE and Relevant AE
Lasso di tempo: Entire follow-up duration, approx. 40 weeks
Relevant AEs include AEs or conditions affecting skin health in the treatment field which may impair proper assessment of the recurrence rate of the treated AK lesions, or other clinically relevant events at the investigator's discretion as well as any SAE that has occurred since the final visit of the treatment phase (Visit 4 or Visit 6); overall and stratified by demographics and treatment area
Entire follow-up duration, approx. 40 weeks
New Lesions Inside the Treatment Field During Follow-up

New lesions: AK, non-melanoma skin cancer [NMSC, including BCC, SCC or BD] or melanoma
