Descripción
Criterios de inclusión:
- Capaz y dispuesto a dar su consentimiento informado por escrito
- Capaz y dispuesto a cumplir con los procedimientos de estudio requeridos y el programa de seguimiento.
- 22 - 75 años de edad en el momento del consentimiento informado
- Síntomas documentados de enfermedad por reflujo gastroesofágico (ERGE) durante más de 6 meses (regurgitación y/o acidez estomacal que se define como dolor epigástrico o retroesternal quemante que responde a la neutralización o supresión del ácido) que requiere el uso diario de inhibidores de la bomba de protones (IBP) u otros terapia con medicamentos antirreflujo, que continúan teniendo síntomas a pesar de la terapia médica máxima o son "intolerantes" - efectos secundarios graves (p. anafilaxia o reacción alérgica grave, C. difficile recurrente, hipomagnesemia grave) a un IBP o efecto secundario leve/moderado (p. náuseas, vómitos, diarrea o dolor abdominal) a al menos 2 IBP de diferentes clases químicas.
- Mejoría sintomática con el tratamiento con IBP demostrada por una puntuación compuesta de calidad de vida relacionada con la salud (HRQL) de GERD de ≥20 sin PPI, y una mejora de ≥10 puntos con PPI en comparación con la puntuación compuesta de GERD-HRQL sin PPI. Los pacientes que cumplen con la definición anterior de intolerancia a los IBP no están obligados a tener una mejora de ≥10 puntos. La puntuación en PPI para satisfacer este criterio será la puntuación de la evaluación GERD-HRQL completada después de reanudar PPI después de la visita de referencia.
- Exposición ácida esofágica inferior excesiva durante la monitorización del pH (definida como pH esofágico distal < 4 durante > 6,0 % del tiempo de monitorización) realizada después de al menos 5 días sin IBP y al menos 2 días sin bloqueadores H2. Al menos 18 horas de registro del pH esofágico se considerarán adecuadas y la inclusión se basará en el día (al menos 18 horas de datos válidos) con el mayor porcentaje de tiempo de exposición al ácido.
- Esofagitis ≤ Grado B (clasificación de Los Ángeles (LA)) según lo medido por endoscopia superior sin PPI y bloqueadores H2 durante 10-14 días
- Amplitud de contracción del cuerpo esofágico > 30 mmHg para > 30 % de las degluciones y > 30 % de contracciones peristálticas en HRM o ≥ 30 % de contracciones peristálticas con DCI > 450.
- Candidato quirúrgico adecuado capaz de someterse a anestesia general y cirugía laparoscópica
Criterio de exclusión:
- Implante anterior del sistema EndoStim LES y/o intento de implante
- Cirugía previa que involucró la unión gastroesofágica o el lugar del implante del cable, como una fundoplicatura de Nissen
- Intervención endoscópica previa para el tratamiento de ERGE y/o esófago de Barrett
- Hernia de hiato mayor de 3 cm determinada por endoscopia
- Historia de la gastroparesia
- Cualquier trastorno de la motilidad esofágica que no sea ERGE que, en opinión del investigador, impida un procedimiento antirreflujo
- Antecedentes o estenosis esofágica conocida o anomalías anatómicas esofágicas significativas (lesiones obstructivas, etc.)
- Esófago de Barrett o cualquier grado de displasia
- Antecedentes documentados de esofagitis Grado C o D (Clasificación LA)
- Antecedentes de cáncer de esófago o gástrico sospechado o confirmado
- Várices esofágicas o gástricas
- Síntomas de disfagia más de una vez por semana cada semana en los últimos 3 meses
- Incapaz de tolerar la abstinencia de bloqueadores H2 o medicamentos IBP
- Alergias sospechadas o conocidas al titanio, platino, iridio, acero inoxidable, silicona, epoxi o nailon
- Índice de masa corporal (IMC) > 35 kg/m2
- Cualquier enfermedad multisistémica importante
- Trastorno autoinmune o del tejido conjuntivo (esclerodermia, dermatomiositis, calcinosis-Síndrome de esófago-esófago de Raynaud (CREST), síndrome de Sjogren, síndrome de Sharp, etc.) que requiere tratamiento en los 2 años anteriores
- Diabetes mellitus tipo 1 o diabetes mellitus tipo 2 no controlada (T2DM) definida como HbA1c > 9,5 en los 6 meses anteriores o en la selección/basal
- Arritmia cardíaca significativa o ectopia o enfermedad cardiovascular significativa (es decir, angina de pecho inestable, enfermedad valvular hemodinámicamente significativa, insuficiencia cardíaca congestiva grave), o cualquier intervención terapéutica cardíaca en los últimos 6 meses.
- Evento cerebrovascular significativo en los últimos 6 meses
- Estimulador eléctrico implantado existente (marcapasos, desfibrilador cardioversor implantable, estimulador cerebral profundo (DBS), estimuladores del crecimiento óseo o del suelo pélvico, bombas de fármacos, etc.)
- Sujeto femenino en edad fértil y está embarazada o amamantando, o tiene la intención de quedar embarazada durante el período de prueba, que no está usando una forma confiable de control de la natalidad
- Actualmente inscrito en otra investigación potencialmente confusora
- Infección activa según lo determinado por el investigador
- Antecedentes de cualquier malignidad, que no sea carcinoma de células basales, en los últimos 2 años
- Esperanza de vida inferior a 3 años aa. Trastorno psiquiátrico mayor diagnosticado (bipolar, esquizofrenia, etc.) bb. Cualquier condición que, a discreción del investigador o patrocinador, podría interferir con la interpretación precisa de los criterios de valoración del estudio o impedir la participación en el ensayo.

Comparador falso: Control Group (delayed stimulation)
This group will undergo laparoscopic implantation surgery with or without crural repair (surgeon discretion).
The device will not be activated: Sham EndoStim stimulation for first six months of study.
It will be activated at the Month 6 visit and provide lower esophageal stimulation from Month 6 thru end of study.

Experimental: Treatment Group (immediate stimulation)
The group will undergo laparoscopic implantation surgery with or without crural repair (surgeon discretion).
The device will be activated two weeks post-implantation and the subject will receive EndoStim stimulation for first six months of study and continue with EndoStim stimulation from Month 6 thru end of study.

Secondary Endpoint (Open-label, 12 Months on Stimulation): GERD-HRQL Score (Off-PPI/H2 Blocker)

The secondary efficacy endpoint was the number of subjects achieving GERD symptom success defined as an improvement in the composite GERD Health-related Quality of Life (GERD-HRQL) patient reported outcome questionnaire score of 50% or more after 12 months on stimulation compared to the baseline off-PPI or H2 blocker composite GERD-HRQL score.
Questions regarding heartburn severity and difficulty swallowing are asked on scale of 0 to 5: 0 = No symptoms; 1 = Symptoms noticeable but not bothersome; 2 = Symptoms noticeable and bothersome but not every day; 3 = Symptoms bothersome every day; 4 = Symptoms affect daily activities; and 5 = Symptoms are incapacitating - unable to do activities.
The composite score is calculated by adding the responses to the individual questions.
The minimum score possible is 0 and the maximum score is 45.

Questions regarding are asked on scale of 0 to 5: 0 = No symptoms; 1 = Symptoms noticeable but not bothersome; 2 = Symptoms noticeable and bothersome but not every day; 3 = Symptoms bothersome every day; 4 = Symptoms affect daily activities; and 5 = Symptoms are incapacitating - unable to do activities.
The composite score is calculated by adding the responses to the individual questions.
The minimum score possible is 0 and the maximum score is 45.
Previous versions of the protocol and case report forms did not include the question about bloating; thus, this question was not included in the calculation of the composite score.
The lower scores are associated with less impactful symptoms.
