Étude des vaccins à protéines recombinantes monovalentes et bivalentes contre le COVID-19 chez les adultes de 18 ans et plus (VAT00008)
Une étude en groupes parallèles, de phase III, en plusieurs étapes, modifiée en double aveugle et à plusieurs bras pour évaluer l'efficacité, l'innocuité et l'immunogénicité de deux vaccins à protéines recombinantes adjuvantées contre le SRAS-CoV-2 (monovalent et bivalent) pour la prévention contre le COVID -19 chez les adultes de 18 ans et plus dans le cadre d'une série primaire et d'une extension en ouvert pour évaluer l'immunogénicité, l'innocuité et l'efficacité d'une dose de rappel monovalente du vaccin à protéine recombinante adjuvantée contre le SRAS-CoV2
Le but de cette étude de phase III est d'évaluer l'efficacité, la sécurité et l'immunogénicité de deux vaccins CoV2 preS dTM-AS03 (monovalent et bivalent) dans le cadre de primo-vaccinations en plusieurs étapes, ainsi qu'une injection de rappel de un vaccin CoV2 preS dTM-AS03, chez les adultes de 18 ans et plus.
Au total, environ 21 046 participants devraient être inscrits (5 080 par groupe d'intervention de l'étude à l'étape 1 et 5 443 par groupe d'intervention de l'étude à l'étape 2).
La conception initiale de l'étude en série primaire à double insu est prévue pour 365 jours après la dernière injection initiale (c'est-à-dire environ 386 jours au total) pour chaque participant.
Sur la base des décisions du groupe de surveillance de l'étude, les participants aux étapes 1 et 2 seront invités à participer à une conception d'étude croisée / de rappel sans insu d'une durée comme suit :
- Pour les participants qui ont initialement reçu le vaccin : 12 mois après le rappel (c.-à-d. environ 18 à 24 mois)
- Pour les participants qui ont initialement reçu un placebo : ≥ 4 mois après la dernière dose de la série primaire + 12 mois après le rappel (c'est-à-dire environ 28 à 34 mois)
- Pour les participants qui ne consentent pas à continuer dans la partie Crossover / Booster sans insu de l'étude, toutes les procédures d'étude seront arrêtées et les participants seront retirés de l'étude.

Description détaillée
La durée de participation à la conception initiale de la série primaire en double aveugle de l'étude sera d'environ 365 jours après la dernière injection (c'est-à-dire environ 386 jours au total) pour chaque participant.

Critère d'intégration:
- Être âgé de 18 ans ou plus au jour de l'inclusion.
- Pour les personnes vivant avec le virus de l'immunodéficience humaine (VIH), infection à VIH stable déterminée par le participant actuellement sous antirétroviraux avec un nombre de CD4 > 200/mm3.
- Test de sérodiagnostic rapide SARS-CoV-2 effectué au moment de l'inscription pour détecter la présence d'anticorps SARS-CoV-2.
- N'a pas l'intention de recevoir un vaccin COVID-19 autorisé / approuvé malgré les encouragements de l'investigateur à recevoir le vaccin autorisé à sa disposition au moment de l'inscription.
- Le formulaire de consentement éclairé a été signé et daté
- Capable d'assister à toutes les visites et de se conformer à toutes les procédures d'étude
- Couvert par une assurance maladie, uniquement si requis par les réglementations locales, régionales ou nationales
Une participante peut participer si elle n'est pas enceinte ou si elle n'allaite pas et si l'une des conditions suivantes s'applique :
- est en âge de procréer. Pour être considérée comme n'ayant pas le potentiel de procréer, une femme doit être ménopausée depuis au moins 1 an ou chirurgicalement stérile, ou
- est en âge de procréer et accepte d'utiliser une méthode contraceptive efficace ou de s'abstenir d'au moins 4 semaines avant la première administration de l'intervention de l'étude jusqu'à au moins 12 semaines après la deuxième administration de l'intervention de l'étude.
Une participante en âge de procréer doit avoir un test de grossesse très sensible négatif (urine ou sérum tel que requis par la réglementation locale) dans les 25 heures avant toute dose d'intervention de l'étude.
Critère d'exclusion:
- Hypersensibilité systémique connue à l'un des composants du vaccin, ou antécédents de réaction potentiellement mortelle à un vaccin contenant l'une des mêmes substances.
- Démence ou toute autre condition cognitive à un stade qui pourrait interférer avec le suivi des procédures d'étude basées sur l'investigateur jugement.
- Thrombocytopénie autodéclarée, contre-indiquant la vaccination intramusculaire (IM) selon l'enquêteur jugement
- Trouble hémorragique ou prise d'anticoagulants au cours des 21 derniers jours précédant l'inclusion, contre-indiquant la vaccination IM selon l'investigateur ? jugement.
- Maladie aiguë ou chronique instable qui, de l'avis de l'investigateur ou de la personne désignée, pose un risque supplémentaire en raison de la participation ou qui pourrait interférer avec les procédures de l'étude.
- Maladie/infection aiguë modérée ou sévère (selon le jugement de l'investigateur) le jour de la vaccination ou maladie fébrile (température ? 38,0 C [? 100,4 F]). Un participant potentiel ne doit pas être inclus dans l'étude tant que l'état n'est pas résolu ou que l'événement fébrile n'a pas disparu.
- Réception de tout vaccin dans les 30 jours précédant ou le jour de la première vaccination à l'étude ou réception prévue de tout vaccin entre la première vaccination à l'étude et dans les 30 jours suivant la deuxième vaccination à l'étude, à l'exception de la vaccination contre la grippe, qui peut être reçue à tout moment temps par rapport à l'intervention de l'étude.
- Administration préalable d'un vaccin contre le coronavirus (SARS-CoV-2, SARS-CoV, Middle East Respiratory Syndrome).
- Réception de greffes d'organe solide ou de moelle osseuse au cours des 180 derniers jours.
- Réception d'une chimiothérapie anticancéreuse au cours des 90 derniers jours.
- Privé de liberté par décision administrative ou judiciaire, ou en urgence, ou hospitalisé contre son gré.
- Identifié en tant que chercheur ou employé du chercheur ou du centre d'étude ayant une implication directe dans l'étude proposée, ou identifié en tant que membre de la famille immédiate (c'est-à-dire parent, conjoint, enfant naturel ou adopté) du chercheur ou de l'employé ayant une implication directe dans l'étude proposée étudier.
- Participation au moment de l'inscription à l'étude (ou dans les 30 jours précédant la première vaccination à l'étude) ou participation prévue pendant la période d'étude actuelle à une autre étude clinique portant sur un vaccin, un médicament, un dispositif médical ou une procédure médicale.

An injection site reaction was an AR at and around the injection site of the study vaccine.
Systemic AR were all ARs that were not injection site reactions and included systemic manifestations such as headache, fever, as well as localized or topical manifestations that are not associated with the injection site.

Virologically-confirmed SARS-CoV-2 infection was defined as a positive result for SARS CoV-2 by nucleic acid amplification test (NAAT) on at least 1 respiratory sample.
This included positive results by any NAAT that included tests performed outside the trial protocol if confirmed by the adjudication committee.
Symptomatic COVID-19 was defined as virologically-confirmed SARS-CoV-2 infection accompanied by protocol-defined COVID-19-like illness.
Percentages are rounded off to the tenth decimal place.
Here, percentage of participants with virologically-confirmed SARS-CoV-2 infection and/or symptomatic COVID-19 (regardless of adjudication) are reported.

The COVID-19 severity scale was based on the 7-point ordinal scale of clinical assessments: 1: death; 2: hospitalized, on invasive mechanical ventilation or extracorporeal membrane oxygenation; 3: hospitalized, on non-invasive ventilation or high flow oxygen devices; 4: hospitalized, that required supplemental oxygen; 5: hospitalized, that did not require supplemental oxygen- discharged but required ongoing medical care (COVID-19 related or otherwise); 6: hospitalized, that did not require supplemental oxygen discharged without ongoing medical care; 7: not hospitalized.
