Critère d'intégration:
- Capable et disposé à fournir un consentement éclairé écrit
- Capable et disposé à se conformer aux procédures d'étude requises et au calendrier de suivi
- 22 à 75 ans au moment du consentement éclairé
- Symptômes documentés de reflux gastro-œsophagien (RGO) depuis plus de 6 mois (régurgitation et/ou brûlures d'estomac définies comme une douleur épigastrique ou sous-sternale brûlante qui répond à la neutralisation ou à la suppression de l'acide) nécessitant l'utilisation quotidienne d'inhibiteurs de la pompe à protons (IPP) ou d'autres traitement médicamenteux anti-reflux, qui continuent à avoir des symptômes malgré un traitement médical maximal ou qui sont "intolérants" - effets secondaires graves (par ex. anaphylaxie ou réaction allergique sévère, C. difficile récurrent, hypomagnésémie sévère) à un IPP ou effet secondaire léger/modéré (par ex. nausées, vomissements, diarrhée ou douleurs abdominales) à au moins 2 IPP de classes chimiques différentes.
- Amélioration symptomatique du traitement par IPP démontrée par un score composite RGO-qualité de vie liée à la santé (HRQL) ≥20 hors IPP, et une amélioration ≥10 points sur IPP par rapport au score composite RGO-HRQL hors IPP. Les patients qui répondent à la définition ci-dessus d'intolérance aux IPP ne sont pas tenus d'avoir une amélioration ≥10 points. Le score sur le PPI pour satisfaire à ce critère sera le score de l'évaluation GERD-HRQL réalisée après la reprise des PPI après la visite de référence.
- Exposition excessive à l'acide œsophagien inférieur pendant la surveillance du pH (définie comme pH œsophagien distal < 4 pendant > 6,0 % du temps de surveillance) effectuée après au moins 5 jours d'arrêt des IPP et au moins 2 jours d'arrêt des anti-H2. Au moins 18 heures d'enregistrement du pH œsophagien seront considérées comme adéquates et l'inclusion sera basée sur le jour (au moins 18 heures de données valides) avec le pourcentage de temps d'exposition à l'acide le plus élevé.
- Oesophagite ≤ Grade B (classification de Los Angeles (LA)) telle que mesurée par endoscopie haute sans IPP et anti-H2 pendant 10 à 14 jours
- Amplitude de contraction du corps œsophagien > 30 mmHg pour > 30 % des déglutitions et > 30 % de contractions péristaltiques sur HRM ou ≥ 30 % de contractions péristaltiques avec DCI > 450.
- Candidat chirurgical apte à subir une anesthésie générale et une chirurgie laparoscopique
Critère d'exclusion:
- Implantation et/ou tentative d'implantation du système EndoStim LES précédent
- Chirurgie antérieure impliquant la jonction gastro-œsophagienne ou le site d'implantation de la sonde, telle qu'une fundoplication de Nissen
- Intervention endoscopique antérieure pour le traitement du RGO et/ou de l'œsophage de Barrett
- Hernie hiatale supérieure à 3 cm déterminée par endoscopie
- Antécédents de gastroparésie
- Tout trouble de la motilité œsophagienne non RGO qui, de l'avis de l'investigateur, exclut une procédure anti-reflux
- Antécédents ou connaissance de sténose oesophagienne ou anomalies anatomiques oesophagiennes importantes (lésions obstructives, etc.)
- Œsophage de Barrett ou tout grade de dysplasie
- Antécédents documentés d'œsophagite Grade C ou D (Classification LA)
- Antécédents de cancer de l'œsophage ou de l'estomac suspecté ou confirmé
- Varices oesophagiennes ou gastriques
- Symptômes de dysphagie plus d'une fois par semaine chaque semaine au cours des 3 derniers mois
- Incapable de tolérer le sevrage des anti-H2 ou des médicaments IPP
- Allergies suspectées ou connues au titane, au platine, à l'iridium, à l'acier inoxydable, au silicone, à l'époxy ou au nylon
- Indice de masse corporelle (IMC) > 35 kg/m2
- Toute maladie multisystémique significative
- Maladie auto-immune ou du tissu conjonctif (sclérodermie, dermatomyosite, syndrome de Calcinose-Raynaud-œsophage sclérodactylie (CREST), syndrome de Sjögren, syndrome de Sharp, etc.) nécessitant un traitement au cours des 2 années précédentes
- Diabète sucré de type 1 ou diabète sucré de type 2 non contrôlé (T2DM) défini comme HbA1c > 9,5 au cours des 6 mois précédents ou au dépistage/au départ
- Arythmie cardiaque importante ou ectopie ou maladie cardiovasculaire importante (c.-à-d. angine de poitrine instable, maladie valvulaire hémodynamiquement significative, insuffisance cardiaque congestive sévère), ou toute intervention thérapeutique cardiaque au cours des 6 derniers mois.
- Événement cérébrovasculaire important au cours des 6 derniers mois
- Stimulateur électrique implanté existant (stimulateur cardiaque, défibrillateur automatique implantable, stimulateur cérébral profond (DBS), stimulateurs de croissance osseuse ou du plancher pelvien, pompes à médicaments, etc.)
- Sujet féminin en âge de procréer et qui est enceinte ou allaite, ou a l'intention de devenir enceinte pendant la période d'essai, qui n'utilise pas une forme fiable de contrôle des naissances
- Actuellement inscrit à d'autres recherches potentiellement source de confusion
- Infection active déterminée par l'investigateur
- Antécédents de toute tumeur maligne, autre que le carcinome basocellulaire, au cours des 2 dernières années
- Espérance de vie inférieure à 3 ans aa. Trouble psychiatrique majeur diagnostiqué (bipolaire, schizophrénie, etc.) bb. Toute condition qui, à la discrétion de l'investigateur ou du promoteur, interférerait avec une interprétation précise des critères d'évaluation de l'étude ou empêcherait la participation à l'essai

Comparateur factice: Control Group (delayed stimulation)
This group will undergo laparoscopic implantation surgery with or without crural repair (surgeon discretion).
The device will not be activated: Sham EndoStim stimulation for first six months of study.
It will be activated at the Month 6 visit and provide lower esophageal stimulation from Month 6 thru end of study.

Expérimental: Treatment Group (immediate stimulation)
The group will undergo laparoscopic implantation surgery with or without crural repair (surgeon discretion).
The device will be activated two weeks post-implantation and the subject will receive EndoStim stimulation for first six months of study and continue with EndoStim stimulation from Month 6 thru end of study.

The secondary efficacy endpoint was the number of subjects achieving GERD symptom success defined as an improvement in the composite GERD Health-related Quality of Life (GERD-HRQL) patient reported outcome questionnaire score of 50% or more after 12 months on stimulation compared to the baseline off-PPI or H2 blocker composite GERD-HRQL score.
Questions regarding heartburn severity and difficulty swallowing are asked on scale of 0 to 5: 0 = No symptoms; 1 = Symptoms noticeable but not bothersome; 2 = Symptoms noticeable and bothersome but not every day; 3 = Symptoms bothersome every day; 4 = Symptoms affect daily activities; and 5 = Symptoms are incapacitating - unable to do activities.
The composite score is calculated by adding the responses to the individual questions.
The minimum score possible is 0 and the maximum score is 45.

Questions regarding are asked on scale of 0 to 5: 0 = No symptoms; 1 = Symptoms noticeable but not bothersome; 2 = Symptoms noticeable and bothersome but not every day; 3 = Symptoms bothersome every day; 4 = Symptoms affect daily activities; and 5 = Symptoms are incapacitating - unable to do activities.
The composite score is calculated by adding the responses to the individual questions.
The minimum score possible is 0 and the maximum score is 45.
Previous versions of the protocol and case report forms did not include the question about bloating; thus, this question was not included in the calculation of the composite score.
The lower scores are associated with less impactful symptoms.
