Dans plusieurs essais cliniques, l'effet positif du peeling de la membrane limitante interne (ILM) sur les résultats anatomiques et fonctionnels de la chirurgie du trou maculaire a été rapporté. Des controverses existent cependant concernant l'application et la toxicité éventuelle des colorants, facilitant l'intervention chirurgicale. Depuis que les colorants ont été introduits pour le pelage de l'ILM, plusieurs auteurs ont signalé l'utilisation du vert d'indocyanine et du bleu trypan (TB) pour le pelage de la membrane ou l'élimination de l'ILM. Des rapports expérimentaux et cliniques récents soulèvent des inquiétudes quant à la sécurité du pelage membranaire assisté par ICG. Des dommages morphologiques et fonctionnels ont été décrits dans un modèle animal, dans les yeux de donneurs humains, dans des cellules épithéliales pigmentaires humaines en culture et dans des études cliniques utilisant l'ICG en chirurgie maculaire. La tolérance intraoculaire de la tuberculose a été démontrée dans des yeux de lapin et dans des études de viabilité des cellules de l'épithélium pigmentaire rétinien. Des concentrations plus élevées de TB (0,6 ou 4 mg/ml) entraînent une toxicité sur les cellules RPE cultivées, mais l'application de niveaux plus faibles cliniquement pertinents n'a montré aucun effet significatif. Une analyse comparative rétrospective récente indique que la tuberculose pourrait être le colorant le plus sûr. Ils ont trouvé de meilleurs résultats anatomiques et fonctionnels qu'avec l'utilisation de l'ICG.
Par conséquent, le but de cette étude était de comparer les résultats anatomiques et fonctionnels après peeling assisté par colorant de l'ILM chez des patients présentant des trous maculaires idiopathiques utilisant l'ICG ou la TB dans un essai prospectif et randomisé.

La description
Critère d'intégration:
- Les patients avec des trous maculaires idiopathiques stade II-IV selon la classification proposée par Gass et une acuité visuelle d'au moins 20/200 sont entrés dans cette étude.
Critère d'exclusion:
- Patients présentant des trous maculaires secondaires, une chirurgie intraoculaire antérieure, à l'exception d'une extraction de cataracte sans incident et des patients présentant des troubles oculaires susceptibles d'interférer avec la vision tels qu'une cataracte sévère, une dégénérescence maculaire exsudative liée à l'âge ou une rétinopathie diabétique.

L'affectation à l'ICG ou au colorant TB a été effectuée par l'Institut de biométrie médicale et de statistique de l'Université de Luebeck par téléphone immédiatement avant la chirurgie. L'attribution aux bras de traitement a été effectuée avec une randomisation en blocs permutés avec une taille de bloc de 20
