Het doel van dit onderzoek is om de veiligheid en effectiviteit van het stimulatiesysteem voor de onderste slokdarmsfincter (LES) bij de behandeling van gastro-oesofageale refluxziekte (GERD) aan te tonen. Dit onderzoek is een multicenter, gerandomiseerde, dubbelblinde, sham-gecontroleerde studie. Na de implantatieprocedure worden proefpersonen gerandomiseerd naar ofwel de behandelingsgroep (onmiddellijke stimulatie) ofwel de controlegroep (vertraagde stimulatie) gedurende zes maanden, gevolgd door een extra open-labelfase waarin alle proefpersonen elektrische stimulatie krijgen. Proefpersonen gaan door met stimulatiebehandeling in een verlengde open-label follow-upfase tot 5 jaar na stimulatie.

Gedetailleerde beschrijving

Het doel van dit onderzoek is om de veiligheid en effectiviteit aan te tonen van het EndoStim® stimulatiesysteem voor de onderste slokdarmsfincter (LES) bij de behandeling van proefpersonen met gastro-oesofageale refluxziekte (GERD). Dit onderzoek is een multicenter, gerandomiseerd, dubbelblind, schijngecontroleerd klinisch onderzoek. Na de implantatieprocedure worden proefpersonen gerandomiseerd naar de behandelingsgroep (onmiddellijke stimulatie) of de controlegroep (vertraagde stimulatie) gedurende een dubbelblinde fase van 6 maanden, gevolgd door een extra open-label behandelingsfase waarin alle proefpersonen elektrische stimulatietherapie gedurende in totaal 12 maanden. Inclusiecriteria:

  1. In staat en bereid om schriftelijke geïnformeerde toestemming te geven
  2. In staat en bereid om te voldoen aan de vereiste studieprocedures en het vervolgschema
  3. 22 - 75 jaar oud op het moment van geïnformeerde toestemming
  4. Gedocumenteerde symptomen van gastro-oesofageale refluxziekte (GORZ) gedurende meer dan 6 maanden (oprispingen en/of brandend maagzuur, gedefinieerd als brandende epigastrische of substernale pijn die reageert op zuurneutralisatie of -onderdrukking), waarvoor dagelijks gebruik van protonpompremmers (PPI's) of andere behandeling met refluxmedicatie, die ondanks maximale medische therapie symptomen blijven houden of "intolerant" zijn - ernstige bijwerkingen (bijv. anafylaxie of ernstige allergische reactie, recidiverende C. difficile, ernstige hypomagnesiëmie) op één PPI of milde/matige bijwerking (bijv. misselijkheid, braken, diarree of buikpijn) tot ten minste 2 PPI's van verschillende chemische klassen.
  5. Symptomatische verbetering op PPI-therapie aangetoond door een samengestelde GERD-gezondheidsgerelateerde kwaliteit van leven (HRQL)-score van ≥20 off PPI, en een verbetering van ≥10 punten op PPI in vergelijking met de off PPI samengestelde GERD-HRQL-score. Patiënten die voldoen aan de bovenstaande definitie van PPI-intolerantie hoeven geen verbetering van ≥10 punten te hebben. De on-PPI-score om aan dit criterium te voldoen, is de score van de GERD-HRQL-beoordeling die is voltooid na het hervatten van PPI's na het baselinebezoek.
  6. Overmatige lagere blootstelling aan slokdarmzuur tijdens pH-monitoring (gedefinieerd als distale slokdarm-pH < 4 gedurende > 6,0% van de monitoringtijd) uitgevoerd na ten minste 5 dagen zonder PPI's en ten minste 2 dagen zonder H2-blokkers. Ten minste 18 uur slokdarm-pH-registratie wordt als voldoende beschouwd en opname wordt gebaseerd op de dag (ten minste 18 uur aan geldige gegevens) met het hoogste zuurblootstellingspercentage.
  7. Oesofagitis ≤ Graad B (classificatie volgens Los Angeles (LA)) zoals gemeten door endoscopie van het bovenste deel van PPI- en H2-blokkers gedurende 10-14 dagen
  8. Contractieamplitude van het slokdarmlichaam > 30 mmHg voor > 30% van de slikbewegingen en > 30% peristaltische contracties op HRM of ≥ 30% peristaltische contracties met DCI >450.
  9. Geschikte chirurgische kandidaat die algemene anesthesie en laparoscopische chirurgie kan ondergaan

Uitsluitingscriteria:

  1. Eerdere implantatie van het EndoStim LES-systeem en/of poging tot implantatie
  2. Eerdere operatie waarbij de gastro-oesofageale overgang of de loden implantatieplaats betrokken was, zoals een Nissen-fundoplicatie
  3. Eerdere endoscopische interventie voor de behandeling van GORZ en/of Barrett-slokdarm
  4. Hiatale hernia groter dan 3 cm zoals bepaald door endoscopie
  5. Geschiedenis van gastroparese
  6. Elke niet-GERD slokdarmmotiliteitsstoornis die naar de mening van de onderzoeker een antirefluxprocedure uitsluit
  7. Geschiedenis van of bekende slokdarmvernauwing of significante anatomische slokdarmafwijkingen (obstructieve laesies, enz.)
  8. Barrett-slokdarm of enige graad van dysplasie
  9. Gedocumenteerde geschiedenis van oesofagitis Graad C of D (LA-classificatie)
  10. Geschiedenis van vermoedelijke of bevestigde slokdarm- of maagkanker
  11. Slokdarm- of maagvarices
  12. Symptomen van dysfagie meer dan eens per week elke week in de afgelopen 3 maanden
  13. Kan ontwenning van H2-blokkers of PPI-medicatie niet tolereren
  14. Vermoedelijke of bekende allergieën voor titanium, platina, iridium, roestvrij staal, siliconen, epoxy of nylon

  16. Alle significante multisysteemziekten
  17. Auto-immuunziekte of een bindweefselaandoening (sclerodermie, dermatomyositis, calcinose - Raynaud's-oesofagus sclerodactyliesyndroom (CREST), syndroom van Sjögren, syndroom van Sharp, enz.) waarvoor in de voorgaande 2 jaar therapie nodig was
  18. Diabetes mellitus type 1 of ongecontroleerde diabetes mellitus type 2 (T2DM) gedefinieerd als HbA1c > 9,5 in de voorgaande 6 maanden of bij screening/baseline
  19. Significante hartritmestoornissen of ectopie of significante hart- en vaatziekten (d.w.z. instabiele angina pectoris, hemodynamisch significante klepaandoening, ernstig congestief hartfalen), of enige andere cardiale therapeutische interventie in de afgelopen 6 maanden.
  20. Significante cerebrovasculaire gebeurtenis in de afgelopen 6 maanden

This group will undergo laparoscopic implantation surgery with or without crural repair (surgeon discretion). The device will not be activated: Sham EndoStim stimulation for first six months of study. It will be activated at the Month 6 visit and provide lower esophageal stimulation from Month 6 thru end of study.

Experimenteel: Treatment Group (immediate stimulation)
The group will undergo laparoscopic implantation surgery with or without crural repair (surgeon discretion). The device will be activated two weeks post-implantation and the subject will receive EndoStim stimulation for first six months of study and continue with EndoStim stimulation from Month 6 thru end of study.
