Critério de inclusão:
- Capaz e disposto a fornecer consentimento informado por escrito
- Capaz e disposto a cumprir os procedimentos de estudo necessários e cronograma de acompanhamento
- 22 - 75 anos de idade no momento do consentimento informado
- Sintomas documentados de doença do refluxo gastroesofágico (DRGE) por mais de 6 meses (regurgitação e/ou azia, definida como dor epigástrica ou subesternal em queimação que responde à neutralização ou supressão ácida) que requer o uso diário de inibidores da bomba de prótons (IBPs) ou outros terapia medicamentosa anti-refluxo, que continuam a ter sintomas apesar da terapia médica máxima ou são "intolerantes" - efeitos colaterais graves (por exemplo, anafilaxia ou reação alérgica grave, C. difficile recorrente, hipomagnesemia grave) a um IBP ou efeito colateral leve/moderado (por exemplo, náusea, vômito, diarreia ou dor abdominal) a pelo menos 2 IBPs de diferentes classes químicas.
- Melhora sintomática na terapia com IBP demonstrada por uma pontuação composta de qualidade de vida relacionada à saúde (HRQL) de DRGE de ≥20 sem PPI e uma melhora de ≥10 pontos com PPI em comparação com a pontuação composta de DRGE-HRQL sem PPI. Os pacientes que atendem à definição acima de intolerante a IBP não precisam apresentar melhora ≥10 pontos. A pontuação no PPI para atender a este critério será a pontuação da avaliação GERD-HRQL concluída após a retomada dos PPIs após a visita de linha de base.
- Exposição excessiva ao ácido esofágico inferior durante o monitoramento do pH (definido como pH do esôfago distal < 4 para > 6,0% do tempo de monitoramento) realizado após pelo menos 5 dias sem IBPs e pelo menos 2 dias sem bloqueadores H2. Pelo menos 18 horas de registro do pH esofágico serão consideradas adequadas e a inclusão será baseada no dia (pelo menos 18 horas de dados válidos) com a maior porcentagem de tempo de exposição ácida.
- Esofagite ≤ Grau B (classificação de Los Angeles (LA)) conforme medido por endoscopia alta sem IBP e bloqueadores H2 por 10 a 14 dias
- Amplitude de contração do corpo esofágico > 30 mmHg para > 30% das deglutições e > 30% das contrações peristálticas na HRM ou ≥ 30% das contrações peristálticas com DCI >450.
- Candidato cirúrgico adequado capaz de se submeter a anestesia geral e cirurgia laparoscópica
Critério de exclusão:
- Implante anterior do sistema EndoStim LES e/ou tentativa de implante
- Cirurgia anterior envolvendo a junção gastroesofágica ou o local do implante do eletrodo, como fundoplicatura de Nissen
- Intervenção endoscópica prévia para tratamento de DRGE e/ou esôfago de Barrett
- Hérnia de hiato maior que 3 cm determinada por endoscopia
- História de gastroparesia
- Quaisquer distúrbios de motilidade esofágica não DRGE que, na opinião do investigador, impeçam um procedimento anti-refluxo
- História ou estenose esofágica conhecida ou anormalidades anatômicas esofágicas significativas (lesões obstrutivas, etc.)
- Esôfago de Barrett ou qualquer grau de displasia
- História documentada de esofagite Grau C ou D (Classificação LA)
- História de câncer de esôfago ou estômago suspeito ou confirmado
- Varizes esofágicas ou gástricas
- Sintomas de disfagia mais de uma vez por semana todas as semanas nos últimos 3 meses
- Incapaz de tolerar a retirada de bloqueadores H2 ou medicamentos PPI
- Alergias suspeitas ou conhecidas a titânio, platina, irídio, aço inoxidável, silicone, epóxi ou nylon
- Índice de massa corporal (IMC) > 35 kg/m2
- Qualquer doença multissistêmica significativa
- Doença autoimune ou do tecido conjuntivo (esclerodermia, dermatomiosite, Síndrome de Calcinose-Raynaud-Esôfago-Esclerodactilia (CREST), Síndrome de Sjögren, Síndrome de Sharp, etc.) que requer terapia nos últimos 2 anos
- Diabetes mellitus tipo 1 ou diabetes mellitus tipo 2 não controlado (T2DM) definido como HbA1c > 9,5 nos últimos 6 meses ou na triagem/basal
- Arritmia cardíaca significativa ou ectopia ou doença cardiovascular significativa (i.e. angina pectoris instável, doença valvular hemodinamicamente significativa, insuficiência cardíaca congestiva grave) ou qualquer intervenção terapêutica cardíaca nos últimos 6 meses.
- Evento cerebrovascular significativo nos últimos 6 meses
- Estimulador elétrico implantado existente (marca-passo, cardioversor desfibrilador implantável, estimulador cerebral profundo (DBS), estimuladores de crescimento ósseo ou do assoalho pélvico, bombas de drogas, etc.)
- Sujeito do sexo feminino com potencial para engravidar e está grávida ou amamentando, ou pretende engravidar durante o período experimental, que não está usando uma forma confiável de controle de natalidade
- Atualmente inscrito em outras pesquisas potencialmente confusas
- Infecção ativa conforme determinado pelo investigador
- História de qualquer malignidade, exceto carcinoma basocelular, nos últimos 2 anos
- Esperança de vida inferior a 3 anos aa. Transtorno psiquiátrico maior diagnosticado (bipolar, esquizofrenia, etc.) bb. Qualquer condição que, a critério do investigador ou patrocinador, interfira na interpretação precisa dos desfechos do estudo ou impeça a participação no estudo

Comparador Falso: Control Group (delayed stimulation)
This group will undergo laparoscopic implantation surgery with or without crural repair (surgeon discretion).
The device will not be activated: Sham EndoStim stimulation for first six months of study.
It will be activated at the Month 6 visit and provide lower esophageal stimulation from Month 6 thru end of study.

Experimental: Treatment Group (immediate stimulation)
The group will undergo laparoscopic implantation surgery with or without crural repair (surgeon discretion).
The device will be activated two weeks post-implantation and the subject will receive EndoStim stimulation for first six months of study and continue with EndoStim stimulation from Month 6 thru end of study.

Secondary Endpoint (Open-label, 12 Months on Stimulation): GERD-HRQL Score (Off-PPI/H2 Blocker)

The secondary efficacy endpoint was the number of subjects achieving GERD symptom success defined as an improvement in the composite GERD Health-related Quality of Life (GERD-HRQL) patient reported outcome questionnaire score of 50% or more after 12 months on stimulation compared to the baseline off-PPI or H2 blocker composite GERD-HRQL score.
Questions regarding heartburn severity and difficulty swallowing are asked on scale of 0 to 5: 0 = No symptoms; 1 = Symptoms noticeable but not bothersome; 2 = Symptoms noticeable and bothersome but not every day; 3 = Symptoms bothersome every day; 4 = Symptoms affect daily activities; and 5 = Symptoms are incapacitating - unable to do activities.
The composite score is calculated by adding the responses to the individual questions.
The minimum score possible is 0 and the maximum score is 45.

Questions regarding are asked on scale of 0 to 5: 0 = No symptoms; 1 = Symptoms noticeable but not bothersome; 2 = Symptoms noticeable and bothersome but not every day; 3 = Symptoms bothersome every day; 4 = Symptoms affect daily activities; and 5 = Symptoms are incapacitating - unable to do activities.
The composite score is calculated by adding the responses to the individual questions.
The minimum score possible is 0 and the maximum score is 45.
Previous versions of the protocol and case report forms did not include the question about bloating; thus, this question was not included in the calculation of the composite score.
The lower scores are associated with less impactful symptoms.
