O principal objetivo do projeto SwissNeuroFoundation AneurysmDataBase é implementar ferramentas baseadas em tecnologia da informação para criar e usar um banco de dados de referência holístico específico para aneurismas intracranianos (IA).
O SwissNeuroFoundation AneurysmDataBase está preocupado em gerar os dados com os quais preencher este banco de dados.
O objetivo de preencher o banco de dados é:
- Rastrear e avaliar marcadores de risco para formação de aneurisma intracraniano e ruptura de aneurisma. São considerados como marcadores os seguintes: fatores genéticos, microbiota, fatores ambientais, fatores congênitos (ou seja: variantes anatômicas cerebrovasculares), assinatura transcriptômica, assinatura proteômica, características de forma, características hemodinâmicas.
- Rastreie e avalie fatores prognósticos de resultado em relação a diferentes estratégias de gerenciamento, incluindo observação atenta, tratamento microcirúrgico, tratamento endovascular ou qualquer combinação dos mesmos.
- Implemente e avalie protocolos de gerenciamento específicos do paciente integrando todas as informações disponíveis.
- Avalie o impacto do banco de dados e uso de ferramentas para melhorar o atendimento, reduzir custos, apoiar a descoberta de conhecimento e promover novos desenvolvimentos industriais.

Histórico clínico, dados de imagem, amostras biológicas e informações derivadas são os principais itens a serem capturados no SwissNeuroFoundation AneurysmDataBase. Não há limites para o número de voluntários saudáveis e casos de pacientes para os quais os dados podem ser inseridos no SwissNeuroFoundation AneurysmDataBase.
Os participantes serão recrutados das seguintes fontes:
- Pacientes que chegam aos centros clínicos parceiros como recém-diagnosticados (casos incidentais) ou já conhecidos pela doença (casos prevalentes)
- Voluntários saudáveis que atendem a uma chamada, indivíduos selecionados aleatoriamente de uma população conhecida que consentem em participar ou familiares não geneticamente ligados de pacientes que consentem em participar
- Familiares de pacientes identificados com história familiar de aneurisma intracraniano (AI).
Pontos de consentimento:
Os itens específicos sobre os quais os participantes são solicitados a consentir:
- a ter sido informado verbalmente sobre os objetivos do projeto
- a ter lido a ficha informativa sobre o projeto, poder tirar dúvidas sobre o projeto e ter recebido respostas satisfatórias e tempo suficiente para tomar a decisão.
- por ter sido informado de que qualquer dano que possa ser causado diretamente pela participação no projeto está coberto por seguro.
- que os prestadores de cuidados hospitalares que trabalham em nome do projeto e representantes das autoridades locais e da comissão de ética possam acessar dados brutos em termos estritamente confidenciais para verificação de qualidade.
- por ter sido informado que a participação poderá ser descontinuada a qualquer momento sem justificativa e implicará em avaliação médica para sua própria segurança. Não haverá impacto nos cuidados futuros resultantes da retirada.
- ser livre para se recusar a responder perguntas sem exigir qualquer justificativa e isso não terá impacto nos cuidados futuros.
- entender que os dados podem ser usados de forma não identificável para publicação.
- entender que os dados fornecidos podem fazer parte de futuras aplicações comerciais, e não se beneficiarão financeiramente com isso.
- entender e concordar que os dados codificados podem ser usados para futuras pesquisas relacionadas a cerebrovasculares.
- que eles entendem que dados codificados sobre eles ou amostras biológicas podem ser transferidos para fora da União Europeia.
Os itens específicos sobre os quais os participantes devem concordar ou discordar:
- que eles concordam em fornecer acesso aos registros de saúde, fornecer um link deles para o SwissNeuroFoundation AneurysmDataBase e preencher um questionário sobre seu histórico clínico.
- concorda em fornecer dados de imagem para SwissNeuroFoundation AneurysmDataBase.
- concordar em fornecer amostras de sangue para SwissNeuroFoundation AneurysmDataBase.
- consentir a utilização de amostras biológicas resultantes de intervenção terapêutica.
- dê consentimento para usar amostras de saliva e fezes para SwissNeuroFoundation AneurysmDataBase.
- concorda em ser contatado novamente pela SwissNeuroFoundation AneurysmDataBase para mais consentimento ou para solicitar mais informações.
- concordar em informar o clínico geral sobre a participação no projeto.
- concordar em entrar em contato com o clínico geral para solicitar informações sobre o paradeiro do participante ou informações médicas relevantes limitadas, se o contato direto não tiver sucesso durante 3 meses.
- solicitar ser informado pelo clínico responsável sobre as observações decorrentes do projeto que possam ter um impacto significativo na saúde do participante.
- solicitar ao clínico geral do participante que seja informado sobre as observações decorrentes do projeto que possam ter um impacto significativo na saúde do participante.

- ter lido a folha de informações do teste genético, poder fazer perguntas sobre o teste genético e entender por que a pesquisa está sendo feita e os riscos envolvidos
- concordar em fornecer amostras de sangue para SwissNeuroFoundation AneurysmDataBase
- entendendo que os dados genéticos provenientes de amostras podem fazer parte de futuras aplicações comerciais, e que não serão beneficiados financeiramente com isso.
- entendendo que eles são livres para desistir a qualquer momento e podem pedir que sua amostra seja destruída então, ou em qualquer outro momento.
- concorda que as amostras de sangue sejam armazenadas e usadas para análise genética.
- concordam que amostras de sangue, cúpula de aneurisma, líquido cefalorraquidiano (LCR) e microdiálise sejam armazenadas e usadas para pesquisas adicionais relacionadas a cerebrovasculares.
- acordo de que as amostras de sangue podem ser transferidas com rótulos codificados para outras Universidades do Mundo.
Para fins do AneurysmDataBase:
As informações clínicas consistem em:
- Informação demográfica (Pseudónimo, Idade, Sexo, Tamanho, Peso, atividades laborais e desportivas, hábitos: tabagismo, consumo de álcool).
- Histórico médico pessoal (AVC anterior, epilepsia anterior, medicação anterior)
- Árvore genealógica que denota eventos de IA ou hemorragia subaracnóidea (SAH) conhecidos ou provavelmente diagnosticados.
- Sinais e sintomas (data do ictus, data do diagnóstico, pontuação mRankin, Escala de Coma de Glasgow (GCS), Escala NIH de AVC (NIHSS), Mini Exame do Estado Mental (MMSE), Escala WFNS da Federação Mundial de Neurocirurgiões, déficits de nervos cranianos)
- Fatores epidemiológicos identificados de acordo com a lista de verificação.
- Análise de imagem (Presença de HSA, grau de Fisher, tamanho dos ventrículos, tamanho das cisternas basais, volume e localização do hematoma, número de aneurismas, localização dos aneurismas, tamanho dos aneurismas, forma do aneurisma)
- Informações sobre tratamento e resultado (método de tratamento, sucesso)
- Informações de acompanhamento e resultados (avaliação da qualidade de vida, MMSE, mRankin, NIHSS, déficits de nervos cranianos, status de aneurismas: estável, crescimento, tratado, parcialmente tratado)

- Angiografia por ressonância magnética (ARM)
- Angiografia por Tomografia Computadorizada (CTA)
- Reconstrução 3D de Angiografia de Rotação Digital (3D-DRA)

Descrição
Critério de inclusão:
- capaz de fornecer consentimento ou, no caso de um paciente ser incapaz de fornecer consentimento, que uma pessoa esteja legalmente autorizada a fazê-lo em nome do paciente de acordo com os requisitos legais e éticos do país onde o centro clínico está localizado.
Pacientes:
- deve ter um aneurisma intracraniano comprovado angiograficamente (ARM< CTA< DSA aceito). A determinação de HSA requer que a presença de uma hemorragia subaracnóidea seja verificada na TC ou por punção lombar.
MRA: Angiografia por Ressonância Magnética CTA: Angiografia por Tomografia Computadorizada DSA: Angiografia por Subtração Digital
Critério de exclusão:
- falha de um candidato em contribuir com dados clínicos,
- recusa em fornecer consentimento informado.
