L'hypoxie-ischémie cérébrale périnatale est une cause importante de décès et d'incapacité neurodéveloppementale. Les données provenant de modèles animaux suggèrent que le refroidissement du cerveau immédiatement après une blessure est neuroprotecteur. L'expérience du refroidissement total du corps pendant la chirurgie, les quasi-noyades accidentelles et un essai de phase I sur des nourrissons nés à terme suggèrent qu'il est efficace et sûr chez les enfants.
Ce grand essai multicentrique a testé si le refroidissement cérébral initié dans les 6 heures suivant la naissance et poursuivi pendant 72 heures réduirait le risque de décès et de lésions neurodéveloppementales modérées à sévères à un âge corrigé de 18 à 22 mois. Les nourrissons d'au moins 36 semaines de gestation présentant des gaz sanguins anormaux dans l'heure suivant la naissance, ou des antécédents d'événement périnatal aigu et un score d'Apgar à 10 minutes <5, ou un besoin continu de ventilation ont été dépistés. Après un examen neurologique, les personnes atteintes d'encéphalopathie modérée à sévère ont été randomisées pour une période de 72 heures de refroidissement corporel total (couverture rafraîchissante, suivie d'un réchauffement lent). L'étude a été menée en deux phases : Phase I (20 nourrissons) ont été examinés pour la sécurité d'une température oesophagienne de 34-35 C ; La phase II (essai principal, 200 nourrissons) a été évaluée pour l'innocuité et l'efficacité d'une température œsophagienne de 33 à 34 C. Les paramètres cardio-respiratoires, les électroencéphalogrammes (EEG), l'état rénal, métabolique et hématologique, et la température cutanée œsophagienne et abdominale ont été évalués. suivi pendant les 72 heures d'intervention.
Les résultats neurodéveloppementaux ont été évalués à 18-22 mois par des examinateurs certifiés masqués. Les résultats à 18-22 mois ont montré que le refroidissement de tout le corps réduit le risque de décès ou d'invalidité modérée à sévère chez les nourrissons atteints d'encéphalopathie ischémique hypoxique.
Les nourrissons survivants ont été évalués à 6-7 ans (âge scolaire).

Critère d'intégration:
- Au moins 36 semaines de gestation
- Tout gaz sanguin (cordon, postnatal) effectué dans les 60 premières minutes avait un pH inférieur ou égal à 7,0
- Tout gaz du sang (cordon postnatal) effectué dans les 60 premières minutes présentait un déficit de base supérieur ou égal à 16 mEq/L
- Tous les nourrissons doivent avoir des convulsions ou des signes d'encéphalopathie modérée à sévère avant la randomisation
Critère d'exclusion:
- Incapacité de randomiser à 6 heures d'âge
- Présence d'une anomalie chromosomique connue ou d'une anomalie congénitale majeure
- Restriction de croissance intra-utérine sévère (poids inférieur à 1800 g)
- Tous les gaz sanguins effectués dans les 60 premières minutes avaient un pH inférieur à 7,15 et un déficit en bases inférieur à 10 mEq/L
- Nourrissons in extremis pour lesquels aucune thérapie intensive supplémentaire ne sera proposée par le néonatologiste traitant
- Les parents refusent le consentement
- Le néonatologiste traitant refuse son consentement
