Prédire la neuropathie périphérique du paclitaxel en chimiothérapie de deuxième intention pour le cancer gastrique
Bien que les progrès de la chimiothérapie aient amélioré le pronostic des patients atteints d’un cancer gastrique, de nombreux patients souffrent encore d’effets indésirables.
Il est donc nécessaire d’établir un traitement personnalisé en identifiant les patients à haut risque d’effets secondaires.
Bien que le traitement à base de paclitaxel soit le traitement standard de deuxième intention, la neuropathie périphérique est un événement indésirable gênant.
Le but de cette étude est d'établir un test de biopsie liquide pour prédire la neuropathie périphérique induite par le paclitaxel chez les patients atteints d'un cancer gastrique.

Description détaillée
Le traitement à base de paclitaxel est le traitement standard de deuxième intention pour les patients atteints d'un cancer gastrique, mais environ 30 à 40 % des patients développent une neuropathie périphérique qui interfère avec la vie quotidienne, et environ 10 % développent une neuropathie périphérique sévère de grade 3.
Des effets secondaires particulièrement graves peuvent entraîner une perte de force chez les patients, les obligeant à interrompre le traitement et perdant ainsi la possibilité de recevoir d'autres traitements qui étaient initialement censés avoir un effet thérapeutique.
Cette étude vise à prédire la neuropathie périphérique dans les effets secondaires du traitement de deuxième intention (paclitaxel plus ramucirumab) chez les patients atteints d'un cancer gastrique à l'aide de biopsies liquides (petits ARN).
Si les patients à haut risque d'effets secondaires peuvent être prédits avant le traitement, les patients à haut risque peuvent se voir proposer des réductions de médicaments ou d'autres options de chimiothérapie.
Le but de cette étude est de prédire la neuropathie périphérique du paclitaxel en chimiothérapie de deuxième intention du cancer gastrique par biopsie liquide.

Les patients ont besoin d'une chimiothérapie de deuxième intention en cas de cancer gastrique non résécable ou récurrent.
La description
Critère d'intégration:
- Cancer gastrique (GC) non résécable ou récurrent confirmé histologiquement comme étant un adénocarcinome primitif de l'estomac.
- âge de plus de 20 ans.
- Score de performance de l’Eastern Cooperative Oncology Group de 0-2.
- un consentement éclairé écrit après que les informations complètes sur l'étude soient fournies au patient.
- progression ou intolérance à la chimiothérapie de première intention comprenant de la pyrimidine fluorée et des médicaments anticancéreux à base de platine (cisplatine ou oxaliplatine) pour les GC avancées.
- présence de lésions évaluables confirmées par une tomodensitométrie (TDM) ou une imagerie par résonance magnétique.
Critère d'exclusion:
- Patients ayant une espérance de vie inférieure à 3 mois
- Patients présentant des complications graves (angine de poitrine, infarctus du myocarde ou arythmie) ou un diabète sucré incontrôlable, une hypertension artérielle ou une tendance hémorragique.
- Patients ayant des antécédents de réactions allergiques graves ou d’allergie médicamenteuse grave.
- Patients présentant un trouble mental cliniquement pertinent qui interdit de répondre aux questionnaires.
- Patients pour lesquels le médecin traitant a estimé que l'inscription à l'étude était inappropriée.
