In verschillende klinische onderzoeken is het positieve effect van inner limiting membrane peeling (ILM) op de anatomische en functionele uitkomst bij maculagatchirurgie gemeld. Er zijn echter controverses over de toepassing en mogelijke toxiciteit van kleurstoffen, die de chirurgische ingreep vergemakkelijken. Sinds de introductie van kleurstoffen voor ILM-peeling rapporteerden verschillende auteurs over het gebruik van indocyaninegroen en trypaanblauw (TB) voor membraanpeeling of ILM-verwijdering. Recente experimentele en klinische rapporten geven aanleiding tot bezorgdheid over de veiligheid van ICG-geassisteerde membraanpeeling. Morfologische en functionele schade werd beschreven in een diermodel, in menselijke donorogen, in gekweekte menselijke pigmentepitheelcellen en in klinische studies met ICG bij maculaire chirurgie. De intraoculaire tolerantie van tuberculose is aangetoond in konijnenogen en in onderzoeken naar de levensvatbaarheid van cellen van het retinale pigmentepitheel. Hogere tbc-concentraties (0,6 of 4 mg/ml) leidden tot toxiciteit op gekweekte RPE-cellen, maar de toepassing van lagere klinisch relevante niveaus liet geen significant effect zien. Een recente retrospectieve vergelijkende analyse geeft aan dat tbc mogelijk de veiligere kleurstof is. Ze vonden betere anatomische en functionele resultaten dan bij het gebruik van ICG.
Daarom was het doel van deze studie om de anatomische en functionele uitkomst te vergelijken na kleurstofondersteunde peeling van de ILM bij patiënten met idiopathische maculaire gaatjes die ICG of TB gebruikten in een prospectieve en gerandomiseerde studie.

Beschrijving
Inclusiecriteria:
- Patiënten met idiopathische maculaire gaatjes stadium II-IV volgens de door Gass voorgestelde classificatie en een gezichtsscherpte van ten minste 20/200 namen deel aan dit onderzoek.
Uitsluitingscriteria:
- Patiënten met secundaire maculaire gaatjes, eerdere intraoculaire chirurgie, behalve ongestoorde cataractextractie en patiënten met oogaandoeningen die het gezichtsvermogen kunnen verstoren, zoals ernstige cataract, exsudatieve leeftijdsgebonden maculaire degeneratie of diabetische retinopathie.
