Beschrijving
Inclusiecriteria:
- Bereidheid en vermogen van proefpersonen om geïnformeerde toestemming te geven en het HIPAA-formulier (Health Insurance Portability and Accountability Act) te ondertekenen. Een studiespecifieke geïnformeerde toestemming en een HIPAA-formulier moeten schriftelijk worden verkregen voordat met enige studieprocedure wordt begonnen.
- 4 - 15 milde tot matige klinisch bevestigde AK-laesies volgens Olsen op de extremiteiten of op de nek/romp met een diameter van ≥ 4 mm die aanwezig moeten zijn in het behandelgebied (gedefinieerd als AK-doellaesies). Bovendien kunnen niet-doel-AK-laesies aanwezig zijn in het behandelgebied, waaronder maximaal twee ernstige AK-laesies ≥ 4 mm. Voor elke ernstige AK-laesie (≥ 4 mm) moet een biopsie worden genomen ter bevestiging van de diagnose. Het behandelveld (continu of in meerdere vlakken) van in totaal ca. 80 cm², 160 cm² of 240 cm2 moet binnen één effectief verlichtingsgebied van de BF-RhodoLED® XL liggen, maar met de BF-RhodoLED® kunnen maximaal drie verlichtingen nodig zijn. Alle AK-doellaesies en, indien van toepassing, ernstige AK-laesies ≥ 4 mm die zich in het behandelgebied bevinden, moeten duidelijk te onderscheiden zijn, zonder beperkingen op de afstand tussen laesies, en moeten een minimale afstand van 1 cm tot de grens van het behandelgebied hebben.
- Alle geslachten, ≥ 18 jaar.
- Bereidheid om een ponsbiopsie van 2 mm te ondergaan voor elke (maximaal twee) ernstige AK-laesie ≥ 4 mm, indien van toepassing, tijdens het screeningsbezoek.
- Bereidheid en bekwaamheid om te voldoen aan studieprocedures, met name de bereidheid om maximaal 2 PDT's te ontvangen binnen ongeveer 12 weken.
- Proefpersonen met een goede algemene gezondheid of met klinisch stabiele medische aandoeningen mogen in het onderzoek worden opgenomen.
- Bereidheid om het gebruik van vochtinbrengende crèmes en andere niet-medische lokale behandelingen binnen het behandelgebied ten minste 24 uur voorafgaand aan het volgende klinische bezoek te stoppen.
- Acceptatie om tijdens de behandelingsfase af te zien van uitgebreid zonnebaden en het gebruik van een zonnebank of zonnebank.
- Voor vrouwelijke proefpersonen met voortplantingsvermogen: negatieve serumzwangerschapstest.
- Voor vrouwelijke proefpersonen met reproductief potentieel: effectieve anticonceptie tijdens het screeningsbezoek en tijdens de behandelingsfase van het onderzoek (tot bezoek 4 of bezoek 6).
Uitsluitingscriteria:
- Elke bekende voorgeschiedenis van overgevoeligheid voor ALA, porfyrines of hulpstoffen van BF-200 ALA.
- Geschiedenis van soja- of pinda-allergie.
- Zonnebrand of andere mogelijk storende huidaandoeningen (bijv. wonden, irritaties, bloedingen of huidinfecties) binnen of in de directe nabijheid (< 2 cm afstand) van het behandelingsveld.
Klinisch significante (cs) medische aandoeningen die de uitvoering van het protocol of de interpretatie van de onderzoeksresultaten bemoeilijken of de veiligheid van de proefpersoon in gevaar brengen, zoals:
- Aanwezigheid van fotodermatose of porfyrie
- Gemetastaseerde tumor of tumor met grote kans op uitzaaiing
- Infiltrerende huidneoplasie (vermoedelijk of bekend)
- Instabiele hart- en vaatziekten (New York Heart Association klasse III, IV)
- Onstabiele hematologische (inclusief myelodysplastisch syndroom), lever-, nier-, neurologische of endocriene aandoening
- Onstabiele collageen-vasculaire toestand
- Onstabiele gastro-intestinale aandoening
- Immunosuppressieve aandoening
- Aanwezigheid van klinisch significant erfelijk of verworven stollingsdefect
- Klinische diagnose van atopische dermatitis, ziekte van Bowen (BD), basaalcelcarcinoom (BCC), eczeem, psoriasis, rosacea, plaveiselcelcarcinoom (SCC), andere kwaadaardige of goedaardige tumoren in of dichtbij (< 2 cm afstand) naar het behandelveld.
- Aanwezigheid van sterke kunstmatige pigmentatie (bijv. tatoeages) of enige andere afwijking die van invloed kan zijn op de beoordeling van laesies of lichtpenetratie in het behandelgebied.
- Elke fysiotherapie zoals cryotherapie, lasertherapie, elektrodessicatie, microdermabrasie, chirurgische verwijdering van laesies, curettage of behandeling met chemische peelings zoals trichloorazijnzuur binnen of in de nabijheid (< 10 cm afstand) van het behandelgebied binnen 4 weken voorafgaand aan screening.
Een van de hieronder gedefinieerde topische behandelingen binnen de aangegeven perioden voorafgaand aan de screening:
- Topische behandeling met ALA of ALA-esters (bijv. Methylaminolevulinezuur (MAL)) binnen het behandelgebied binnen 3 maanden.
- Topische behandeling met immunomodulerende, cytostatische of cytotoxische geneesmiddelen binnen of in de directe nabijheid (< 10 cm afstand) van het behandelingsveld binnen 3 maanden.
- Start van topische toediening van een medicatie met hypericine of andere geneesmiddelen met fototoxisch of fotoallergisch potentieel binnen of in de nabijheid (< 10 cm afstand) van het behandelingsveld binnen 4 weken. Proefpersonen kunnen echter in aanmerking komen als dergelijke medicatie meer dan 4 weken voorafgaand aan de screening is toegepast zonder bewijs van een daadwerkelijke fototoxische/fotoallergische reactie.
Elk gebruik van de systemische behandelingen binnen de daarvoor bestemde periodes voorafgaand aan de screening:
- Cytostatische of cytotoxische geneesmiddelen binnen 6 maanden.
- Immunosuppressieve therapieën of ALA of ALA-esters (bijv. MAL) binnen 12 weken.
- Geneesmiddelen waarvan bekend is dat ze binnen 8 weken ernstige orgaantoxiciteit hebben.
- Interferon of glucocorticosteroïden binnen 6 weken.
- Start van inname van medicatie met hypericine of geneesmiddelen met fototoxisch of fotoallergisch potentieel binnen 8 weken voorafgaand aan de screening. Proefpersonen kunnen echter in aanmerking komen als dergelijke medicatie meer dan 8 weken voorafgaand aan het screeningsbezoek is ingenomen zonder bewijs van een daadwerkelijke fototoxische/fotoallergische reactie.
- Vrouwen die borstvoeding geven.
- Verdenking van drugs- of alcoholmisbruik.
- Proefpersonen die zich waarschijnlijk niet aan het protocol houden, bijv. niet in staat om terug te komen voor bezoeken, het onderzoek waarschijnlijk niet af te ronden of ongepast naar de mening van de onderzoeker.
- Een personeelslid van de onderzoekslocatie of sponsorpersoneel dat direct betrokken is bij de uitvoering van het protocol of een naast familielid daarvan.
- Ontvangst van een onderzoeksgeneesmiddel of medisch product binnen 8 weken vóór screening of gelijktijdige deelname aan een andere klinische studie.
Herbeoordeling van proefpersonen is eenmalig toegestaan indien aan uitsluitingscriterium 3 is voldaan en geschiktheid binnen 4 weken kan worden bereikt. Herbeoordeling kan op de dag van de eigenlijke behandeling.
Uitsluitingscriteria doseringsdag:
Bij bezoek 2 (baseline, PDT-1)
Proefpersonen met zonnebrand of andere mogelijk verwarrende huidaandoeningen (bijv. wonden, irritaties, bloedingen of huidinfecties) binnen of in de directe nabijheid (< 2 cm afstand) van het te behandelen gebied. Herbeoordeling van proefpersonen is eenmalig toegestaan als de zonnebrand of andere verstorende huidaandoeningen naar verwachting binnen 2 weken zullen verdwijnen.
Bij bezoek 4 (PDT-2)
Proefpersonen met zonnebrand of andere mogelijk verwarrende huidaandoeningen (bijv. wonden, irritaties, bloedingen of huidinfecties) binnen of in de directe nabijheid (< 2 cm afstand) van het te behandelen gebied. Het opnieuw plannen van PDT-2 kan zo spoedig mogelijk na het oplossen eenmalig worden uitgevoerd, maar het opnieuw plannen mag niet langer dan 2 weken duren.

Maximaal twee PDT's met een RhodoLED-lamp (RhodoLED® XL of BF-RhodoLED®) (Vehicle-PDT): Topische toepassing van maximaal 3 buisjes op het uitgebreide behandelveld (tot 240 cm²), gevolgd door roodlichtverlichting met een RhodoLED-lamp na 3 uur incubatie van de studiemedicatie onder occlusief verband. PDT-1 zal worden uitgevoerd bij bezoek 2. Klinische klaring zal 12 weken na PDT-1 worden beoordeeld (bezoek 4). In het geval van resterende laesies bij bezoek 4, zal PDT-2 tijdens hetzelfde bezoek worden uitgevoerd. |

Satisfaction With PDT at Follow-up Visits

Satisfaction with PDT treatment after the last PDT as assessed by the subject via questionnaire, 1. if they would chose PDT treatment in the future in case of recurrence or similar disease (yes/no) and 2. how they would rate the PDT treatment in comparison to other treatment modalities, if applicable (better than/ similar/ worse).

Relevant AEs include AEs or conditions affecting skin health in the treatment field which may impair proper assessment of the recurrence rate of the treated AK lesions, or other clinically relevant events at the investigator's discretion as well as any SAE that has occurred since the final visit of the treatment phase (Visit 4 or Visit 6); overall and stratified by demographics and treatment area
