Immunothérapie vaccinale pour le médulloblastome récurrent et la tumeur neuroectodermique primitive (Re-MATCH)
Médulloblastome récurrent et tumeur neuroectodermique primitive Thérapie cellulaire T adoptive pendant la récupération après une chimiothérapie myéloablative et une greffe de cellules souches hématopoïétiques
L'immunothérapie est une approche spécifique du traitement du cancer qui s'est révélée prometteuse chez les patients adultes pour le traitement du mélanome, des tumeurs cérébrales malignes et d'autres cancers. Les chercheurs de l'étude utiliseront l'expérience qu'ils ont acquise de ces études pour tenter d'améliorer les résultats pour les enfants touchés par une tumeur cérébrale récurrente.
Environ 35 patients présentant une première récidive de médulloblastome (reMB)/tumeurs neuroectodermiques primitives supratentorielles (PNET) seront traités avec des cellules immunitaires spécifiques à la tumeur et des vaccins à base de cellules dendritiques pour voir quel impact ils ont sur la tumeur.

Les tumeurs cérébrales malignes représentent désormais la cause la plus fréquente de décès par cancer chez les enfants. Malgré une thérapie multimodale agressive et hautement toxique comprenant la chirurgie, la radiothérapie craniospinale et la chimiothérapie à haute dose associée à la greffe de cellules souches du sang périphérique, près de la moitié des enfants diagnostiqués avec les tumeurs cérébrales malignes les plus courantes, le médulloblastome (MB) et les tumeurs neuroectodermiques primitives ( PNET), mourront toujours d'une maladie récurrente. De plus, les survivants se retrouvent souvent avec des déficits cognitifs et moteurs graves et permanents associés au traitement. Le développement de thérapies plus efficaces et spécifiques aux tumeurs qui n'ajouteront pas de toxicité supplémentaire aux traitements existants est primordial pour améliorer les résultats cliniques des enfants atteints de MB/PNET. L'immunothérapie ciblant les antigènes spécifiques des tumeurs exprimés dans les tumeurs cérébrales est une modalité potentiellement capable de répondre à ce besoin clair et urgent.
Malgré des progrès considérables et des résultats cliniques prometteurs observés dans les essais d'immunothérapie dirigés contre les tumeurs cérébrales malignes adultes, les efforts dans le traitement immunologique des tumeurs cérébrales pédiatriques ont été limités à relativement peu d'études notables. Cela est dû, au moins en partie, au tissu tumoral viable souvent limité disponible pour les préparations de vaccins à base de cellules tumorales, et au manque d'identification d'antigènes spécifiques à la tumeur exprimés de manière cohérente dans ces cancers.
L'utilisation de cellules dendritiques chargées d'ARN tumoral total (TTRNA) a été lancée à l'Université Duke, en tant que nouvelle plate-forme pour induire de puissantes réponses immunologiques contre la variété d'antigènes non caractérisés et spécifiques au patient présents dans les cellules tumorales malignes. Duke a démontré qu'une quantité suffisante d'ARN pour les préparations de vaccins cliniques peut être amplifiée avec une haute fidélité à l'aide des technologies moléculaires existantes à partir de seulement 500 cellules tumorales cérébrales pédiatriques et adultes isolées, permettant ainsi la préparation de vaccins à partir de biopsies chirurgicales et même d'échantillons tumoraux d'archives microdisséqués.
L'immunothérapie administrée pendant la récupération après une chimiothérapie peut présenter d'énormes avantages, car la thérapie cellulaire adoptive suivant des schémas de conditionnement lymphodéplétif est apparue comme la stratégie de traitement la plus efficace pour le mélanome avancé et réfractaire. Notre hypothèse est que la thérapie DC + ex vivo élargie par transfert lymphocytaire autologue (xALT) ciblant les MB/PNET récurrents pendant la récupération après une chimiothérapie myéloablative sera sûre et prolongera la survie des enfants et des jeunes adultes atteints de MB/PNET récurrents.
Dans cette étude, les chercheurs traiteront les patients présentant une première récidive de reMB/PNET après la fin de la radiothérapie définitive avec une immunothérapie par cellules T autologue spécifique à la tumeur (TTRNA-xALT) plus un vaccin à cellules dendritiques chargées en TTRNA.
Après une résection chirurgicale, une biopsie ou un examen cytologique avec diagnostic pathologique de confirmation, les patients seront inscrits dans le groupe A (chimiothérapie à haute dose ou HDC) ou le groupe B (chimiothérapie de sauvetage non myéloablative ou NMA) en fonction de leur éligibilité pour HDC. Les patients présentant une rechute localisée et n'ayant pas déjà échoué à la greffe de cellules souches du sang périphérique (PBSCT) HDC+ seront inscrits dans le groupe A. Les patients atteints d'une maladie disséminée, ont déjà échoué à l'HDC+PBSCT ou sont autrement considérés comme de mauvais candidats pour la HDC en fonction de l'état de santé général , mais qui répondent par ailleurs aux critères d'éligibilité, seront inscrits dans le groupe B. Tous les patients recevront un traitement DC + xALT.

Critère d'intégration:
Dépistage:
- Âge ≤ 30 ans.
- Première récidive/progression suspectée de MB/PNET depuis l'achèvement de l'irradiation définitive focale +/- craniospinale. La progression de la maladie avant de recevoir une irradiation focale +/- craniospinale définitive ne disqualifiera pas les patients de l'inscription s'ils ont ensuite échoué à la radiothérapie définitive et sont à la première récidive/progression au moment de l'inscription. Les patients qui ne peuvent pas recevoir de radiothérapie en raison de troubles génétiques qui les exposent à un risque important de cancers secondaires radio-induits (c.-à-d. syndrome de Gorlin ou mutation NF1) sont éligibles pour l'inscription à la première récidive/progression de la maladie.
Protocole Re-MATCH :
- Les patients doivent avoir une MB/PNET récurrente histologiquement confirmée qui est une première rechute/progression après la fin de la radiothérapie définitive +/- irradiation craniospinale. Les patients présentant une première rechute/progression qui ne peuvent pas recevoir de radiothérapie en raison de troubles génétiques qui les exposent à un risque important de tumeurs malignes secondaires radio-induites (c. syndrome de Gorlin ou mutation NF1) sont éligibles à l'inscription.
- Les patients présentant des déficits neurologiques doivent avoir des déficits stables pendant au moins 1 semaine avant l'enregistrement.
- Statut de performance Karnofsky ≥ 50 % ou score de performance Lansky ≥ 50.
- Nombre absolu de neutrophiles (ANC) ≥ 1 000/µl (non pris en charge).
- Plaquettes ≥ 100 000/µl (non pris en charge).
- Hémoglobine > 8 g/dL (peut être pris en charge).
- Créatinine sérique ≤ limite supérieure de la normale institutionnelle
- Bilirubine ≤ 1,5 fois la limite supérieure de la normale pour l'âge.
- Transaminase glutamique oxaloacétique (ALT) sérique ≤ 3 fois la limite supérieure institutionnelle de la normale pour l'âge.
- Transaminase glutamique oxaloacétique (AST) sérique ≤ 3 fois la limite supérieure institutionnelle de la normale pour l'âge.
- Les patientes susceptibles de procréer ou d'avoir des enfants doivent être disposées à utiliser une forme de contraception médicalement acceptable, qui comprend l'abstinence, pendant leur traitement dans le cadre de cette étude.
- Consentement du patient ou du tuteur du patient au prélèvement de cellules souches du sang périphérique (PBSC) et/ou de moelle osseuse après l'enregistrement si les PBSC ou la moelle osseuse (nombre de CD34 d'au moins 2x10^6/kg) n'ont pas été précédemment stockés et disponibles pour utilisation.
- Un consentement éclairé signé conformément aux directives institutionnelles doit être obtenu avant l'inscription.
Critère d'exclusion:
- Enceinte ou besoin d'allaiter pendant la période d'étude.
- Infection active nécessitant un traitement ou maladie fébrile inexpliquée (> 101,5 F).
- Maladie immunosuppressive connue, infection par le virus de l'immunodéficience humaine ou porteur du virus de l'hépatite B ou de l'hépatite C.
- Patients atteints d'une maladie rénale, cardiaque (insuffisance cardiaque congestive, infarctus du myocarde, myocardite) ou pulmonaire active.
- Patients recevant des agents immunosuppresseurs concomitants pour leur état de santé.
- Patients qui ont besoin d'une radiothérapie définitive pour le traitement des MB/PNET récurrents. Une radiothérapie focale de rappel peut être administrée avant l'immunothérapie si nécessaire pour un contrôle local.
- Patients recevant tout autre traitement médicamenteux anticancéreux ou expérimental concomitant.
- Patients atteints d'une maladie systémique non liée cliniquement significative (infections graves ou dysfonctionnement cardiaque, pulmonaire, hépatique ou d'un autre organe significatif).
- Patients incapables de revenir pour des visites de suivi ou d'obtenir des études de suivi nécessaires pour évaluer la toxicité du traitement.

We quantified pathway activity using GSVA applied to normalized RNA-seq expression data.
The enrichment score for each sample was defined as the maximum deviation from zero of this running sum, yielding a dimensionless GSVA score that represents the relative coordinated up- or down-regulation of TLR pathway genes within that sample compared to the background transcriptome.
Higher GSVA score represents upregulation and lower GSVA score represents downregulation.
There is no clinical relevance threshold.
