Opis
Kryteria przyjęcia:
- Chęć i zdolność osób do wyrażenia świadomej zgody i podpisania formularza ustawy o przenośności i odpowiedzialności w ubezpieczeniach zdrowotnych (HIPAA). Świadomą zgodę dotyczącą konkretnego badania i formularz HIPAA należy uzyskać na piśmie przed rozpoczęciem jakichkolwiek procedur badawczych.
- 4–15 łagodnych do umiarkowanych klinicznie potwierdzonych zmian AK według Olsena na kończynach lub szyi/tułowiu o średnicy ≥ 4 mm, które muszą być obecne w polu zabiegowym (określane jako zmiany docelowe AK). Ponadto w polu leczenia mogą występować niedocelowe zmiany AK, w tym do dwóch ciężkich zmian AK ≥ 4 mm. W przypadku każdej ciężkiej zmiany AK (≥ 4 mm) należy wykonać biopsję w celu potwierdzenia rozpoznania. Pole zabiegowe (ciągłe lub w kilku płatach) o łącznej powierzchni ok. 80 cm², 160 cm² lub 240 cm2 musi znajdować się w obrębie jednego efektywnego obszaru oświetlenia BF-RhodoLED® XL, ale może wymagać do trzech naświetleń z BF-RhodoLED®. Wszystkie zmiany docelowe AK i, jeśli dotyczy, ciężkie zmiany AK ≥ 4 mm zlokalizowane w polu leczenia powinny być wyraźnie widoczne, bez ograniczeń odległości między zmianami i powinny znajdować się w minimalnej odległości 1 cm od granicy pola leczenia.
- Wszystkie płci, ≥ 18 lat.
- Gotowość do poddania się biopsji punktowej 2 mm dla każdej (do dwóch) ciężkich zmian AK ≥ 4 mm, jeśli dotyczy, podczas wizyty przesiewowej.
- Chęć i zdolność do przestrzegania procedur badawczych, w szczególności chęć otrzymania do 2 PDT w ciągu około 12 tygodni.
- Pacjenci o dobrym ogólnym stanie zdrowia lub ze stabilnymi klinicznie schorzeniami będą mogli zostać włączeni do badania.
- Gotowość do zaprzestania stosowania środków nawilżających i wszelkich innych niemedycznych zabiegów miejscowych w obszarze leczenia co najmniej 24 godziny przed następną wizytą kliniczną.
- Akceptacja powstrzymania się od ekstensywnego opalania i korzystania z solarium lub solarium w fazie leczenia.
- Dla kobiet w wieku rozrodczym: Ujemny wynik testu ciążowego z surowicy.
- Dla kobiet w wieku rozrodczym: Skuteczna antykoncepcja podczas wizyty przesiewowej i podczas fazy leczenia badania (do wizyty 4 lub wizyty 6).
Kryteria wyłączenia:
- Jakakolwiek znana historia nadwrażliwości na ALA, porfiryny lub substancje pomocnicze BF-200 ALA.
- Historia alergii na soję lub orzeszki ziemne.
- Oparzenia słoneczne lub inne możliwe zaburzenia skóry (np. rany, podrażnienia, krwawienia lub infekcje skóry) wewnątrz lub w bliskiej odległości (< 2 cm odległości) od pola zabiegowego.
Klinicznie istotne (cs) stany chorobowe utrudniające wykonanie protokołu lub interpretację wyników badania lub zagrażające bezpieczeństwu uczestnika, takie jak:
- Obecność fotodermatoz lub porfirii
- Guz z przerzutami lub guz z dużym prawdopodobieństwem przerzutów
- Naciekający nowotwór skóry (podejrzewany lub znany)
- Niestabilna choroba sercowo-naczyniowa (klasa III, IV według New York Heart Association)
- Niestabilny stan hematologiczny (w tym zespół mielodysplastyczny), wątroby, nerek, neurologiczny lub hormonalny
- Niestabilny stan kolagenowo-naczyniowy
- Niestabilny stan przewodu pokarmowego
- Stan immunosupresyjny
- Obecność istotnej klinicznie wrodzonej lub nabytej wady krzepnięcia
- Kliniczna diagnostyka atopowego zapalenia skóry, choroby Bowena (ChAD), raka podstawnokomórkowego (BCC), egzemy, łuszczycy, trądziku różowatego, raka płaskonabłonkowego (SCC), innych złośliwych lub łagodnych guzów wewnątrz lub w bliskiej odległości (< 2 cm odległości) na pole zabiegowe.
- Obecność silnej sztucznej pigmentacji (np. tatuaży) lub innych nieprawidłowości, które mogą mieć wpływ na ocenę zmiany chorobowej lub penetrację światła w polu zabiegowym.
- Wszelkie zabiegi fizjoterapeutyczne, takie jak krioterapia, laseroterapia, elektrodessykacja, mikrodermabrazja, chirurgiczne usuwanie zmian chorobowych, łyżeczkowanie lub zabiegi z użyciem peelingów chemicznych, takich jak kwas trójchlorooctowy, wewnątrz lub w bliskiej odległości (< 10 cm) pola zabiegowego w ciągu 4 tygodni przed zabiegiem ekranizacja.
Dowolne z miejscowych terapii określonych poniżej w wyznaczonych okresach przed badaniem przesiewowym:
- Miejscowe leczenie za pomocą ALA lub estrów ALA (np. kwasu metyloaminolewulinowego (MAL)) w obrębie pola zabiegowego w ciągu 3 miesięcy.
- Miejscowe leczenie lekami immunomodulującymi, cytostatycznymi lub cytotoksycznymi wewnątrz lub w bliskiej odległości (< 10 cm) od pola zabiegowego w ciągu 3 miesięcy.
- Rozpoczęcie miejscowego podawania leku z hiperycyną lub innymi lekami o potencjale fototoksycznym lub fotoalergicznym wewnątrz lub w bliskiej odległości (< 10 cm) od pola zabiegowego w ciągu 4 tygodni. Pacjenci mogą jednak kwalifikować się, jeśli taki lek był stosowany przez ponad 4 tygodnie przed badaniem przesiewowym bez dowodów na rzeczywistą reakcję fototoksyczną/fotoalergiczną.
Jakiekolwiek zastosowanie leczenia systemowego w wyznaczonych okresach przed skriningiem:
- Leki cytostatyczne lub cytotoksyczne w ciągu 6 miesięcy.
- Terapie immunosupresyjne lub ALA lub estry ALA (np. MAL) w ciągu 12 tygodni.
- Leki, o których wiadomo, że mają poważną toksyczność narządową w ciągu 8 tygodni.
- Interferon lub glikokortykosteroidy w ciągu 6 tygodni.
- Rozpoczęcie przyjmowania leków zawierających hiperycynę lub leków o potencjale fototoksycznym lub fotoalergicznym w ciągu 8 tygodni przed skriningiem. Pacjenci mogą jednak kwalifikować się, jeśli takie leki były przyjmowane przez ponad 8 tygodni przed wizytą przesiewową bez dowodów rzeczywistej reakcji fototoksycznej/fotoalergicznej.
- Kobiety karmiące piersią.
- Podejrzenie nadużywania narkotyków lub alkoholu.
- Osoby, które prawdopodobnie nie będą przestrzegać protokołu, np. niemożność powrotu na wizyty, mało prawdopodobne ukończenie badania lub nieodpowiednie w opinii badacza.
- Członek personelu ośrodka badawczego lub personel sponsora bezpośrednio zaangażowany w prowadzenie protokołu lub jego bliski krewny.
- Otrzymanie jakiegokolwiek badanego leku lub produktu medycznego w ciągu 8 tygodni przed badaniem przesiewowym lub równoczesnym udziałem w innym badaniu klinicznym.
Ponowna ocena uczestników jest dozwolona jednorazowo, jeżeli kryterium wykluczenia 3 jest spełnione, a kwalifikowalność można uzyskać w ciągu 4 tygodni. Ponowną ocenę można przeprowadzić w dniu właściwego leczenia.
Kryteria wykluczenia dnia dawkowania:
Podczas wizyty 2 (poziom wyjściowy, PDT-1)
Osoby z oparzeniami słonecznymi lub innymi potencjalnie zakłócającymi chorobami skóry (np. ranami, podrażnieniami, krwawieniami lub infekcjami skóry) wewnątrz lub w bliskiej odległości (< 2 cm odległości) od pola zabiegowego. Ponowna ocena uczestników jest dozwolona raz, jeśli oparzenia słoneczne lub inne zakłócające problemy skórne mają ustąpić w ciągu 2 tygodni.
Podczas wizyty 4 (PDT-2)
Osoby z oparzeniami słonecznymi lub innymi potencjalnie zakłócającymi chorobami skóry (np. ranami, podrażnieniami, krwawieniami lub infekcjami skóry) wewnątrz lub w bliskiej odległości (< 2 cm odległości) od pola zabiegowego. Zmiana terminu PDT-2 może być przeprowadzona jednorazowo w najbliższym możliwym terminie po ustąpieniu, ale zmiana terminu nie powinna przekraczać 2 tygodni.

Terapia fotodynamiczna światłem czerwonym (PDT)

Do dwóch PDT z lampą RhodoLED (RhodoLED® XL lub BF-RhodoLED®) (Vehicle-PDT): Miejscowe nałożenie do 3 tubek nośnika na rozszerzone pole zabiegowe (do 240 cm²), a następnie naświetlanie światłem czerwonym lampą RhodoLED po 3 h inkubacji badanego leku pod opatrunkiem okluzyjnym. PDT-1 zostanie przeprowadzona podczas wizyty 2. Klirens kliniczny zostanie oceniony 12 tygodni po PDT-1 (wizyta 4). W przypadku pozostania zmian na wizycie 4, PDT-2 zostanie wykonane na tej samej wizycie. |

Satisfaction with PDT treatment after the last PDT as assessed by the subject via questionnaire, 1. if they would chose PDT treatment in the future in case of recurrence or similar disease (yes/no) and 2. how they would rate the PDT treatment in comparison to other treatment modalities, if applicable (better than/ similar/ worse).
The treatment modalities should be documented, if applicable); overall and stratified by treatment area

Any SAE and Relevant AE

Relevant AEs include AEs or conditions affecting skin health in the treatment field which may impair proper assessment of the recurrence rate of the treated AK lesions, or other clinically relevant events at the investigator's discretion as well as any SAE that has occurred since the final visit of the treatment phase (Visit 4 or Visit 6); overall and stratified by demographics and treatment area
