El objetivo principal del Proyecto SwissNeuroFoundation AneurysmDataBase es implementar herramientas basadas en tecnología de la información para crear y utilizar una base de datos de referencia holística específica para los aneurismas intracraneales (IA).
La SwissNeuroFoundation AneurysmDataBase se ocupa de generar los datos con los que poblar esta base de datos.
El propósito de llenar la base de datos es:
- Detectar y evaluar marcadores de riesgo de formación de aneurismas intracraneales y ruptura de aneurismas. Se consideran marcadores los siguientes: factores genéticos, microbiota, factores ambientales, factores congénitos (ie: variantes anatómicas cerebrovasculares), firma transcriptómica, firma proteómica, características de forma, características hemodinámicas.
- Examine y evalúe los factores pronósticos del resultado con respecto a las diferentes estrategias de manejo, incluida la observación atenta, el tratamiento microquirúrgico, el tratamiento endovascular o cualquier combinación de los mismos.
- Implementar y evaluar protocolos de manejo específicos del paciente integrando toda la información disponible.
- Evaluar el impacto de la base de datos y el uso de herramientas para mejorar la atención, reducir costos, apoyar el descubrimiento de conocimientos y promover nuevos desarrollos industriales.

Descripción detallada
El historial clínico, los datos de imágenes, las muestras biológicas y la información derivada son los elementos principales que se capturan en la SwissNeuroFoundation AneurysmDataBase. No se establecen límites en el número de voluntarios sanos y casos de pacientes para los que se pueden ingresar datos en SwissNeuroFoundation AneurysmDataBase.
Los participantes serán reclutados de las siguientes fuentes:
- Pacientes que llegan a los centros clínicos asociados ya sea como recién diagnosticados (casos incidentales) o ya conocidos por la enfermedad (casos prevalentes)
- Voluntarios sanos que responden a una llamada, sujetos seleccionados al azar de una población conocida que consienten en participar o familiares de pacientes no vinculados genéticamente que consienten en participar
- Familiares de pacientes identificados con antecedentes familiares de aneurisma intracraneal (IA).
Puntos de consentimiento:
Los elementos específicos sobre los que se pide el consentimiento de los participantes:
- haber sido informado verbalmente de los objetivos del proyecto
- a haber leído la ficha informativa sobre el proyecto, haber podido hacer preguntas sobre el proyecto, y haber recibido respuestas satisfactorias y tiempo suficiente para tomar la decisión.
- a haber sido informado de que cualquier daño que pudiera ser causado directamente por la participación en el proyecto está cubierto por un seguro.
- que los proveedores de atención hospitalaria que trabajan en nombre del proyecto y los representantes de las autoridades locales y la comisión de ética pueden acceder en términos estrictamente confidenciales a los datos sin procesar para el control de calidad.
- a haber sido informado de que la participación puede ser retirada en cualquier momento sin justificación y dará lugar a una evaluación médica por seguridad propia. No habrá impacto en la atención futura como resultado de la retirada.
- ser libre de negarse a responder a las preguntas sin necesidad de justificación alguna y no tendrá impacto en la atención futura.
- entender que los datos pueden ser utilizados en una forma no identificable para su publicación.
- entender que los datos proporcionados pueden formar parte de futuras aplicaciones comerciales y no se beneficiarán económicamente de esto.
- comprender y aceptar que los datos codificados se pueden utilizar para futuras investigaciones relacionadas con el accidente cerebrovascular.
- que entiende que datos codificados sobre ellos o muestras biológicas pueden ser transferidos fuera de la Unión Europea.
Los elementos específicos en los que se les pide a los participantes que estén de acuerdo o en desacuerdo:
- que aceptan proporcionar acceso a los registros de salud, proporcionar un enlace de ellos a la SwissNeuroFoundation AneurysmDataBase y completar un cuestionario sobre su historial clínico.
- dar su consentimiento para proporcionar datos de imágenes a SwissNeuroFoundation AneurysmDataBase.
- dar su consentimiento para proporcionar muestras de sangre a SwissNeuroFoundation AneurysmDataBase.
- dar su consentimiento para el uso de muestras biológicas resultantes de la intervención terapéutica.
- dar su consentimiento para usar muestras de saliva y heces a SwissNeuroFoundation AneurysmDataBase.
- acepta ser contactado nuevamente por SwissNeuroFoundation AneurysmDataBase para obtener más consentimiento o solicitar más información.
- Dar acuerdo para informar al médico general sobre la participación en el proyecto.
- dar su consentimiento para contactar al médico general para solicitar información sobre el paradero del participante o información médica relevante limitada, si el contacto directo no tuvo éxito durante 3 meses.
- solicitar ser informado por el clínico a cargo sobre las observaciones resultantes del proyecto que puedan tener un impacto significativo en la salud del participante.
- solicitar al Médico General del participante que sea informado sobre las observaciones resultantes del proyecto que puedan tener un impacto significativo en la salud del participante.
Separar puntos específicos de consentimiento relativos a muestras biológicas y genética:
- haber leído la hoja de información sobre las pruebas genéticas, haber podido hacer preguntas sobre las pruebas genéticas y comprender por qué se realiza la investigación y los riesgos involucrados
- otorgar un acuerdo para proporcionar muestras de sangre a SwissNeuroFoundation AneurysmDataBase
- entendiendo que los datos genéticos que surjan de las muestras pueden formar parte de futuras aplicaciones comerciales, y que no se beneficiarán económicamente de ello.
- entendiendo que son libres de retirarse en cualquier momento y pueden solicitar que su muestra sea destruida en ese momento o en cualquier otro momento.
- aceptar que las muestras de sangre se almacenen y utilicen para análisis genéticos.
- aceptar que las muestras de sangre, cúpula del aneurisma, líquido cefalorraquídeo (LCR) y microdiálisis se almacenen y utilicen para futuras investigaciones relacionadas con el accidente cerebrovascular.
- acuerdo de que las muestras de sangre pueden ser transferidas con etiquetas codificadas a otras Universidades del Mundo.
A los efectos de AneurysmDataBase:
La información clínica consta de:
- Información demográfica (Seudónimo, Edad, Sexo, Talla, Peso, Actividades laborales y deportivas, Hábitos: tabaquismo, Consumo de alcohol).
- Antecedentes médicos personales (Accidentes cerebrovasculares previos, epilepsia previa, medicación previa)
- Árbol genealógico que denota eventos conocidos o probablemente diagnosticados de IA o hemorragia subaracnoidea (HSA).
- Signos y síntomas (fecha del ictus, fecha del diagnóstico, puntuación mRankin, Escala de coma de Glasgow (GCS), Escala de accidente cerebrovascular NIH (NIHSS), Mini examen del estado mental (MMSE), Escala WFNS de la Federación Mundial de Neurocirujanos, Déficit de los nervios craneales)
- Factores epidemiológicos identificados según lista de verificación.
- Análisis de imagen (Presencia de HSA, grado de Fisher, tamaño de ventrículos, tamaño de cisternas basales, volumen de hematoma y ubicación, número de aneurismas, ubicación de aneurismas, tamaño de aneurismas, forma de aneurisma)
- Información sobre el tratamiento y los resultados (método de tratamiento, éxito)
- Información de seguimiento y resultados (evaluación de la calidad de vida, MMSE, mRankin, NIHSS, déficit de los nervios craneales, estado de los aneurismas: estable, en crecimiento, tratado, parcialmente tratado)

- Archivos DICOM seudonimizados de:
- Angiografía por resonancia magnética (ARM)
- Angiografía por tomografía computarizada (CTA)
- Reconstrucción 3D de Angiografía de Rotación Digital (3D-DRA)

Descripción
Criterios de inclusión:
- capaz de dar su consentimiento o, en el caso de que un paciente no pueda dar su consentimiento, que una persona esté legalmente autorizada para hacerlo en nombre del paciente de acuerdo con los requisitos legales y éticos del país donde se encuentra el centro clínico.
Pacientes:
- debe tener un aneurisma intracraneal comprobado angiográficamente (MRA< CTA< DSA aceptado). La determinación de HSA requiere que se verifique la presencia de una hemorragia subaracnoidea en TC o por punción lumbar.
MRA: Angiografía por resonancia magnética CTA: Angiografía por tomografía computarizada DSA: Angiografía por sustracción digital
Criterio de exclusión:
- falta de un candidato para contribuir con datos clínicos,
- negativa a proporcionar el consentimiento informado.
