Isolement de la veine pulmonaire avec ablation en champ pulsé avec ou sans ablation de la paroi postérieure chez les patients atteints de fibrillation auriculaire persistante symptomatique - Un essai clinique randomisé multicentrique : l'étude PIFPAF-PFA
L'isolation de la veine pulmonaire (PVI) est très efficace pour le contrôle du rythme chez les patients atteints de fibrillation auriculaire paroxystique (FA), mais moins efficace chez les patients atteints de FA persistante.
L'ajout de l'ablation de la paroi postérieure (PWA) à la PVI est l'une des stratégies d'ablation les plus prometteuses pour améliorer les résultats sans arythmie chez les patients atteints de FA persistante.
Les patients avec une cicatrice de la paroi postérieure de l'oreillette gauche peuvent bénéficier le plus de l'ajout de PWA à PVI.
Avec la technologie d'ablation précédente, l'isolation de la paroi postérieure (PWI) était difficile à réaliser et augmentait le risque de complications procédurales.
Avec l'ablation en champ pulsé (PFA), on dispose désormais d'une technologie à la fois très efficace et sûre pour l'ablation complète de la paroi postérieure.
Le but de cet essai est donc de comparer l'efficacité et l'innocuité procédurale de deux stratégies d'ablation pour le traitement de la FA persistante à l'aide de PFA : PVI seul versus PVI avec PWA ajouté.
Le point final de la récurrence de l'arythmie auriculaire dans les 12 mois sera évalué par un moniteur cardiaque implantable (ICM) avec des capacités de surveillance à distance.

Critère d'intégration:
- Fibrillation auriculaire persistante documentée sur un ECG à 12 dérivations, un moniteur Holter ou un dispositif cardiaque implantable au cours des 2 dernières années suivant l'inscription
- La fibrillation auriculaire persistante est définie comme un épisode soutenu durant > 7 jours
- Candidat à l'ablation selon les directives actuelles sur la fibrillation auriculaire
- Anticoagulation continue avec des antagonistes de la vitamine K ou un NACO pendant ≥ 4 semaines avant l'ablation ; ou une échocardiographie transoesophagienne et/ou une tomodensitométrie qui exclut un thrombus auriculaire gauche ≤ 48 heures avant la procédure d'ablation
- Âge de 18 ans ou plus à la date du consentement éclairé
- Consentement éclairé signé
Critère d'exclusion:
- Antécédent d'ablation auriculaire gauche ou de chirurgie auriculaire gauche
- Diamètre auriculaire gauche > 60 mm dans le grand axe parasternal
- Patients atteints de fibrillation auriculaire paroxystique
- Patients atteints de fibrillation auriculaire persistante depuis plus de 3 ans
- FA due à des causes réversibles (par ex. hyperthyroïdie, chirurgie cardiothoracique)
- Thrombus intracardiaque
- Sténose veineuse pulmonaire préexistante ou stent veineux pulmonaire
- Paralysie hémidiaphragmatique préexistante
- Contre-indication à l'anticoagulation ou aux produits de radiocontraste
- Chirurgie de la valve mitrale antérieure
- Régurgitation mitrale sévère ou sténose mitrale modérée/sévère
- Infarctus du myocarde au cours de la période de 3 mois précédant la date de consentement
- Trithérapie antithrombotique/anticoagulante en cours
- Chirurgie cardiaque au cours de l'intervalle de 3 mois précédant la date de consentement éclairé ou chirurgie cardiaque programmée/implantation de valve aortique transcathéter
- Malformation cardiaque congénitale importante (y compris les malformations septales auriculaires ou les anomalies de la veine pulmonaire, mais à l'exclusion d'un foramen ovale perméable)
- Insuffisance cardiaque congestive de classe NYHA III ou IV
- Fraction d'éjection ventriculaire gauche (FEVG) < 35 %
- Cardiomyopathie hypertrophique (épaisseur de paroi > 1,5 cm)
- Maladie rénale chronique importante (DFGe <30 ml/min)
- Hyperthyroïdie non contrôlée
- Événement ischémique cérébral (AVC ou AIT) au cours de l'intervalle de 6 mois précédant la date de consentement éclairé
- Infections systémiques en cours
- Antécédents de cryoglobulinémie
- Amylose cardiaque
- Grossesse (pour exclure une grossesse un test sanguin (HCG) est réalisé chez les femmes < 50 ans avant l'inclusion)
- Espérance de vie inférieure à un an par avis médical
- Participe actuellement à tout autre essai clinique, ce qui peut fausser les résultats de cet essai
- Refus ou incapacité de se conformer pleinement aux procédures et au suivi de l'étude
