Thérapie génique par cellules souches pour la cystinose

Description détaillée
La cystinose est une maladie récessive héréditaire rare appartenant à la famille des troubles du stockage lysosomal et se caractérise par une accumulation lysosomale de cystine dans toutes les cellules du corps entraînant une défaillance multiviscérale. La cystinose a un impact dévastateur sur les personnes touchées, principalement les enfants et les jeunes adultes, même avec un traitement à la cystéamine. La prévalence de la cystinose est de 1 sur 100 000 à 1 sur 200 000. Le gène impliqué dans la cystinose est le gène CTNS qui code pour le transporteur lysosomal transmembranaire de la cystine - la cystinosine. La norme de soins actuelle n'empêche pas la progression de la maladie et a un impact significatif sur la qualité de vie des patients atteints de cystinose.
Pour cette étude, jusqu'à 6 sujets répondant aux critères d'éligibilité seront transplantés selon un plan de traitement échelonné à 3 cohortes avec 2 sujets par cohorte. Les 2 premières cohortes seront composées de 4 adultes (18 ans ou plus), éventuellement suivies d'une cohorte composée de 2 adolescents ou adultes (> 14 ans). Après le processus de consentement éclairé, les sujets inscrits seront sélectionnés pour confirmer leur pleine éligibilité à la participation. Les sujets éligibles subiront une mobilisation et une collecte de cellules souches hématopoïétiques (CSH) (leucaphérèse). Une partie des cellules sera conservée comme "sauvegarde" à des fins de sauvetage si nécessaire, et une partie sera modifiée ex vivo avec un vecteur lentiviral, pCCL-CTNS ou pCDY.EFS.CTNS.T260I, pour exprimer le gène CTNS (nom du produit : CTNS-RD-04). La fabrication clinique pour les patients de la cohorte 3 introduira un activateur de transduction LentiBOOST (le nom du produit pour ces patients sera CTNS-RD-04-LB, où le suffixe "-LB" signifie LentiBOOST). Les sujets recevront une cytoréduction médullaire avec du busulfan avant la perfusion de CTNS-RD-04. Les sujets interrompront le traitement à la cystéamine pendant la période d'évaluation. La période de suivi de l'évaluation comprendra 2 premières années d'évaluations actives des points finaux, où les sujets seront évalués à 3, 6, 9, 12, 18 et 24 mois après la transplantation. Une étude de suivi à long terme (LTFU) pour une période de suivi totale de 15 ans sera proposée à tous les sujets.
Les objectifs de cette étude clinique de Phase 1/2 sont d'évaluer l'innocuité/la tolérance de CTNS-RD-04, et son efficacité à travers un certain nombre d'évaluations cliniques, moléculaires et biochimiques.

Critère d'intégration:
Les critères suivants doivent être remplis par tous les sujets considérés pour la participation à l'étude.
- Cohortes 1 et 2 : le sujet masculin ou féminin est âgé de ≥ 18 ans.
- Cohorte 3 : Le sujet masculin ou féminin est âgé de ≥ 14 ans.
- Le sujet a reçu un diagnostic de cystinose, c'est-à-dire l'apparition précoce du syndrome de Fanconi et des antécédents de taux élevé de cystine dans les globules blancs et/ou des antécédents ou la présence de cristaux de cystine dans l'œil.
- Le sujet a un statut de performance Karnofsky ou une performance Lansky en fonction de l'âge ≥ 60.
- Si le sujet a subi une greffe de rein, il doit être au moins un an après la greffe de rein.
Le sujet a une fonction hématologique adéquate :
- Nombre absolu de neutrophiles (ANC) ≥ 1,5 x 1000/mm^3
- Numération plaquettaire ≥ 100 x 1000/mm^3
- Hémoglobine ≥ 9,0 g/dL
Le sujet a une fonction hépatique adéquate :
- Bilirubine ≤ 2,0 mg/dL
- ALT ≤ 3 x la limite supérieure de la normale (LSN) U/L de l'établissement
Le sujet a une fonction rénale adéquate :
- Créatinine sérique <2x LSN mg/dL
- Clairance de la créatinine ≥ 50 mL/min/1,73 m^2
Le sujet a une coagulation adéquate :
- PT/aPTT ≤ 1,2 x LSN secondes
- RIN ≤ 2
Le sujet a une fonction thyroïdienne adéquate (avec ou sans thérapie de remplacement de la thyroïde) :
- TSH 0,27-4,2 mUI/mL
- T4 totale ≤ 2 x LSN mcg/dL
S'il s'agit d'une femme : une femme en âge de procréer (c'est-à-dire non stérile chirurgicalement [ligature des trompes, hystérectomie ou ovariectomie bilatérale] ou pas au moins 2 ans naturellement ménopausée) accepte de rester sexuellement abstinente ou d'utiliser la même forme acceptable de contraception hautement efficace depuis le dépistage jusqu'à deux ans après la greffe.
Les formes de contraception acceptables pour cette étude incluent les contraceptifs hormonaux (oral, implant, patch transdermique ou injection) associés à une inhibition de l'ovulation à dose stable pendant au moins 3 mois avant le dépistage, barrière (préservatif avec spermicide, diaphragme avec spermicide) , un dispositif intra-utérin ou un partenaire qui a subi une vasectomie pendant au moins 6 mois et qui a documenté une évaluation médicale du succès chirurgical d'une vasectomie.
Remarque : les mâles atteints de cystinose sont stériles.
- S'il s'agit d'un homme : les hommes doivent accepter de rester abstinents sexuellement ou d'utiliser une forme acceptable de contraception très efficace depuis le dépistage jusqu'à deux ans après la greffe.
- Le sujet est disposé et capable de se conformer aux restrictions et aux exigences de l'étude.
- Le sujet est disposé à fournir un consentement éclairé écrit / autorisation / assentiment avant de participer à l'étude.
- Le sujet doit être disposé à s'abstenir de donner du sperme après avoir reçu le régime de conditionnement. Pour les sujets prévoyant (ou pour lesquels il existe une possibilité) d'avoir des enfants dans le futur, une banque de sperme avant l'administration du régime de conditionnement sera recommandée.
- Le sujet doit être disposé à s'abstenir de donner du sang, des organes, des tissus ou des cellules pour la transplantation à partir de 30 jours avant le dépistage jusqu'à tout moment après le traitement CTNS-RD-04.
- Le sujet doit être disposé et doit être en mesure (selon le jugement de l'investigateur) d'interrompre son traitement à la cystéamine (gouttes orales et/ou oculaires).
Critère d'exclusion:
- - Le sujet a une infection bactérienne, virale ou fongique active, non contrôlée et aiguë lors du dépistage ou dans les 30 jours précédant le début du régime de conditionnement.
Le sujet a une sérologie positive lors du dépistage pour l'un des éléments suivants :
- Virus de l'immunodéficience humaine (VIH) 1-2
- Virus lymphotrope humain à cellules T (HTLV) - I/II
- Core de l'hépatite B et PCR de l'hépatite B positif

- Le sujet a un trouble d'immunodéficience cliniquement significatif connu.
- Le sujet est une femme en âge de procréer qui allaite, planifie une grossesse ou a un test de grossesse sérique positif.
- - Le sujet a déjà reçu une greffe de moelle ou de cellules souches ou prévoit d'en recevoir une dans les 90 jours suivant le début de l'étude.
- - Le sujet a eu un trouble hémorragique actif dans les 90 jours précédant le dépistage OU nécessite un traitement anticoagulant avant le traitement par thérapie génique ex vivo.
- - Le sujet a une malignité active ou des antécédents de malignité, y compris un lymphome (à l'exception du cancer primitif cutané basocellulaire ou épidermoïde traité de manière appropriée avant la transplantation).
- - Le sujet a une insuffisance rénale terminale (définie comme un DFG <15 ml/min) et est déjà sur une liste de transplantation ou qui peut envisager de s'inscrire pour une greffe de rein dans les 90 jours suivant le début de l'étude.
- Le sujet a une fonction pulmonaire altérée (sur la base d'un CVF ou d'un VEMS < 50 % prévu, ou d'un rapport VEMS/CVF inférieur à la valeur seuil normale spécifique à l'âge et au sexe).
Le sujet a une fonction cardiaque altérée dans les 90 jours précédant le dépistage, y compris l'un des éléments suivants :
- Infarctus du myocarde
- Électrocardiogramme (ECG) anormal cliniquement significatif
- Fraction d'éjection < 40%
- Arythmie incontrôlée
- Autre maladie cardiaque cliniquement significative (par exemple, insuffisance cardiaque congestive, hypertension non contrôlée, antécédents d'hypertension labile).
- Le sujet a un trouble médical grave ou incontrôlé (par exemple, une pancréatite, une maladie hépatique grave, un diabète sucré instable) qui, de l'avis des investigateurs, nuirait à sa capacité à recevoir le traitement de l'étude et à suivre les procédures de l'étude.
- Le sujet a des antécédents de réactions allergiques attribuées à des composés de composition chimique ou biologique similaire à Busulfan ou une allergie ou une contre-indication à l'utilisation d'autres agents utilisés dans l'étude, y compris l'iohexol, l'acide-citrate-dextrose de formule A (ACDA), le G-CSF ou plérixafor.

- - Le sujet reçoit actuellement ou prévoit de recevoir un autre agent expérimental, dispositif ou procédure à partir de 30 jours avant le dépistage jusqu'à la fin de l'étude.
- Le sujet a une condition, de l'avis de l'investigateur, qui compromet le respect des exigences de l'étude.

Expérimental: Gene Therapy with CTNS-RD-04 (including product manufactured with and without LentiBOOST)
This is a single-arm, open-label study without randomization.
Eligible subjects received CTNS-RD-04, an autologous CD34+ hematopoietic stem cell product transduced ex vivo with a lentiviral vector encoding CTNS.
During the study, manufacturing incorporated a transduction enhancer (LentiBOOST) in later participants.

Cryopreserved autologous CD34+ enriched hematopoietic stem/progenitor cells collected from mobilized peripheral blood and transduced ex vivo with a self-inactivating lentiviral vector (pCCL-CTNS or pCDY.EFS.CTNS.T260I) encoding the human CTNS complementary deoxyribonucleic acid (cDNA) sequence.
In later participants, a transduction enhancer (LentiBOOST) was incorporated into the manufacturing process.
