Pulmonale aderisolatie (PVI) is zeer effectief voor ritmecontrole bij patiënten met paroxismaal atriumfibrilleren (AF), maar minder succesvol bij patiënten met aanhoudend AF. Het toevoegen van posterieure wandablatie (PWA) aan PVI is een van de meest veelbelovende ablatiestrategieën om de aritmievrije uitkomst bij patiënten met aanhoudend AF te verbeteren. Patiënten met littekens op de achterwand van het linker atrium kunnen het meeste baat hebben bij toevoeging van PWA aan PVI. Met eerdere ablatietechnologie was achterwandisolatie (PWI) moeilijk te bereiken en verhoogde het risico op procedurele complicaties. Met pulsed-field ablatie (PFA) is nu een technologie beschikbaar die zowel zeer effectief als veilig is voor volledige ablatie van de achterwand. Het doel van dit onderzoek is daarom om de werkzaamheid en procedurele veiligheid te vergelijken van twee ablatiestrategieën voor de behandeling van persisterend AF met behulp van PFA: alleen PVI versus PVI met toegevoegde PWA. Het eindpunt van recidief van atriale aritmie binnen 12 maanden zal worden beoordeeld door een implanteerbare hartmonitor (ICM) met bewakingsmogelijkheden op afstand.

Inclusiecriteria:

  1. Aanhoudend atriumfibrilleren gedocumenteerd op een 12-afleidingen ECG, Holter-monitor of implanteerbaar hartapparaat binnen de laatste 2 jaar na inschrijving
  2. Aanhoudend boezemfibrilleren wordt gedefinieerd als een aanhoudende episode die > 7 dagen duurt
  3. Kandidaat voor ablatie op basis van de huidige richtlijnen voor atriumfibrilleren
  4. Continue antistolling met vitamine K-antagonisten of een NOAC gedurende ≥4 weken voorafgaand aan de ablatie; of een transoesofageale echocardiografie en/of CT-scan die linker atriale trombus uitsluit ≤48 uur vóór de ablatieprocedure
  5. Leeftijd van 18 jaar of ouder op de datum van geïnformeerde toestemming
  6. Ondertekende geïnformeerde toestemming

Uitsluitingscriteria:

  1. Eerdere linker atriumablatie of linker atriumoperatie
  2. Linker atriale diameter> 60 mm in de parasternale lange as
  3. Patiënten met paroxismaal atriumfibrilleren
  4. Patiënten met aanhoudend atriumfibrilleren dat >3 jaar aanhoudt
  5. AF door omkeerbare oorzaken (bijv. hyperthyreoïdie, cardiothoracale chirurgie)
  6. Intracardiale trombus
  7. Reeds bestaande longaderstenose of longaderstent
  8. Reeds bestaande hemidiafragmatische verlamming
  9. Contra-indicatie voor anticoagulantia of radiocontrastmiddelen
  10. Eerdere mitralisklepoperatie
  11. Ernstige mitralisinsufficiëntie of matige/ernstige mitralisklepstenose
  12. Myocardinfarct gedurende de periode van 3 maanden voorafgaand aan de toestemmingsdatum
  13. Lopende drievoudige antitrombotische/antistollingstherapie
  14. Hartchirurgie tijdens het interval van 3 maanden voorafgaand aan de geïnformeerde toestemmingsdatum of geplande hartchirurgie/transkatheter aortaklepimplantatie
  15. Significante aangeboren hartafwijking (waaronder atriumseptumdefecten of longaderafwijkingen, maar exclusief een patent foramen ovale)
  16. NYHA klasse III of IV congestief hartfalen
  17. Linkerventrikelejectiefractie (LVEF) <35%
  18. Hypertrofische cardiomyopathie (wanddikte >1,5 cm)
  19. Significante chronische nierziekte (eGFR <30 ml/min)
  20. Ongecontroleerde hyperthyreoïdie
  21. Cerebrale ischemische gebeurtenis (beroerte of TIA) tijdens het interval van 6 maanden voorafgaand aan de geïnformeerde toestemmingsdatum
  22. Aanhoudende systemische infecties
  23. Geschiedenis van cryoglobulinemie
  24. Cardiale amyloïdose
