Description détaillée
OBJECTIFS PRINCIPAUX:
1) Déterminer s'il existe une association entre l'activation initiale de la cible mammalienne de la rapamycine (mTOR) associée à l'inhibition de la cible mTOR après le traitement dans les blastes leucémiques et la réponse clinique chez les patients atteints de leucémie myéloïde aiguë (LAM) nouvellement diagnostiquée et traités avec le sirolimus idarubicine/cytarabine .
OBJECTIFS SECONDAIRES :
- Estimer le taux de réponse du sirolimus idarubicine/cytarabine chez les patients atteints de LAM nouvellement diagnostiquée par rapport aux données historiques utilisant l'idarubicine/cytarabine seule.
- Déterminer la capacité du sirolimus oral à inhiber mTOR dans les blastes leucémiques.
- Évaluer si l'inhibition de la voie mTOR est corrélée à la réponse clinique.
- Recueillir des informations supplémentaires sur l'innocuité, la tolérabilité et l'efficacité du sirolimus en association avec l'idarubicine/cytarabine chez les patients atteints de LAM nouvellement diagnostiquée.
- Décrire la survie sans progression et la survie globale (1 an, 2 ans et 5 ans) des patients traités par sirolimus idarubicine/cytarabine.
CONTOUR:
Les patients reçoivent du sirolimus par voie orale (PO) une fois par jour (QD) les jours 1 à 10, de l'idarubicine par voie intraveineuse (IV) pendant 3 à 5 minutes les jours 4 à 6 et de la cytarabine IV en continu pendant 24 heures les jours 4 à 10.
Après la fin du traitement à l'étude, les patients sont suivis tous les 3 mois pendant 5 ans.

Critère d'intégration:
- Les patients doivent avoir des preuves histologiques de leucémie myéloïde aiguë nouvellement diagnostiquée (LMA non-M3) comme documenté par la présence de> 20% de blastes myéloïdes dans la moelle osseuse
- Les sujets doivent être âgés de 18 ans et <= 60
- Les sujets doivent avoir un statut de performance ECOG de 2 ou moins. (voir annexe 1).
- Les sujets doivent avoir une espérance de vie d'au moins 4 semaines.
- Les sujets doivent être capables de consommer des médicaments par voie orale.
- Valeurs de laboratoire initiales requises : Créatinine 2,0 mg/dL ; bilirubine totale ou directe 1,5 mg/dL ; SGPT(ALT) 3xULN (si non dû à la leucémie elle-même) ; test de grossesse négatif pour les femmes en âge de procréer.
- Les patients doivent être en mesure de signer leur consentement et être disposés et capables de se conformer aux visites prévues, au plan de traitement et aux tests de laboratoire.
- Les sujets doivent avoir une fraction d'éjection ventriculaire gauche (FEVG) >/= 45 %.
Critère d'exclusion:
- Les sujets avec APL - FAB M3 (t(15;17)(q22;q21)[PML-RAR] ne sont pas éligibles
- Les sujets ne doivent avoir reçu aucun agent chimiothérapeutique pour la LAM (à l'exception de l'hydroxyurée). L'ARA-C intrathécal et le méthotrexate intrathécal sont autorisés (car ils ne sont pas systémiques et seulement isolés du système nerveux central).
- Les sujets ne doivent pas recevoir de facteurs de croissance, à l'exception de l'érythropoïétine.
- Les sujets atteints d'une seconde tumeur maligne "actuellement active", autre que les cancers de la peau autres que les mélanomes, ne sont pas éligibles.
- Les sujets souffrant d'hypertension artérielle non contrôlée, d'angor instable, d'insuffisance cardiaque congestive symptomatique, d'infarctus du myocarde au cours des 6 derniers mois ou d'arythmie cardiaque grave non contrôlée ne sont pas éligibles.
Les sujets prenant ce qui suit ne sont pas éligibles:
- Carbamazépine (par exemple, Tegretol)
- Rifabutine (par exemple, mycobutine)
- Rifampine (par exemple, Rifadin)
- Rifapentine (par exemple, Priftin)
- millepertuis
- Clarithromycine (par exemple, Biaxin)
- Ciclosporine (par ex. Neoral ou Sandimmun)
- Diltiazem (par exemple, Cardizem)
- Érythromycine (p. ex., Akne-Mycin, Ery-Tab)
- Itraconazole (par exemple, Sporanox)
- Kétoconazole (par exemple, Nizoral)
- Télithromycine (par exemple, Ketek)
- Vérapamil (par exemple, Calan SR, Isoptin, Verelan)
- Voriconazole (par exemple, VFEND)
- Tacrolimus (par exemple Prograf)
- Les sujets prenant du fluconazole, du voriconazole, de l'itraconazole, du posaconazole et du kétoconazole dans les 72 heures suivant l'entrée à l'étude ne sont pas éligibles. La réintroduction du fluconazole, du voriconazole, de l'itraconazole, du posaconazole, du kétoconazole et du diltiazem est autorisée 72 heures après la dernière dose de sirolimus.
- Sujets nécessitant des inhibiteurs de la protéase du VIH ou atteints d'une maladie liée au sida
- Les sujets atteints d'une autre maladie concomitante grave qui, de l'avis de l'investigateur, rendrait le patient inapproprié pour participer à cette étude ne sont pas éligibles.
- Les sujets ne doivent pas être enceintes ni allaiter. Des tests de grossesse doivent être obtenus pour toutes les femmes en âge de procréer. Les patientes enceintes ou allaitantes ne sont pas éligibles pour cette étude en raison de la toxicité fœtale humaine ou tératogène inconnue du sirolimus. Les hommes ou les femmes en âge de procréer ne peuvent participer que s'ils ont accepté d'utiliser une méthode contraceptive efficace.
- Les sujets qui ont une infection non contrôlée ne sont pas éligibles. Les patients doivent contrôler toutes les infections actives. La maladie fongique doit être stable pendant au moins 2 semaines avant l'entrée à l'étude.
- Les sujets atteints de bactériémie doivent avoir documenté des hémocultures négatives avant l'entrée à l'étude.

Les patients reçoivent Sirolimus PO QD les jours 1-10, l'idarubicine IV en 3 à 5 minutes les jours 4-6 et la cytarabine IV en continu sur 24 heures les jours 4-10.
