Comparaison du PolarX et du Cryoballon Arctic Front pour l'isolement des veines pulmonaires chez les patients atteints de fibrillation auriculaire paroxystique symptomatique - Un essai clinique multicentrique de non-infériorité
L'isolement des veines pulmonaires (IVP) est un traitement efficace de la fibrillation auriculaire (FA). Les appareils monocoup sont de plus en plus utilisés pour le PVI. Actuellement, le cryoballon Medtronic Arctic Front est la technologie à tir unique la plus fréquemment utilisée et constitue donc la référence pour les technologies à venir. Une nouvelle technologie de cryoballon a récemment été introduite (PolarX, Boston Scientific). Cependant, la question de savoir si PolarX fournit une efficacité similaire à la norme de pratique Medtronic Arctic Front cryoballon n'a pas encore été étudiée. Étant donné que PolarX a été développé en tenant compte des limitations signalées et des échecs potentiels associés au cryoballon Medtronic Arctic Front, il pourrait être encore plus efficace et sûr pour une utilisation dans les procédures d'ablation de la FA.
L'objectif de cet essai est de comparer l'efficacité et l'innocuité du PolarX Cryoballoon (Boston Scientific) et du Arctic Front Cryoballoon (Medtronic) chez des patients atteints de FA paroxystique symptomatique subissant leur première IVP.
Il s'agit d'un essai ouvert, multicentrique, randomisé, contrôlé, initié par l'investigateur, avec évaluation en aveugle des critères d'évaluation. Étant donné que le Medtronic Arctic Front Cryoballoon est la norme de pratique pour le PVI à un seul coup et que le PolarX est la nouvelle technologie, cet essai a une conception de non-infériorité.
L'hypothèse concernant le critère principal d'évaluation de l'efficacité est que le cryoballon PolarX (Boston Scientific) présente une efficacité inférieure à celle du cryoballon Arctic Front (Medtronic) et que, par conséquent, davantage d'épisodes de première récidive de toute arythmie auriculaire entre les jours 91 et 365 seront observés. chez les patients atteints de FA paroxystique symptomatique subissant leur première IVP. Par conséquent, l'hypothèse alternative postule que le PolarX Cryoballoon est non inférieur au Arctic Front Cryoballoon. Le rejet de l'hypothèse nulle est nécessaire pour conclure à la non-infériorité.

Critère d'intégration:
- Fibrillation auriculaire paroxystique documentée sur un électrocardiogramme (ECG) à 12 dérivations ou un moniteur Holter (d'une durée ≥ 30 secondes) au cours des 24 derniers mois. Selon les directives actuelles, paroxystique est définie comme toute fibrillation auriculaire (FA) qui se transforme en rythme sinusal dans les 7 jours, soit spontanément, soit par cardioversion pharmacologique ou électrique.
- Candidat à l'ablation selon les directives AF actuelles
- Anticoagulation continue avec de la warfarine (rapport international normalisé [INR] 2-3) ou un nouvel anticoagulant oral (NOAC) pendant ≥ 4 semaines avant l'ablation ; ou un échocardiogramme transoesophagien (TEE) qui exclut un thrombus auriculaire gauche (LA) ≤ 48 heures avant l'ablation
- Âge de 18 ans ou plus à la date du consentement
- Consentement éclairé documenté par la signature (formulaire de consentement éclairé en annexe)
Critère d'exclusion:
- Antécédents d'ablation LA ou de chirurgie LA
- FA due à des causes réversibles (par ex. hyperthyroïdie, chirurgie cardiothoracique)
- Thrombus intracardiaque
- Sténose veineuse pulmonaire préexistante ou stent veineux pulmonaire
- Paralysie hémidiaphragmatique préexistante
- Contre-indication à l'anticoagulation ou aux produits de radiocontraste
- Prothèse valvulaire cardiaque
- Régurgitation ou sténose mitrale cliniquement significative (modérément sévère ou sévère)
- Infarctus du myocarde, intervention coronarienne percutanée (ICP)/angioplastie coronarienne transluminale percutanée (ACTP) ou stenting de l'artère coronaire au cours de la période de 3 mois précédant la date de consentement
- Chirurgie cardiaque au cours de l'intervalle de trois mois précédant la date de consentement ou la procédure programmée de chirurgie cardiaque/d'implantation de valve aortique transcathéter (TAVI)
- Malformation cardiaque congénitale importante (y compris les malformations septales auriculaires ou les anomalies de la veine pulmonaire, mais pas le foramen ovale perméable)
- Insuffisance cardiaque congestive de classe III ou IV de la New York Heart Association (NYHA)
- Fraction d'éjection ventriculaire gauche (FEVG) < 35 %
- Cardiomyopathie hypertrophique (épaisseur de paroi > 1,5 cm)
- Maladie rénale chronique importante (CKD ; taux de filtration glomérulaire estimé [eGFR] <30 μMol/L)
- Hyperthyroïdie non contrôlée
- Événement ischémique cérébral (AVC ou AIT) au cours de l'intervalle de six mois précédant la date de consentement
- Infections systémiques en cours
- Antécédents de cryoglobulinémie
- Grossesse*
- Espérance de vie inférieure à un (1) an par avis médical
- Participer actuellement à tout autre essai clinique d'un médicament, d'un dispositif ou d'un matériel biologique pendant la durée de cette étude.
Refus ou incapacité de se conformer pleinement aux procédures et au suivi de l'étude.
- Pour exclure une grossesse, un test sanguin (gonadotrophine chorionique humaine [HCG]) est utilisé.

Number of patients with recurrence of any atrial tachyarrhythmia (atrial fibrillation [AF], atrial flutter [AFL] or atrial tachycardia [AT]) between days 91 and 365 post ablation as detected on continuous implantable cardiac monitor (ICM).
AF, AFL or AT will qualify as a recurrence after ablation if it lasts 120 s or longer on ICM (the minimum programmable episode interval).
