Description détaillée
Comme traitement standard pour les nausées et les vomissements après la chimiothérapie, les sujets recevront du fosaprépitant 150 mg IV une fois, du palonosétron 0,25 mg IV une fois, de la dexaméthasone 12 mg par voie orale ou IV une fois le jour 1.
Les sujets seront assignés au hasard soit au groupe DEX pour recevoir de la dexaméthasone, soit au groupe OLA pour recevoir de l'olanzapine pour le premier cycle de chimiothérapie.
- Groupe DEX : dexaméthasone (Decadron) 8 mg par jour par voie orale les jours 2 à 4.
- Groupe OLA : olanzapine (Zyprexa) 10 mg par voie orale chaque soir les jours 1 à 4.
Pour le deuxième cycle de chimiothérapie, le sujet passera à l'autre groupe.
Pour les futurs cycles de chimiothérapie, le sujet choisira le médicament qui a le mieux fonctionné.
Les sujets seront invités à répondre à une enquête avant le traitement le jour 1 du cycle 1 et du cycle 2 avant le traitement. Les jours 2 et 6, un membre de l'étude contactera les sujets par téléphone pour répondre à une autre enquête sur les symptômes que vous pourriez ressentir.

Critère d'intégration:
- 18 ans et plus
- diagnostic de cancer confirmé
- commencer le cycle 1 d'un traitement approuvé par la FDA qui est classé comme chimiothérapie hautement émétogène (induisant des nausées et des vomissements) selon les directives du National Comprehensive Cancer Network®
- Score de performance du Eastern Cooperative Oncology Group de 0 ou 1
- fonction rénale appropriée
- fonction hépatique appropriée
- fonction hématologique appropriée.
Critère d'exclusion:
- Les patients seront exclus s'ils présentent des nausées ou des vomissements jusqu'à 24 heures avant la chimiothérapie,
- actuellement sous corticothérapie
- contre-indication à la corticothérapie
- prendre tout médicament ayant des propriétés antiémétiques.
- prévu ou prévu de recevoir une radiothérapie dans la semaine suivant ou en même temps que la chimiothérapie
- métastases cérébrales.
