Het primaire doel van het SwissNeuroFoundation AneurysmDataBase Project is het implementeren van op informatietechnologie gebaseerde hulpmiddelen om een holistische referentiedatabase te creëren en te gebruiken die specifiek is voor intracraniale aneurysma's (IA).
De SwissNeuroFoundation AneurysmDataBase houdt zich bezig met het genereren van de gegevens waarmee deze database kan worden gevuld.
Het doel van het vullen van de database is:
- Screenen op en evalueren van risicomarkers voor intracraniële aneurysmavorming en aneurysmaruptuur. Worden als markers beschouwd: genetische factoren, microbiota, omgevingsfactoren, congenitale factoren (dwz: cerebrovasculaire anatomische varianten), transcriptomics-signatuur, proteomics-signatuur, vormkenmerken, hemodynamische kenmerken.
- Screen op en evalueer prognostische uitkomstfactoren met betrekking tot verschillende managementstrategieën, waaronder waakzame observatie, microchirurgische behandeling, endovasculaire behandeling of een combinatie daarvan.
- Implementeer en evalueer patiëntspecifieke managementprotocollen waarin alle beschikbare informatie is geïntegreerd.
- Evalueer de impact van de database en het gebruik van tools om de zorg te verbeteren, kosten te verlagen, kennisontdekking te ondersteunen en nieuwe industriële ontwikkelingen te promoten.

Gedetailleerde beschrijving
Klinische geschiedenis, beeldvormingsgegevens, biologische monsters en daaruit afgeleide informatie zijn de belangrijkste items die moeten worden vastgelegd in de SwissNeuroFoundation AneurysmDataBase. Er zijn geen limieten gesteld aan het aantal gezonde vrijwilligers en patiëntgevallen waarvoor gegevens kunnen worden ingevoerd in de SwissNeuroFoundation AneurysmDataBase.
De deelnemers worden geworven uit de volgende bronnen:
- Patiënten die aankomen in klinische partnercentra als nieuw gediagnosticeerde (incidentele gevallen) of al bekend zijn vanwege de ziekte (veel voorkomende gevallen)
- Gezonde vrijwilligers die een oproep beantwoorden, willekeurig geselecteerde proefpersonen uit een bekende populatie die toestemming geven om deel te nemen of niet genetisch gekoppelde familieleden van patiënten die toestemming geven om deel te nemen
- Familieleden van patiënten met een familiale voorgeschiedenis van intracraniaal aneurysma (IA).
Punten van toestemming:
De specifieke items waarvoor deelnemers toestemming wordt gevraagd:
- mondeling geïnformeerd te zijn over de doelstellingen van het project
- het informatieblad over het project te hebben gelezen, vragen over het project te hebben kunnen stellen en bevredigende antwoorden en voldoende tijd te hebben gekregen om een beslissing te nemen.
- op de hoogte te zijn gebracht dat alle schade die rechtstreeks kan worden veroorzaakt door de deelname aan het project, wordt gedekt door een verzekering.
- dat ziekenhuiszorgverleners die namens het project werken en vertegenwoordigers van de lokale autoriteiten en de ethische commissie strikt vertrouwelijk toegang hebben tot ruwe gegevens voor kwaliteitscontrole.
- op de hoogte te zijn gebracht dat deelname op elk moment zonder rechtvaardiging kan worden ingetrokken en zal resulteren in een medische beoordeling voor eigen veiligheid. De stopzetting heeft geen gevolgen voor toekomstige zorg.
- vrij zijn om het beantwoorden van vragen te weigeren zonder enige rechtvaardiging en het zal geen invloed hebben op toekomstige zorg.
- om te begrijpen dat gegevens in een niet-identificeerbare vorm kunnen worden gebruikt voor publicatie.
- te begrijpen dat verstrekte gegevens deel kunnen uitmaken van toekomstige commerciële toepassingen en hier financieel niet van zullen profiteren.
- te begrijpen en ermee akkoord te gaan dat gecodeerde gegevens kunnen worden gebruikt voor verder cerebrovasculair gerelateerd onderzoek.
- dat ze begrijpen dat gecodeerde gegevens over hen of biologische monsters buiten de Europese Unie kunnen worden overgedragen.
De specifieke items waarover deelnemers wordt gevraagd om het eens of oneens te zijn:
- dat ze ermee instemmen toegang te verlenen tot medische dossiers, een link van hen naar de SwissNeuroFoundation AneurysmDataBase te verstrekken en een vragenlijst over hun klinische geschiedenis in te vullen.
- akkoord gaan om beeldgegevens te verstrekken aan SwissNeuroFoundation AneurysmDataBase.
- toestemming geven om bloedmonsters te verstrekken aan SwissNeuroFoundation AneurysmDataBase.
- toestemming geven om biologische monsters te gebruiken die het resultaat zijn van therapeutische interventie.
- geef toestemming om speeksel- en ontlastingsmonsters te gebruiken voor SwissNeuroFoundation AneurysmDataBase.
- stem ermee in om opnieuw gecontacteerd te worden door SwissNeuroFoundation AneurysmDataBase voor verdere toestemming of om meer informatie te vragen.
- akkoord gaan om de huisarts te informeren over de deelname aan het project.
- geef toestemming om contact op te nemen met de huisarts om informatie op te vragen over de verblijfplaats van de deelnemer of beperkte relevante medische informatie, als direct contact gedurende 3 maanden niet is gelukt.
- verzoeken om geïnformeerd te worden door de verantwoordelijke clinicus over observaties die voortkomen uit het project en die een significante impact kunnen hebben op de gezondheid van de deelnemer.
- de huisarts van de deelnemer verzoeken te worden geïnformeerd over waarnemingen die voortkomen uit het project en die een significante impact kunnen hebben op de gezondheid van de deelnemer.
Scheid specifieke punten van toestemming met betrekking tot biologische monsters en genetica:
- het informatieblad over genetische tests hebben gelezen, vragen hebben kunnen stellen over genetische testen en begrijpen waarom het onderzoek wordt gedaan en welke risico's daaraan verbonden zijn
- toestemming geven om bloedmonsters te verstrekken aan SwissNeuroFoundation AneurysmDataBase
- begrijpen dat genetische gegevens die uit monsters voortkomen deel kunnen uitmaken van toekomstige commerciële toepassingen en dat zij hier financieel niet van zullen profiteren.
- met dien verstande dat ze vrij zijn om zich op elk moment terug te trekken en kunnen vragen dat hun monster op dat moment of op elk ander moment wordt vernietigd.
- ermee instemmen dat bloedmonsters worden opgeslagen en gebruikt voor genetische analyse.
- ermee instemmen dat monsters van bloed, aneurysmakoepel, cerebrospinale vloeistof (CSF) en microdialyse worden opgeslagen en gebruikt voor verder cerebrovasculair gerelateerd onderzoek.
- akkoord dat bloedmonsters met gecodeerde labels kunnen worden overgedragen aan andere universiteiten in de wereld.
Ten behoeve van de AneurysmDataBase:
Klinische informatie bestaat uit:
- Demografische informatie (pseudoniem, leeftijd, geslacht, lengte, gewicht, werk- en sportactiviteiten, gewoonten: roken, alcoholgebruik).
- Persoonlijke medische geschiedenis (eerdere beroertes, eerdere epilepsie, eerdere medicatie)
- Stamboom die bekende of waarschijnlijk gediagnosticeerde IA- of subarachnoïdale bloeding (SAH) -gebeurtenissen aangeeft.
- Tekenen en symptomen (datum van ictus, datum van diagnose, mRankin-score, Glasgow Coma Scale (GCS), NIH Stroke Scale (NIHSS), Mini Mental State Exam (MMSE), World Federation of Neurosurgeons Scale WFNS, craniale zenuwdefecten)
- Geïdentificeerde epidemiologische factoren volgens checklist.
- Beeldvormingsanalyse (aanwezigheid van SAH, Fisher-kwaliteit, grootte van ventrikels, grootte van basale reservoirs, volume van hematoom en locatie, aantal aneurysma's, locatie van aneurysma's, grootte van aneurysma's, vorm van aneurysma)
- Behandel- en uitkomstinformatie (behandelmethode, succes)
- Follow-up en uitkomstinformatie (beoordeling van kwaliteit van leven, MMSE, mRankin, NIHSS, hersenzenuwdeficiënties, aneurysmastatus: stabiel, groei, behandeld, gedeeltelijk behandeld)

Beschrijving
Inclusiecriteria:
- in staat is om toestemming te geven of, in het geval dat een patiënt niet in staat is om toestemming te geven, dat een persoon wettelijk bevoegd is om dit namens de patiënt te doen volgens de wettelijke en ethische vereisten van het land waar het klinisch centrum is gevestigd.
Patiënten:
- moet een angiografisch bewezen intracraniaal aneurysma hebben (MRA< CTA< DSA geaccepteerd). De bepaling van SAH vereist de aanwezigheid van een subarachnoïdale bloeding die wordt geverifieerd op CT of door lumbaalpunctie.

Uitsluitingscriteria:
- falen van een kandidaat om klinische gegevens aan te dragen,
- weigering om geïnformeerde toestemming te geven.
