SPAD chez les patients adultes (DETECT)

Description détaillée
This multicenter observational prospective study will be conducted in 15 university or general hospitals, mostly centers included in the national network of the reference center CEREDIH. Pulmonologists, clinical immunologists and ENT specialists will be solicited in each center to refer patients for inclusions.
All patients who fulfill eligibility criteria will be tested for anti-pneumococcal polysaccharides (PnPSs) response with the 23-valent pneumococcal (unconjugated) vaccine (PPV23) (V1, Day 0: blood sample anti-PnPS antibodies, and immunization; V2, Day 28-56): blood sample for anti-PnPS antibodies).
If a SPAD would be diagnosed, the investigator will be free to consider prophylactic antibiotics and/or IgRT according to the medical history of the patient (notably in case of persistent infections while using prophylactic antibiotics). The efficiency on infections prevention and the impact on health-related quality of life (HRQoL) will be assessed 12 months after initiation of IgRT or prophylactic antibiotics (Visit 3).

Critère d'intégration:
- Patients de 18 à 65 ans
- Avec des antécédents d'infections bactériennes récurrentes des voies respiratoires supérieures et/ou inférieures depuis au moins 2 ans et répondant aux critères spécifiques suivants :
- Infections bénignes récurrentes nécessitant actuellement 6 cures d'antibiotiques /an ou plus, ou Bronchiectasie bilatérale (après exclusion de la mucoviscidose et de la dyskinésie ciliaire) ET infections bénignes récurrentes nécessitant actuellement 3 cures d'antibiotiques /an ou plus ou Antécédents de infection bactérienne des voies respiratoires inférieures, et/ou infection invasive à Streptococcus pneumonia, Streptococcus pyogenes ou Haemophilus influenzae, ayant nécessité une hospitalisation au cours des 2 dernières années, ET infections bénignes récurrentes nécessitant actuellement 3 cures d'antibiotiques/an.
- Taux sériques normaux des sous-classes d'IgG, IgA, IgM et IgG, taux normaux de CH50 et de protéines C3 et C4 du complément sérique, numération normale des lymphocytes T
- Numération normale des lymphocytes B, électrophorèse et immunofixation normales des protéines sériques. (* sauf colonisation par Pseudomonas aeruginosa)
Critère d'exclusion:
- Toute affection générale qui prédispose aux infections : tumeurs malignes solides ou hématologiques, diabète sucré, abus sévère d'alcool ou de drogues par voie intraveineuse, insuffisance hépatique ou rénale chronique, infection par le virus de l'immunodéficience humaine, asplénie anatomique ou fonctionnelle, neutropénie d'origine médicamenteuse 1 ou organe solide ou hématopoïétique greffe de cellules souches;
- Tout facteur local prédisposant aux infections : tabagisme (> 10 paquets-an et/ou 5 cigarettes/jour), infection sous-jacente (tuberculose, grippe…), bronchopneumopathie chronique obstructive, affections buccales, dentaires ou cutanées favorisant les infections, infections cutanées streptococciques
- Tout autre SID ou PID diagnostiqué avant l'inclusion
- Grossesse
- Administration du PPV23 au cours des 2 dernières années (risque d'hyporéactivité)
