PRIMAIRE DOELEN:
1) Om te bepalen of er een verband bestaat tussen activatie van het zoogdierdoelwit van rapamycine (mTOR) in de uitgangssituatie gecombineerd met mTOR-doelremming na behandeling bij leukemieblasten en klinische respons bij patiënten met nieuw gediagnosticeerde acute myeloïde leukemie (AML) behandeld met sirolimus idarubicine/cytarabine .
SECUNDAIRE DOELSTELLINGEN:
- Om het responspercentage van sirolimus idarubicine/cytarabine bij patiënten met nieuw gediagnosticeerde AML te schatten in vergelijking met historische gegevens met alleen idarubicine/cytarabine.
- Om het vermogen van orale sirolimus te bepalen om mTOR in leukemische blasten te remmen.
- Om te beoordelen of remming van de mTOR-route correleert met de klinische respons.
- Meer informatie verzamelen over de veiligheid, verdraagbaarheid en werkzaamheid van sirolimus in combinatie met idarubicine/cytarabine bij patiënten met nieuw gediagnosticeerde AML.
- Beschrijven van de progressievrije overleving en algehele overleving (1 jaar, 2 jaar en 5 jaar) van patiënten die werden behandeld met sirolimus idarubicine/cytarabine.
OVERZICHT:
Patiënten krijgen sirolimus oraal (PO) eenmaal daags (QD) op dagen 1-10, idarubicine intraveneus (IV) gedurende 3-5 minuten op dagen 4-6, en cytarabine IV continu gedurende 24 uur op dagen 4-10.
Na voltooiing van de studiebehandeling worden patiënten gedurende 5 jaar elke 3 maanden opgevolgd.

Beschrijving
Inclusiecriteria:
- Patiënten moeten histologisch bewijs hebben van nieuw gediagnosticeerde acute myeloïde leukemie (niet-M3 AML), zoals gedocumenteerd door de aanwezigheid van> 20% myeloïde blasten in het beenmerg
- Proefpersonen moeten 18 jaar en <= 60 jaar zijn
- Proefpersonen moeten een ECOG-prestatiestatus van 2 of minder hebben. (zie bijlage 1).
- Proefpersonen moeten een levensverwachting hebben van minimaal 4 weken.
- Proefpersonen moeten orale medicatie kunnen consumeren.
- Vereiste aanvankelijke laboratoriumwaarden: creatinine 2,0 mg/dl; totaal of direct bilirubine 1,5 mg/dL; SGPT(ALT) 3xULN (indien niet vanwege de leukemie zelf); negatieve zwangerschapstest voor vrouwen die zwanger kunnen worden.
- Patiënten moeten toestemming kunnen ondertekenen en bereid en in staat zijn zich te houden aan geplande bezoeken, behandelplan en laboratoriumonderzoek.
- Proefpersonen moeten een linkerventrikelejectiefractie (LVEF) van >/= 45% hebben.
Uitsluitingscriteria:
- Proefpersonen met APL - FAB M3 (t(15;17)(q22;q21)[PML-RAR] komen niet in aanmerking
- Proefpersonen mogen geen chemotherapeutica voor de AML hebben gekregen (behalve Hydroxyurea). Intrathecaal ARA-C en intrathecaal methotrexaat zijn toegestaan (aangezien ze niet systemisch zijn en alleen geïsoleerd zijn voor het centrale zenuwstelsel).
- Proefpersonen mogen geen groeifactoren krijgen, behalve erytropoëtine.
- Onderwerpen met een "momenteel actieve" tweede maligniteit, anders dan niet-melanoom huidkanker, komen niet in aanmerking.
- Onderwerpen met ongecontroleerde hoge bloeddruk, onstabiele angina pectoris, symptomatisch congestief hartfalen, myocardinfarct in de afgelopen 6 maanden of ernstige ongecontroleerde hartritmestoornissen komen niet in aanmerking.

- Diltiazem (bijv. Cardizem)
- Erytromycine (bijv. Akne-Mycin, Ery-Tab)
- Itraconazol (bijv. Sporanox)
- Ketoconazol (bijv. Nizoral)
- Telithromycine (bijv. Ketek)
- Verapamil (bijv. Calan SR, Isoptin, Verelan)
- Voriconazol (bijv. VFEND)
- Tacrolimus (bijv. Prograft)
- Proefpersonen die fluconazol, voriconazol, itraconazol, posaconazol en ketoconazol gebruiken binnen 72 uur na deelname aan het onderzoek komen niet in aanmerking. Herstel van fluconazol, voriconazol, itraconazol, posaconazol, ketoconazol en diltiazem is toegestaan 72 uur na de laatste dosis sirolimus.
- Onderwerpen die hiv-proteaseremmers nodig hebben of mensen met een aan aids gerelateerde ziekte
- Proefpersonen met een andere ernstige gelijktijdige ziekte die naar het oordeel van de onderzoeker de patiënt ongeschikt zou maken voor deelname aan dit onderzoek, komen niet in aanmerking.
- Onderwerpen mogen niet zwanger zijn of borstvoeding geven. Zwangerschapstests moeten worden verkregen voor alle vrouwen in de vruchtbare leeftijd. Zwangere of zogende patiënten komen niet in aanmerking voor deze studie vanwege de onbekende menselijke foetale of teratogene toxiciteit van sirolimus. Mannen of vrouwen in de vruchtbare leeftijd mogen niet deelnemen, tenzij ze ermee hebben ingestemd een effectieve anticonceptiemethode te gebruiken.
- Proefpersonen met een ongecontroleerde infectie komen niet in aanmerking. Patiënten moeten eventuele actieve infecties onder controle hebben. Schimmelziekte moet gedurende ten minste 2 weken stabiel zijn voordat het onderzoek wordt gestart.
- Proefpersonen met bacteriëmie moeten gedocumenteerde negatieve bloedkweken hebben voordat ze aan het onderzoek beginnen.

Zal worden geëvalueerd met behulp van de Kaplan-Meier-methode, gestratificeerd op basis van mTOR-respons.
De log-ranktest zal worden gebruikt om de algehele overleving van patiënten met en zonder mTOR-respons te vergelijken.
Op basis van de geschatte overlevingscurven worden de 1-jaars, 2-jaars en 5-jaars overlevingspercentages berekend met de overeenkomstige 95% betrouwbaarheidsintervallen.
