Stamcelle genterapi for cystinose

Cystinose er en sjelden arvelig recessiv sykdom som tilhører familien av lysosomale lagringsforstyrrelser og er preget av lysosomal akkumulering av cystin i alle cellene i kroppen som fører til multiorgansvikt. Cystinose har en ødeleggende innvirkning på de berørte individene, først og fremst barn og unge voksne, selv med cysteaminbehandling. Prevalensen av cystinose er 1 av 100 000 til 1 av 200 000. Genet involvert i cystinose er genet CTNS som koder for den transmembrane lysosomale cystintransportøren - cystinosin. Dagens standard for omsorg forhindrer ikke utviklingen av sykdommen og påvirker livskvaliteten til pasienter med cystinose betydelig.
For denne studien vil opptil 6 individer som oppfyller kvalifikasjonskriteriene bli transplantert etter et 3-kohort forskjøvet behandlingsdesign med 2 individer per kohort. De første 2 årskullene vil bestå av 4 voksne (18 år eller eldre), potensielt etterfulgt av en kohort bestående av 2 ungdommer eller voksne (> 14 år). Etter informert samtykkeprosessen, vil påmeldte emner bli screenet for å bekrefte full kvalifisering for deltakelse. Kvalifiserte personer vil gjennomgå mobilisering og innsamling av hematopoetiske stamceller (HSC) (leukaferese). En del av cellene vil bli holdt som "back-up" for redningsformål om nødvendig, og en del vil bli ex vivo genmodifisert med en lentiviral vektor, pCCL-CTNS eller pCDY.EFS.CTNS.T260I, for å uttrykke CTNS-genet (produktnavn: CTNS-RD-04). Klinisk produksjon for pasienter i kohort 3 vil introdusere en transduksjonsforsterker LentiBOOST (produktnavnet for disse pasientene vil være CTNS-RD-04-LB, hvor suffikset "-LB" står for LentiBOOST). Forsøkspersonene vil få margcytoreduksjon med busulfan før infusjon av CTNS-RD-04. Forsøkspersonene vil avbryte cysteaminbehandling i løpet av vurderingsperioden. Vurderingsoppfølgingsperioden vil inkludere innledende 2 år med aktive endepunktsevalueringer, hvor forsøkspersonene vil bli evaluert 3-, 6-, 9-, 12-, 18- og 24-måneder etter transplantasjon. En langtidsoppfølgingsstudie (LTFU) for en total 15-års oppfølgingsperiode vil bli tilbudt alle fag.
Målet med denne kliniske fase 1/2-studien er å vurdere sikkerheten/toleransen til CTNS-RD-04, og dens effektivitet gjennom en rekke kliniske, molekylære og biokjemiske vurderinger.

Inklusjonskriterier:
Følgende kriterier må oppfylles av alle emner som vurderes for studiedeltakelse.
- Kohorter 1 og 2: Mann eller kvinne er ≥ 18 år.
- Kohort 3: Mann eller kvinne er ≥ 14 år.
- Pasienten er diagnostisert med cystinose, dvs. tidlig begynnelse av Fanconi-syndrom, og historie med forhøyet cystinnivå i hvite blodlegemer og/eller historie med eller tilstedeværelse av cystinkrystaller i øyet.
- Personen har en Karnofsky Performance Status eller aldersavhengig Lansky Performance på ≥ 60.
- Hvis forsøkspersonen har hatt en nyretransplantasjon, må han eller hun være minst ett år etter nyretransplantasjon.
Personen har tilstrekkelig hematologisk funksjon:
- Absolutt nøytrofiltall (ANC) ≥ 1,5 x 1000/mm^3
- Blodplateantall ≥ 100 x 1000/mm^3
- Hemoglobin ≥ 9,0 g/dL
Personen har en adekvat leverfunksjon:
- Bilirubin ≤ 2,0 mg/dL

Personen har tilstrekkelig nyrefunksjon:
- Serumkreatinin <2x ULN mg/dL
- Kreatininclearance ≥ 50 ml/min/1,73 m^2
Personen har tilstrekkelig koagulasjon:
- PT/aPTT ≤ 1,2 x ULN sekunder
- INR ≤ 2
Personen har tilstrekkelig skjoldbruskkjertelfunksjon (med eller uten tyreoideabehandling):
- TSH 0,27-4,2 mIU/ml
- Total T4 ≤ 2 x ULN mcg/dL
Hvis kvinne: kvinne i fertil alder (dvs. ikke kirurgisk steril [tubal ligering, hysterektomi eller bilateral ooforektomi] eller ikke minst 2 år naturlig postmenopausal) godtar å forbli seksuelt avholdende eller bruke samme akseptable form for svært effektiv prevensjon fra screening gjennom to år etter transplantasjon.
De akseptable formene for prevensjon for denne studien inkluderer hormonelle prevensjonsmidler (oral, implantat, depotplaster eller injeksjon) assosiert med hemming av eggløsning ved en stabil dose i minst 3 måneder før screening, barriere (kondom med spermicid, membran med spermicid) , intrauterin enhet, eller en partner som har vært vasektomiert i minst 6 måneder og har dokumentert medisinsk vurdering av kirurgisk suksess ved en vasektomi.
Merk: menn med cystinose er sterile.
- Hvis menn: menn må gå med på å forbli seksuelt avholdende eller bruke en akseptabel form for svært effektiv prevensjon fra screening til to år etter transplantasjon.
- Emnet er villig og i stand til å overholde studierestriksjonene og kravene.
- Forsøkspersonen er villig til å gi skriftlig informert samtykke/tillatelse/samtykke før deltakelse i studien.
- Forsøkspersonen må være villig til å avstå fra å donere sæd etter å ha mottatt kondisjoneringsregimet. For personer som planlegger (eller for hvem det er mulighet for) å bli far til barn i fremtiden, vil sædbanking før administrering av kondisjoneringsregime anbefales.
- Forsøkspersonen må være villig til å avstå fra å donere blod, organer, vev eller celler for transplantasjon fra 30 dager før screening til når som helst etter CTNS-RD-04-behandling.
- Forsøkspersonen må være villig og må kunne (etter etterforskerens vurdering) avbryte sin cysteaminbehandling (oral og/eller øyedråpe).
Ekskluderingskriterier:
- Personen har en aktiv, ukontrollert, akutt bakteriell, viral eller soppinfeksjon under screening eller innen 30 dager før oppstart av kondisjoneringsregimet.
Personen har positiv serologi ved screening for ett av følgende:
- Humant immunsviktvirus (HIV) 1-2
- Human T-celle lymfotropisk virus (HTLV) - I/II
- Hepatitt B kjerne og hepatitt B PCR positive
- Hepatitt C-virus (HCV)
- Rapid Plasma Reagin (RPR)
- Chagas' sykdom (T. curzi)
- QuantiferonTB
- Nukleinsyretest (NAT) for HIV

- Personen har en kjent klinisk signifikant immunsviktforstyrrelse.
- Subjektet er en kvinne i fertil alder som ammer, planlegger en graviditet eller har en positiv serumgraviditetstest.
- Forsøkspersonen har mottatt en tidligere marg- eller stamcelletransplantasjon eller planlegger å motta en innen 90 dager etter studiestart.
- Personen har hatt en aktiv blødningsforstyrrelse innen 90 dager før screening ELLER krever antikoagulasjonsbehandling før behandling med ex vivo genterapi.
- Personen har en aktiv malignitet eller historie med malignitet inkludert lymfom (unntatt primær, kutan basalcelle- eller plateepitelkreft behandlet på passende måte før transplantasjon).
- Forsøkspersonen har en nyresykdom i sluttstadiet (definert som GFR <15 ml/min) og er allerede på en transplantasjonsliste eller som kanskje planlegger å registrere seg for en nyretransplantasjon innen 90 dager etter studiestart.
- Personen har nedsatt lungefunksjon (basert på FVC eller FEV1 på <50 % predikert, eller et FEV1/FVC-forhold som er mindre enn alders- og kjønnsspesifikk normal terskelverdi).
Personen har nedsatt hjertefunksjon innen 90 dager før screening, inkludert noe av følgende:
- Hjerteinfarkt
- Klinisk signifikant unormalt elektrokardiogram (EKG)
- Utkastningsandel på < 40 %
- Ukontrollert arytmi
- Annen klinisk signifikant hjertesykdom (f.eks. kongestiv hjertesvikt, ukontrollert hypertensjon, anamnese med labil hypertensjon).
- Personen har en alvorlig eller ukontrollert medisinsk lidelse (f.eks. pankreatitt, alvorlig leversykdom, ustabil diabetes mellitus) som etter etterforskernes mening vil svekke deres evne til å motta studiebehandling og følge studieprosedyrene.
- Forsøkspersonen har en historie med allergiske reaksjoner tilskrevet forbindelser med lignende kjemisk eller biologisk sammensetning som Busulfan eller allergi eller kontraindikasjon for bruk av andre midler brukt i studien, inkludert iohexol, syre-sitrat-dekstrose Formel A (ACDA), G-CSF eller plerixafor.
- Personen har en kjent historie med narkotika- eller alkoholavhengighet.

- Personen får cytotoksiske eller immunsuppressive midler, annet enn for nyretransplantasjon, innen 60 dager før screening eller krever behandling med slike midler før behandling med ex vivo genterapi.
- Forsøkspersonen har tidligere mottatt genterapi til enhver tid.
- Forsøkspersonen mottar for øyeblikket eller forventer å motta et annet undersøkelsesmiddel, enhet eller prosedyre fra 30 dager før screening gjennom fullføring av studien.
- Forsøkspersonen har noen forhold, etter etterforskerens mening, som kompromitterer etterlevelse av studiekrav.

Eksperimentell: Gene Therapy with CTNS-RD-04 (including product manufactured with and without LentiBOOST)
This is a single-arm, open-label study without randomization.
Eligible subjects received CTNS-RD-04, an autologous CD34+ hematopoietic stem cell product transduced ex vivo with a lentiviral vector encoding CTNS.
During the study, manufacturing incorporated a transduction enhancer (LentiBOOST) in later participants.

Cryopreserved autologous CD34+ enriched hematopoietic stem/progenitor cells collected from mobilized peripheral blood and transduced ex vivo with a self-inactivating lentiviral vector (pCCL-CTNS or pCDY.EFS.CTNS.T260I) encoding the human CTNS complementary deoxyribonucleic acid (cDNA) sequence.
In later participants, a transduction enhancer (LentiBOOST) was incorporated into the manufacturing process.
