Stamcelgentherapie voor cystinose
Een fase 1/2-onderzoek om de veiligheid en werkzaamheid van transplantatie met autologe menselijke CD34+ hematopoëtische stamcellen (HSC) te bepalen uit gemobiliseerde perifere bloedstamcellen (PBSC) van patiënten met cystinose gemodificeerd door ex vivo transductie met behulp van pCCL-CTNS of pCDY.EFS. Cystinose is een zeldzame erfelijke recessieve ziekte die behoort tot de familie van lysosomale stapelingsziekten en wordt gekenmerkt door lysosomale accumulatie van cystine in alle cellen van het lichaam, wat leidt tot multi-orgaanfalen. Cystinose heeft een verwoestende impact op de getroffen personen, voornamelijk kinderen en jonge volwassenen, zelfs met behandeling met cysteamine. De prevalentie van cystinose is 1 op 100.000 tot 1 op 200.000. Het gen dat betrokken is bij cystinose is het gen CTNS dat codeert voor de transmembraan lysosomale cystinetransporter - cystinosine. De huidige zorgstandaard voorkomt de progressie van de ziekte niet en heeft een aanzienlijke invloed op de kwaliteit van leven van patiënten met cystinose.
Voor deze studie zullen maximaal 6 proefpersonen die aan de geschiktheidscriteria voldoen, worden getransplanteerd volgens een 3-cohort gespreid behandelingsontwerp met 2 proefpersonen per cohort. De eerste 2 cohorten zullen bestaan uit 4 volwassenen (18 jaar of ouder), eventueel gevolgd door een cohort bestaande uit 2 adolescenten of volwassenen (> 14 jaar). Na het geïnformeerde toestemmingsproces zullen ingeschreven proefpersonen worden gescreend om te bevestigen dat ze volledig in aanmerking komen voor deelname. In aanmerking komende proefpersonen ondergaan mobilisatie en verzameling van hematopoëtische stamcellen (HSC) (leukaferese). Een deel van de cellen zal worden bewaard als "back-up" voor reddingsdoeleinden indien nodig, en een deel zal ex vivo gen-gemodificeerd worden met een lentivirale vector, pCCL-CTNS of pCDY.EFS.CTNS.T260I, om het CTNS-gen tot expressie te brengen (productnaam: CTNS-RD-04). Klinische productie voor patiënten in Cohort 3 zal een transductieversterker LentiBOOST introduceren (de productnaam voor deze patiënten zal CTNS-RD-04-LB zijn, waarbij het achtervoegsel "-LB" staat voor LentiBOOST). De proefpersonen zullen vóór de infusie van CTNS-RD-04 mergcytoreductie met busulfan ondergaan. Proefpersonen zullen tijdens de beoordelingsperiode stoppen met de behandeling met cysteamine. De follow-upperiode van de beoordeling omvat in eerste instantie 2 jaar actieve eindpuntevaluaties, waarbij de proefpersonen 3, 6, 9, 12, 18 en 24 maanden na de transplantatie worden geëvalueerd. Aan alle proefpersonen wordt een langetermijnvervolgonderzoek (LTFU) voor een totale follow-upperiode van 15 jaar aangeboden.
De doelstellingen van deze klinische fase 1/2-studie zijn het beoordelen van de veiligheid/verdraagbaarheid van CTNS-RD-04 en de doeltreffendheid ervan door middel van een aantal klinische, moleculaire en biochemische beoordelingen.

Inclusiecriteria:
Aan de volgende criteria moet worden voldaan door alle vakken die in aanmerking komen voor deelname aan het onderzoek.
- Cohorten 1 en 2: mannelijke of vrouwelijke proefpersoon is ≥ 18 jaar.
- Cohort 3: mannelijke of vrouwelijke proefpersoon is ≥ 14 jaar.
- Patiënt wordt gediagnosticeerd met cystinose, d.w.z. vroeg begin van Fanconi-syndroom, en geschiedenis van verhoogde cystinespiegels in witte bloedcellen en/of geschiedenis van of aanwezigheid van cystinekristallen in het oog.
- Proefpersoon heeft een Karnofsky Performance Status of leeftijdsafhankelijke Lansky Performance van ≥ 60.
- Als de proefpersoon een niertransplantatie heeft gehad, moet hij of zij ten minste één jaar post-niertransplantatiestatus hebben.
Proefpersoon heeft een adequate hematologische functie:
- Absoluut aantal neutrofielen (ANC) ≥ 1,5 x 1000/mm^3
- Aantal bloedplaatjes ≥ 100 x 1000/mm^3
- Hemoglobine ≥ 9,0 g/dl
Proefpersoon heeft een adequate leverfunctie:
- Bilirubine ≤ 2,0 mg/dl
- ALAT ≤ 3 x de bovengrens van normaal (ULN) U/L van de instelling
Proefpersoon heeft een adequate nierfunctie:
- Serumcreatinine <2x ULN mg/dL
- Creatinineklaring ≥ 50 ml/min/1,73 m^2
Proefpersoon heeft voldoende stolling:
- PT/aPTT ≤ 1,2 x ULN seconden
- INR ≤ 2
Proefpersoon heeft een adequate schildklierfunctie (met of zonder schildkliervervangingstherapie):
- TSH 0,27-4,2 mIU/ml
- Totaal T4 ≤ 2 x ULN mcg/dL
Indien vrouw: vrouw die zwanger kan worden (d.w.z. niet chirurgisch steriel [tubaligatie, hysterectomie of bilaterale ovariëctomie] of niet ten minste 2 jaar natuurlijke postmenopauzale) ermee instemt seksueel onthouding te blijven of dezelfde aanvaardbare vorm van zeer effectieve anticonceptie te gebruiken vanaf screening tot en met twee jaar na transplantatie.
De aanvaardbare vormen van anticonceptie voor dit onderzoek zijn onder meer hormonale anticonceptiemiddelen (oraal, implantaat, transdermale pleister of injectie) geassocieerd met remming van de ovulatie bij een stabiele dosis gedurende ten minste 3 maanden voorafgaand aan de screening, barrière (condoom met zaaddodend middel, pessarium met zaaddodend middel) , spiraaltje, of een partner die gedurende ten minste 6 maanden gevasectomiseerd is en een gedocumenteerde medische beoordeling heeft van het chirurgisch succes van een vasectomie.
Let op: reuen met cystinose zijn steriel.
- Indien man: mannen moeten ermee instemmen seksueel onthouding te blijven of een aanvaardbare vorm van zeer effectieve anticonceptie te gebruiken vanaf de screening tot en met twee jaar na de transplantatie.
- Proefpersoon is bereid en in staat om te voldoen aan de studiebeperkingen en -eisen.
- De proefpersoon is bereid schriftelijke geïnformeerde toestemming/toestemming/instemming te geven voorafgaand aan deelname aan het onderzoek.
- De proefpersoon moet bereid zijn af te zien van het doneren van sperma na ontvangst van het conditioneringsregime. Voor proefpersonen die plannen hebben (of voor wie er een mogelijkheid is om) in de toekomst kinderen te verwekken, zal het opslaan van sperma voorafgaand aan toediening van het conditioneringsregime worden aanbevolen.
- De proefpersoon moet bereid zijn af te zien van het doneren van bloed, organen, weefsels of cellen voor transplantatie vanaf 30 dagen voorafgaand aan de screening tot en met elk moment na behandeling met CTNS-RD-04.
- Proefpersoon moet bereid en in staat zijn (naar het oordeel van de onderzoeker) om zijn of haar cysteaminetherapie (oraal en/of oogdruppel) te staken.
Uitsluitingscriteria:
- Proefpersoon heeft een actieve, ongecontroleerde, acute bacteriële, virale of schimmelinfectie tijdens de screening of binnen 30 dagen voorafgaand aan het starten van het conditioneringsregime.
Proefpersoon heeft positieve serologie bij screening voor een van de volgende:
- Humaan Immunodeficiëntie Virus (HIV) 1-2
- Humaan T-cel lymfotroop virus (HTLV) - I/II
- Hepatitis B kern en Hepatitis B PCR positief
- Hepatitis C-virus (HCV)
- Rapid Plasma Reagin (RPR)
- Ziekte van Chagas (T. curzi)
- QuantiferonTB
- Nucleïnezuurtest (NAT) voor HIV
- West-Nijlvirus (WNV)
- Proefpersoon heeft een bekende klinisch significante immunodeficiëntiestoornis.
- Onderwerp is een vrouw in de vruchtbare leeftijd die borstvoeding geeft, een zwangerschap plant of een positieve serumzwangerschapstest heeft.
- Proefpersoon heeft eerder een merg- of stamceltransplantatie ondergaan of is van plan er binnen 90 dagen na aanvang van de studie een te ontvangen.
- Proefpersoon heeft binnen 90 dagen voorafgaand aan screening een actieve bloedingsstoornis gehad OF heeft antistollingstherapie nodig voorafgaand aan behandeling met ex vivo gentherapie.
- Proefpersoon heeft een actieve maligniteit of een voorgeschiedenis van maligniteit, waaronder lymfoom (behalve primaire, cutane basaalcel- of plaveiselcelkanker die voorafgaand aan transplantatie op de juiste manier is behandeld).
- Proefpersoon heeft een nierziekte in het eindstadium (gedefinieerd als GFR <15 ml/min) en staat al op een transplantatielijst of is van plan om zich binnen 90 dagen na aanvang van de studie in te schrijven voor een niertransplantatie.
- Proefpersoon heeft een verminderde longfunctie (gebaseerd op FVC of FEV1 van <50% voorspeld, of een FEV1/FVC-ratio lager dan leeftijds- en geslachtsspecifieke normale drempelwaarde).
Proefpersoon heeft binnen 90 dagen voorafgaand aan de screening een verminderde hartfunctie, waaronder een van de volgende:
- Myocardinfarct
- Klinisch significant abnormaal elektrocardiogram (ECG)
- Ejectiefractie van < 40%
- Ongecontroleerde aritmie
- Andere klinisch significante hartaandoeningen (bijv. congestief hartfalen, ongecontroleerde hypertensie, voorgeschiedenis van labiele hypertensie).
- Proefpersoon heeft een ernstige of ongecontroleerde medische stoornis (bijv. pancreatitis, ernstige leverziekte, instabiele diabetes mellitus) die, naar de mening van de onderzoekers, hun vermogen om een studiebehandeling te krijgen en de studieprocedures te volgen, zou aantasten.
- Proefpersoon heeft een voorgeschiedenis van allergische reacties die worden toegeschreven aan verbindingen met een vergelijkbare chemische of biologische samenstelling als Busulfan of allergie of contra-indicatie voor het gebruik van andere middelen die in het onderzoek zijn gebruikt, waaronder iohexol, zuur-citraat-dextrose Formule A (ACDA), G-CSF of plerixafor.
- Onderwerp heeft een bekende geschiedenis van drugs- of alcoholverslaving.
- Proefpersoon heeft een grote operatie ondergaan binnen 90 dagen (of langer indien niet volledig hersteld) voorafgaand aan de screening.
- Proefpersoon ontvangt cytotoxische of immunosuppressieve middelen, anders dan voor niertransplantatie, binnen 60 dagen voorafgaand aan screening of moet met dergelijke middelen worden behandeld voorafgaand aan behandeling met ex vivo gentherapie.
- Proefpersoon heeft ooit eerder gentherapie gekregen.
- De proefpersoon ontvangt momenteel of verwacht een ander onderzoeksmiddel, apparaat of procedure vanaf 30 dagen voorafgaand aan de screening tot en met de voltooiing van de studie.
- De proefpersoon heeft naar de mening van de onderzoeker een aandoening die de naleving van de studievereisten in gevaar brengt.

Experimenteel: Gene Therapy with CTNS-RD-04 (including product manufactured with and without LentiBOOST)
This is a single-arm, open-label study without randomization.
Eligible subjects received CTNS-RD-04, an autologous CD34+ hematopoietic stem cell product transduced ex vivo with a lentiviral vector encoding CTNS.
During the study, manufacturing incorporated a transduction enhancer (LentiBOOST) in later participants.

Cryopreserved autologous CD34+ enriched hematopoietic stem/progenitor cells collected from mobilized peripheral blood and transduced ex vivo with a self-inactivating lentiviral vector (pCCL-CTNS or pCDY.EFS.CTNS.T260I) encoding the human CTNS complementary deoxyribonucleic acid (cDNA) sequence.
In later participants, a transduction enhancer (LentiBOOST) was incorporated into the manufacturing process.

Vector copy number (VCN) in peripheral blood cells collected at approximately 12 months and approximately 24 months post-transplant to assess engraftment and transduction efficiency of gene-modified hematopoietic stem cells.
VCN was measured by droplet digital PCR (ddPCR).
Results are summarized as mean (SD) across dosed participants with evaluable data at each timepoint.
