Detaillierte Beschreibung

Cystinose ist eine seltene rezessive Erbkrankheit, die zur Familie der lysosomalen Speicherkrankheiten gehört und durch eine lysosomale Ansammlung von Cystin in allen Zellen des Körpers gekennzeichnet ist, was zu Multiorganversagen führt. Cystinose hat verheerende Auswirkungen auf die betroffenen Personen, vor allem Kinder und junge Erwachsene, selbst bei Behandlung mit Cysteamin. Die Prävalenz der Cystinose beträgt 1 zu 100.000 bis 1 zu 200.000. Das an Cystinose beteiligte Gen ist das Gen CTNS, das für den transmembranen lysosomalen Cystintransporter – Cystinosin – kodiert. Der derzeitige Behandlungsstandard verhindert nicht das Fortschreiten der Erkrankung und beeinträchtigt die Lebensqualität von Patienten mit Cystinose erheblich.

Für diese Studie werden bis zu 6 Probanden, die die Eignungskriterien erfüllen, nach einem gestaffelten 3-Kohorten-Behandlungsdesign mit 2 Probanden pro Kohorte transplantiert. Die ersten 2 Kohorten bestehen aus 4 Erwachsenen (18 Jahre oder älter), möglicherweise gefolgt von einer Kohorte bestehend aus 2 Jugendlichen oder Erwachsenen (> 14 Jahre). Nach dem Einwilligungsverfahren werden die eingeschriebenen Probanden überprüft, um die volle Berechtigung zur Teilnahme zu bestätigen. Geeignete Probanden werden einer Mobilisierung und Entnahme hämatopoetischer Stammzellen (HSC) unterzogen (Leukapherese). Ein Teil der Zellen wird bei Bedarf als „Reserve“ für Rettungszwecke aufbewahrt, und ein Teil wird ex vivo mit einem lentiviralen Vektor, pCCL-CTNS oder pCDY.EFS.CTNS.T260I, genmodifiziert, um das CTNS-Gen zu exprimieren (Produktname: CTNS-RD-04). Die klinische Herstellung für Patienten in Kohorte 3 wird einen Transduktionsverstärker LentiBOOST einführen (der Produktname für diese Patienten lautet CTNS-RD-04-LB, wobei das Suffix „-LB“ für LentiBOOST steht). Einschlusskriterien:

Die folgenden Kriterien müssen von allen für die Studienteilnahme in Frage kommenden Probanden erfüllt werden.

  1. Kohorten 1 und 2: Der männliche oder weibliche Proband ist ≥ 18 Jahre alt.
  2. Kohorte 3: Männliches oder weibliches Subjekt ist ≥ 14 Jahre alt.
  3. Beim Subjekt wird Cystinose diagnostiziert, d. H. Frühes Einsetzen des Fanconi-Syndroms, und Vorgeschichte eines erhöhten Cystinspiegels der weißen Blutkörperchen und/oder Vorgeschichte oder Vorhandensein von Cystinkristallen im Auge.

  7. Das Subjekt hat eine angemessene Leberfunktion:

    1. Bilirubin ≤ 2,0 mg/dL

    Zu den akzeptablen Formen der Empfängnisverhütung für diese Studie gehören hormonelle Kontrazeptiva (oral, implantiert, transdermales Pflaster oder Injektion), die mit einer Hemmung des Eisprungs bei einer stabilen Dosis für mindestens 3 Monate vor dem Screening verbunden sind, Barriere (Kondom mit Spermizid, Diaphragma mit Spermizid) , Intrauterinpessar oder ein Partner, der seit mindestens 6 Monaten vasektomiert ist und eine dokumentierte medizinische Beurteilung des chirurgischen Erfolgs einer Vasektomie hat.

    Hinweis: Männer mit Cystinose sind unfruchtbar.

  12. Wenn männlich: Männer müssen zustimmen, sexuell abstinent zu bleiben oder eine akzeptable Form einer hochwirksamen Empfängnisverhütung vom Screening bis zwei Jahre nach der Transplantation anzuwenden.
  13. Der Proband ist bereit und in der Lage, die Studienbeschränkungen und -anforderungen einzuhalten.
  14. Der Proband ist bereit, vor der Teilnahme an der Studie eine schriftliche Einverständniserklärung/Erlaubnis/Zustimmung abzugeben.
  15. Das Subjekt muss bereit sein, nach Erhalt des Konditionierungsschemas auf die Spende von Sperma zu verzichten. Für Probanden, die planen (oder für die die Möglichkeit besteht), in Zukunft Kinder zu zeugen, wird eine Samenbank vor der Verabreichung des Konditionierungsschemas empfohlen.
  16. Das Subjekt muss bereit sein, Blut, Organe, Gewebe oder Zellen für die Transplantation ab 30 Tage vor dem Screening bis zu keinem Zeitpunkt nach der CTNS-RD-04-Behandlung zu spenden.
  17. Der Proband muss bereit und in der Lage sein (nach Einschätzung des Prüfarztes), seine Cysteamin-Therapie (oral und/oder Augentropfen) abzubrechen.

Ausschlusskriterien:

  1. Das Subjekt hat während des Screenings oder innerhalb von 30 Tagen vor Beginn des Konditionierungsschemas eine aktive, unkontrollierte, akute bakterielle, virale oder Pilzinfektion.
  2. Das Subjekt hat eine positive Serologie beim Screening auf eines der folgenden:

    9. West-Nil-Virus (WNV)
  3. Das Subjekt hat eine bekannte klinisch signifikante Immunschwächekrankheit.
  4. Das Subjekt ist eine Frau im gebärfähigen Alter, die stillt, eine Schwangerschaft plant oder einen positiven Serum-Schwangerschaftstest hat.
  5. Der Proband hat zuvor eine Knochenmark- oder Stammzelltransplantation erhalten oder plant, innerhalb von 90 Tagen nach Studienbeginn eine zu erhalten.
  6. Das Subjekt hatte innerhalb von 90 Tagen vor dem Screening eine aktive Blutungsstörung ODER benötigt vor der Behandlung mit einer Ex-vivo-Gentherapie eine Antikoagulationstherapie.

  16. Das Subjekt hat zuvor zu irgendeinem Zeitpunkt eine Gentherapie erhalten.
  17. Der Proband erhält derzeit oder erwartet ab 30 Tagen vor dem Screening bis zum Abschluss der Studie einen anderen Prüfwirkstoff, ein anderes Gerät oder ein anderes Verfahren.
  18. Der Proband hat nach Ansicht des Ermittlers eine Bedingung, die die Einhaltung der Studienanforderungen beeinträchtigt.

Experimental: Gene Therapy with CTNS-RD-04 (including product manufactured with and without LentiBOOST)
This is a single-arm, open-label study without randomization. Eligible subjects received CTNS-RD-04, an autologous CD34+ hematopoietic stem cell product transduced ex vivo with a lentiviral vector encoding CTNS. During the study, manufacturing incorporated a transduction enhancer (LentiBOOST) in later participants.
Cryopreserved autologous CD34+ enriched hematopoietic stem/progenitor cells collected from mobilized peripheral blood and transduced ex vivo with a self-inactivating lentiviral vector (pCCL-CTNS or pCDY.EFS.CTNS.T260I) encoding the human CTNS complementary deoxyribonucleic acid (cDNA) sequence. In later participants, a transduction enhancer (LentiBOOST) was incorporated into the manufacturing process.
