Em vários ensaios clínicos, foi relatado o efeito positivo do peeling da membrana limitante interna (ILM) no resultado anatômico e funcional na cirurgia do buraco macular. Porém, existem controvérsias quanto à aplicação e possível toxicidade dos corantes, facilitando o procedimento cirúrgico. Desde que os corantes foram introduzidos para peeling de ILM, vários autores relataram o uso de indocianina verde e azul de tripano (TB) para peeling de membrana ou remoção de ILM. Relatórios experimentais e clínicos recentes levantam preocupações sobre a segurança do peeling de membrana assistido por ICG. Danos morfológicos e funcionais foram descritos em modelo animal, em olhos de doadores humanos, em cultura de células epiteliais pigmentares humanas e em estudos clínicos usando ICG em cirurgia macular. A tolerância intraocular da TB foi demonstrada em olhos de coelho e em estudos de viabilidade de células do epitélio pigmentar da retina. Concentrações mais altas de TB (0,6 ou 4 mg/ml) levam à toxicidade em células RPE cultivadas, mas a aplicação de níveis clinicamente relevantes mais baixos não mostrou nenhum efeito significativo. Uma análise comparativa retrospectiva recente indica que TB pode ser o corante mais seguro. Eles encontraram melhores resultados anatômicos e funcionais do que com o uso de ICG.
Portanto, o objetivo deste estudo foi comparar o resultado anatômico e funcional após o peeling assistido por corante da ILM em pacientes com buracos maculares idiopáticos usando ICG ou TB em um estudo prospectivo e randomizado.

Descrição
Critério de inclusão:
- Pacientes com buraco macular idiopático estágio II-IV de acordo com a classificação proposta por Gass e acuidade visual de pelo menos 20/200 foram incluídos neste estudo.
Critério de exclusão:
- Pacientes com buracos maculares secundários, cirurgia intraocular anterior, exceto para extração de catarata sem intercorrências e pacientes com distúrbios oculares que possam interferir na visão, como catarata grave, degeneração macular relacionada à idade exsudativa ou retinopatia diabética.

O desfecho primário foi a acuidade visual ETDRS (AV: equivalente a snellen) a dois metros 3 meses após a cirurgia.

A atribuição de corante ICG ou TB foi realizada pelo Instituto de Biometria e Estatística Médica da Universidade Luebeck por telefone imediatamente antes da cirurgia. A alocação aos braços de tratamento foi feita com uma randomização de bloco permutado com tamanho de bloco 20
