L'objectif principal du projet SwissNeuroFoundation AneurysmDataBase est de mettre en œuvre des outils basés sur les technologies de l'information pour créer et utiliser une base de données de référence holistique spécifique aux anévrismes intracrâniens (IA).
La SwissNeuroFoundation AneurysmDataBase s'occupe de générer les données avec lesquelles remplir cette base de données.
Le remplissage de la base de données a pour but de :
- Dépister et évaluer les marqueurs de risque de formation d'anévrisme intracrânien et de rupture d'anévrisme. Sont considérés comme marqueurs les facteurs suivants : facteurs génétiques, microbiote, facteurs environnementaux, facteurs congénitaux (ie : variantes anatomiques cérébrovasculaires), signature transcriptomique, signature protéomique, caractéristiques de forme, caractéristiques hémodynamiques.
- Dépistez et évaluez les facteurs pronostiques des résultats concernant différentes stratégies de gestion, y compris l'observation attentive, le traitement microchirurgical, le traitement endovasculaire ou toute combinaison de ceux-ci.
- Mettre en œuvre et évaluer des protocoles de prise en charge spécifiques aux patients intégrant toutes les informations disponibles.
- Évaluer l'impact de la base de données et l'utilisation d'outils pour améliorer les soins, réduire les coûts, soutenir la découverte des connaissances et promouvoir de nouveaux développements industriels.

Les conditions
Description détaillée
Les antécédents cliniques, les données d'imagerie, les échantillons biologiques et les informations dérivées sont les principaux éléments à saisir dans la SwissNeuroFoundation AneurysmDataBase. Aucune limite n'est fixée quant au nombre de volontaires sains et de cas de patients pour lesquels des données peuvent être saisies dans la SwissNeuroFoundation AneurysmDataBase.
Les participants seront recrutés parmi les sources suivantes :
- Patients arrivant dans les centres cliniques partenaires soit nouvellement diagnostiqués (cas incidents), soit déjà connus pour la maladie (cas prévalents)
- Des volontaires sains répondant à un appel, des sujets choisis au hasard parmi une population connue qui consentent à participer ou des membres de la famille non génétiquement liés de patients qui consentent à participer
- Membres de la famille des patients identifiés comme ayant des antécédents familiaux d'anévrisme intracrânien (IA).
Points de consentement :
Les éléments spécifiques sur lesquels les participants sont invités à consentir :
- avoir été informé verbalement des objectifs du projet
- d'avoir lu la fiche d'information sur le projet, d'avoir pu poser des questions sur le projet, et d'avoir reçu des réponses satisfaisantes et suffisamment de temps pour prendre une décision.
- d'avoir été informé que tout dommage qui pourrait être directement causé par la participation au projet est couvert par une assurance.
- que les prestataires de soins hospitaliers travaillant pour le compte du projet et les représentants des autorités locales et de la commission d'éthique peuvent accéder dans un délai strictement confidentiel aux données brutes pour le contrôle de la qualité.
- d'avoir été informé que la participation peut être retirée à tout moment sans justification et entraînera une évaluation médicale pour sa propre sécurité. Il n'y aura aucun impact sur les soins futurs résultant du sevrage.
- être libre de refuser de répondre aux questions sans exiger aucune justification et cela n'aura aucun impact sur les soins futurs.
- comprendre que les données peuvent être utilisées sous une forme non identifiante à des fins de publication.
- de comprendre que les données fournies peuvent faire partie de futures applications commerciales, et n'en bénéficieront pas financièrement.
- comprendre et accepter que les données codées puissent être utilisées pour d'autres recherches liées à l'activité cérébrovasculaire.
- qu'ils comprennent que des données codées les concernant ou des échantillons biologiques peuvent être transférés en dehors de l'Union européenne.
Les éléments spécifiques sur lesquels les participants sont invités à être d'accord ou en désaccord :
- qu'ils acceptent de donner accès aux dossiers de santé, de fournir un lien de ceux-ci vers la SwissNeuroFoundation AneurysmDataBase et de remplir un questionnaire sur leurs antécédents cliniques.
- donner son accord pour fournir des données d'image à SwissNeuroFoundation AneurysmDataBase.
- donner son accord pour fournir des échantillons de sang à SwissNeuroFoundation AneurysmDataBase.
- donner son accord pour l'utilisation d'échantillons biologiques résultant d'une intervention thérapeutique.
- donner son accord pour l'utilisation d'échantillons de salive et de selles à SwissNeuroFoundation AneurysmDataBase.
- donner son accord pour être recontacté par SwissNeuroFoundation AneurysmDataBase pour un consentement supplémentaire ou pour demander des informations complémentaires.
- donner son accord pour informer le médecin généraliste de la participation au projet.
- donner son accord pour contacter le médecin généraliste pour demander des informations sur la localisation du participant ou des informations médicales pertinentes limitées, si le contact direct a échoué pendant 3 mois.
- demander à être informé par le clinicien responsable des observations résultant du projet pouvant avoir un impact significatif sur la santé du participant.
- demander au médecin généraliste du participant d'être informé des observations résultant du projet pouvant avoir un impact significatif sur la santé du participant.
Séparez les points de consentement spécifiques relatifs aux échantillons biologiques et à la génétique :
- avoir lu la fiche d'information sur les tests génétiques, avoir pu poser des questions sur les tests génétiques et comprendre pourquoi la recherche est effectuée et les risques encourus
- donner son accord pour fournir des échantillons de sang à SwissNeuroFoundation AneurysmDataBase
- comprendre que les données génétiques issues d'échantillons peuvent faire partie de futures applications commerciales, et qu'ils n'en tireront aucun bénéfice financier.
- étant entendu qu'ils sont libres de se retirer à tout moment et peuvent demander la destruction de leur échantillon à ce moment-là ou à tout autre moment.
- accepter que les échantillons de sang soient conservés et utilisés à des fins d'analyse génétique.
- conviennent que les échantillons de sang, de dôme d'anévrisme, de liquide céphalo-rachidien (LCR) et de microdialyse soient conservés et utilisés pour d'autres recherches liées à l'activité cérébrovasculaire.
- accord que les échantillons de sang peuvent être transférés avec des étiquettes codées à d'autres universités dans le monde.
Aux fins de l'AneurysmDataBase :
Les informations cliniques comprennent :
- Informations démographiques (Pseudonyme, Âge, Sexe, Taille, Poids, activités professionnelles et sportives, habitudes : tabagisme, consommation d'alcool).
- Antécédents médicaux personnels (AVC antérieurs, épilepsie antérieure, médication antérieure)
- Arbre généalogique indiquant les événements d'IA ou d'hémorragie sous-arachnoïdienne (HSA) connus ou probablement diagnostiqués.
- Signes et symptômes (date de l'ictus, date du diagnostic, score mRankin, Glasgow Coma Scale (GCS), NIH Stroke Scale (NIHSS), Mini Mental State Exam (MMSE), World Federation of Neurosurgeons Scale WFNS, déficits des nerfs crâniens)
- Facteurs épidémiologiques identifiés selon la liste de contrôle.
- Analyse d'imagerie (présence d'HSA, grade de Fisher, taille des ventricules, taille des citernes basales, volume de l'hématome et emplacement, nombre d'anévrismes, emplacement des anévrismes, taille des anévrismes, forme de l'anévrisme)
- Informations sur le traitement et les résultats (méthode de traitement, succès)
- Informations sur le suivi et les résultats (évaluation de la qualité de vie, MMSE, mRankin, NIHSS, déficits des nerfs crâniens, état des anévrismes : stable, croissance, traité, partiellement traité)

- Fichiers DICOM pseudonymisés de :
- Angiographie IRM (ARM)
- Angiographie par tomodensitométrie (CTA)
- Reconstruction 3D de l'angiographie par rotation numérique (3D-DRA)

Les patients:
- doit avoir un anévrysme intracrânien prouvé par angiographie (ARM< CTA< DSA acceptée).

- défaut d'un candidat de fournir des données cliniques,
