Beschreibung
Einschlusskriterien:
- In der Lage und bereit, eine schriftliche Einverständniserklärung abzugeben
- In der Lage und bereit, die erforderlichen Studienverfahren und den Nachsorgeplan einzuhalten
- 22 - 75 Jahre zum Zeitpunkt der Einverständniserklärung
- Dokumentierte Symptome einer gastroösophagealen Refluxkrankheit (GERD) für mehr als 6 Monate (Regurgitation und/oder Sodbrennen, definiert als brennender epigastrischer oder substernaler Schmerz, der auf Säureneutralisierung oder -unterdrückung anspricht), was die tägliche Anwendung von Protonenpumpenhemmern (PPIs) oder anderen erfordert medikamentöse Anti-Reflux-Therapie, die trotz maximaler medikamentöser Therapie weiterhin Beschwerden haben oder „intolerant“ sind -schwere Nebenwirkungen (z.B. Anaphylaxie oder schwere allergische Reaktion, rezidivierende C. difficile, schwere Hypomagnesiämie) bis zu einem PPI oder leichte/mittelschwere Nebenwirkung (z. Übelkeit, Erbrechen, Durchfall oder Bauchschmerzen) gegen mindestens 2 PPI verschiedener chemischer Klassen.
- Symptomatische Verbesserung der PPI-Therapie, nachgewiesen durch einen zusammengesetzten GERD-gesundheitsbezogenen Lebensqualitäts-Score (HRQL) von ≥20 vom PPI und eine Verbesserung von ≥10 Punkten vom PPI im Vergleich zum zusammengesetzten GERD-HRQL-Score vom PPI. Patienten, die die obige Definition einer PPI-Intoleranz erfüllen, müssen keine Verbesserung um ≥ 10 Punkte aufweisen. Der On-PPI-Score zur Erfüllung dieses Kriteriums ist der Score aus der GERD-HRQL-Bewertung, die nach Wiederaufnahme der PPI nach dem Baseline-Besuch abgeschlossen wurde.
- Übermäßige Exposition gegenüber unterer Speiseröhrensäure während der pH-Überwachung (definiert als distaler Ösophagus-pH < 4 für > 6,0 % der Überwachungszeit), die nach mindestens 5 Tagen ohne PPIs und mindestens 2 Tagen ohne H2-Blocker durchgeführt wurde. Mindestens 18 Stunden Aufzeichnung des Ösophagus-pH-Werts werden als angemessen angesehen und die Aufnahme basiert auf dem Tag (mindestens 18 Stunden gültige Daten) mit der höchsten prozentualen Säureexpositionszeit.
- Ösophagitis ≤ Grad B (Klassifikation nach Los Angeles (LA)), gemessen durch obere Endoskopie ohne PPI und H2-Blocker für 10-14 Tage
- Kontraktionsamplitude des Ösophaguskörpers > 30 mmHg bei > 30 % der Schluckvorgänge und > 30 % peristaltische Kontraktionen bei HRM oder ≥ 30 % peristaltische Kontraktionen bei DCI >450.
- Geeigneter chirurgischer Kandidat, der sich einer Vollnarkose und einer laparoskopischen Operation unterziehen kann
Ausschlusskriterien:
- Frühere Implantation des EndoStim LES-Systems und/oder Implantationsversuch
- Vorheriger chirurgischer Eingriff am gastroösophagealen Übergang oder an der Führungsimplantatstelle, wie z. B. eine Nissen-Fundoplikatio
- Früherer endoskopischer Eingriff zur Behandlung von GERD und/oder Barrett-Ösophagus
- Hiatushernie größer als 3 cm, wie durch Endoskopie festgestellt
- Geschichte der Gastroparese
- Alle Nicht-GERD-Motilitätsstörungen des Ösophagus, die nach Meinung des Prüfarztes ein Antirefluxverfahren ausschließen
- Geschichte oder bekannte Ösophagusstriktur oder signifikante anatomische Anomalien der Speiseröhre (obstruktive Läsionen usw.)
- Barrett-Ösophagus oder jede Art von Dysplasie
- Dokumentierte Vorgeschichte von Ösophagitis Grad C oder D (LA-Klassifikation)
- Vorgeschichte von vermutetem oder bestätigtem Speiseröhren- oder Magenkrebs
- Ösophagus- oder Magenvarizen
- Symptome einer Dysphagie mehr als einmal pro Woche jede Woche innerhalb der letzten 3 Monate
- Kann den Entzug von H2-Blockern oder PPI-Medikamenten nicht tolerieren
- Vermutete oder bekannte Allergien gegen Titan, Platin, Iridium, Edelstahl, Silikon, Epoxid oder Nylon
- Body-Mass-Index (BMI) > 35 kg/m2
- Alle signifikanten Multisystemerkrankungen
- therapiebedürftige Autoimmun- oder Bindegewebserkrankung (Sklerodermie, Dermatomyositis, Calcinosis-Raynaud-Ösophagus-Sklerodaktylie-Syndrom (CREST), Sjögren-Syndrom, Sharp-Syndrom etc.) in den letzten 2 Jahren
- Typ-1-Diabetes mellitus oder unkontrollierter Typ-2-Diabetes mellitus (T2DM), definiert als HbA1c > 9,5 in den letzten 6 Monaten oder beim Screening/Baseline
- Signifikante Herzrhythmusstörungen oder Ektopie oder signifikante kardiovaskuläre Erkrankung (d. h. instabile Angina pectoris, hämodynamisch signifikante Herzklappenerkrankung, schwere dekompensierte Herzinsuffizienz) oder jegliche kardiologische Intervention innerhalb der letzten 6 Monate.
- Signifikantes zerebrovaskuläres Ereignis innerhalb der letzten 6 Monate
- Vorhandener implantierter Elektrostimulator (Herzschrittmacher, implantierbarer Kardioverter-Defibrillator, Tiefenhirnstimulator (DBS), Knochenwachstums- oder Beckenbodenstimulatoren, Medikamentenpumpen usw.)
- Weibliches Subjekt im gebärfähigen Alter, das schwanger ist oder stillt oder beabsichtigt, während des Testzeitraums schwanger zu werden, das keine zuverlässige Form der Empfängnisverhütung anwendet
- Derzeit in andere potenziell verwirrende Forschung eingeschrieben
- Aktive Infektion, wie vom Prüfarzt festgestellt
- Anamnese jeglicher Malignität, außer Basalzellkarzinom, in den letzten 2 Jahren

Schein-Komparator: Control Group (delayed stimulation)
This group will undergo laparoscopic implantation surgery with or without crural repair (surgeon discretion).
The device will not be activated: Sham EndoStim stimulation for first six months of study.
It will be activated at the Month 6 visit and provide lower esophageal stimulation from Month 6 thru end of study.

The group will undergo laparoscopic implantation surgery with or without crural repair (surgeon discretion).
The device will be activated two weeks post-implantation and the subject will receive EndoStim stimulation for first six months of study and continue with EndoStim stimulation from Month 6 thru end of study.

The secondary efficacy endpoint was the number of subjects achieving GERD symptom success defined as an improvement in the composite GERD Health-related Quality of Life (GERD-HRQL) patient reported outcome questionnaire score of 50% or more after 12 months on stimulation compared to the baseline off-PPI or H2 blocker composite GERD-HRQL score.
Questions regarding heartburn severity and difficulty swallowing are asked on scale of 0 to 5: 0 = No symptoms; 1 = Symptoms noticeable but not bothersome; 2 = Symptoms noticeable and bothersome but not every day; 3 = Symptoms bothersome every day; 4 = Symptoms affect daily activities; and 5 = Symptoms are incapacitating - unable to do activities.
The composite score is calculated by adding the responses to the individual questions.
The minimum score possible is 0 and the maximum score is 45.

Questions regarding are asked on scale of 0 to 5: 0 = No symptoms; 1 = Symptoms noticeable but not bothersome; 2 = Symptoms noticeable and bothersome but not every day; 3 = Symptoms bothersome every day; 4 = Symptoms affect daily activities; and 5 = Symptoms are incapacitating - unable to do activities.
The composite score is calculated by adding the responses to the individual questions.
The minimum score possible is 0 and the maximum score is 45.
Previous versions of the protocol and case report forms did not include the question about bloating; thus, this question was not included in the calculation of the composite score.
The lower scores are associated with less impactful symptoms.
