Vergelijking van PolarX en de Arctic Front Cryoballoons voor PVI bij patiënten met symptomatische paroxysmale AF (COMPARE-CRYO)
Vergelijking van de PolarX en de Arctic Front Cryoballoon voor isolatie van longaderen bij patiënten met symptomatische paroxismale atriumfibrillatie - een multicenter non-inferiority design klinische studie
Pulmonale aderisolatie (PVI) is een effectieve behandeling voor atriumfibrilleren (AF). Single shot apparaten worden steeds vaker gebruikt voor PVI. Momenteel is de cryoballon Arctic Front van Medtronic de meest gebruikte enkelschotstechnologie en daarom de maatstaf voor opkomende technologieën. Onlangs is een nieuwe cryoballontechnologie geïntroduceerd (PolarX, Boston Scientific). Of PolarX echter dezelfde effectiviteit biedt als de standaard Medtronic Arctic Front cryoballon, moet nog worden onderzocht. Gezien het feit dat PolarX is ontwikkeld met inachtneming van de gemelde beperkingen en mogelijke storingen in verband met de Medtronic Arctic Front-cryoballon, zou het zelfs nog effectiever en veiliger kunnen zijn voor gebruik bij AF-ablatieprocedures.
Het doel van deze studie is om de werkzaamheid en veiligheid van de PolarX Cryoballoon (Boston Scientific) en de Arctic Front Cryoballoon (Medtronic) te vergelijken bij patiënten met symptomatische paroxismale AF die hun eerste PVI ondergaan.
Dit is een door de onderzoeker geïnitieerde, multicenter, gerandomiseerde, gecontroleerde, open-label studie met geblindeerde eindpunttoekenning. Gezien het feit dat de Medtronic Arctic Front Cryoballoon de standaard is voor single shot PVI en de PolarX de nieuwe technologie is, heeft deze proef een non-inferioriteitsontwerp.
De hypothese met betrekking tot het primaire werkzaamheidseindpunt is dat de PolarX Cryoballoon (Boston Scientific) een lagere werkzaamheid vertoont in vergelijking met de Arctic Front Cryoballoon (Medtronic) en dat er daarom tussen dag 91 en 365 meer episodes van eerste recidief van een atriale aritmie zullen worden waargenomen bij patiënten met symptomatische paroxismale AF die hun eerste PVI ondergaan. Vandaar dat de alternatieve hypothese stelt dat de PolarX Cryoballoon niet inferieur is aan de Arctic Front Cryoballoon. Verwerping van de nulhypothese is nodig om non-inferioriteit te concluderen.

Beschrijving
Inclusiecriteria:
- Paroxysmaal atriumfibrilleren gedocumenteerd op een 12-afleidingen elektrocardiogram (ECG) of Holter-monitor (duur ≥30 seconden) in de afgelopen 24 maanden. Volgens de huidige richtlijnen wordt paroxysmaal gedefinieerd als elk atriumfibrilleren (AF) dat binnen 7 dagen spontaan of door farmacologische of elektrische cardioversie in sinusritme verandert.
- Kandidaat voor ablatie op basis van de huidige AF-richtlijnen
- Continue antistolling met warfarine (International Normalized Ratio [INR] 2-3) of een nieuw oraal anticoagulans (NOAC) gedurende ≥4 weken voorafgaand aan de ablatie; of een transoesofageale echocardiogram (TEE) die linker atriale (LA) trombus ≤48 uur vóór ablatie uitsluit
- Leeftijd van 18 jaar of ouder op de datum van toestemming
- Geïnformeerde toestemming zoals gedocumenteerd door handtekening (Bijlage Geïnformeerde toestemmingsformulier)
Uitsluitingscriteria:
- Eerdere LA-ablatie of LA-operatie
- AF door omkeerbare oorzaken (bijv. hyperthyreoïdie, cardiothoracale chirurgie)
- Intracardiale trombus
- Reeds bestaande longaderstenose of longaderstent
- Reeds bestaande hemidiafragmatische verlamming
- Contra-indicatie voor anticoagulantia of radiocontrastmiddelen
- Hartklep prothese
- Klinisch significante (matig ernstige of ernstige) mitralisinsufficiëntie of stenose
- Myocardinfarct, percutane coronaire interventie (PCI)/percutane transluminale coronaire angioplastiek (PTCA) of stenting van de kransslagader gedurende de periode van 3 maanden voorafgaand aan de toestemmingsdatum
- Hartchirurgie gedurende het interval van drie maanden voorafgaand aan de toestemmingsdatum of geplande hartchirurgie/transkatheter aortaklepimplantatie (TAVI) procedure
- Significante aangeboren hartafwijking (inclusief atriumseptumdefecten of afwijkingen in de longader, maar exclusief patent foramen ovale)
- New York Heart Association (NYHA) klasse III of IV congestief hartfalen
- Linkerventrikelejectiefractie (LVEF) <35%
- Hypertrofische cardiomyopathie (wanddikte >1,5 cm)
- Significante chronische nierziekte (CKD; geschatte glomerulaire filtratiesnelheid [eGFR] <30 μMol/L)
- Ongecontroleerde hyperthyreoïdie
- Cerebrale ischemische gebeurtenis (beroerte of TIA) tijdens het interval van zes maanden voorafgaand aan de toestemmingsdatum
- Aanhoudende systemische infecties
- Geschiedenis van cryoglobulinemie
- Zwangerschap*
- Levensverwachting minder dan één (1) jaar per advies van de arts
- Momenteel deelnemen aan een andere klinische proef van een geneesmiddel, apparaat of biologisch materiaal tijdens de duur van deze studie.
Niet bereid of niet in staat om volledig te voldoen aan studieprocedures en follow-up.
- Om zwangerschap uit te sluiten wordt een bloedtest (humaan choriongonadotrofine [HCG]) gebruikt.
