Forutsi perifer nevropati av paklitaksel i andrelinjekjemoterapi for gastrisk kreft
Selv om fremskritt innen kjemoterapi har forbedret prognosen for magekreftpasienter, lider fortsatt mange pasienter av uønskede hendelser.
Derfor er det nødvendig å etablere personlig behandling ved å identifisere pasienter med høy risiko for bivirkninger.
Selv om paklitakselbasert terapi er standard andrelinjebehandling, er perifer nevropati en plagsom bivirkning.
Hensikten med denne studien er å etablere en flytende biopsianalyse for å forutsi paklitaksel-indusert perifer nevropati hos magekreftpasienter.

Detaljert beskrivelse
Paclitaxel-basert terapi er standard andrelinjebehandling for magekreftpasienter, men ca. 30-40 % av pasientene utvikler perifer nevropati som forstyrrer dagliglivet, og ca. 10 % utvikler alvorlig grad 3 perifer nevropati.
Spesielt alvorlige bivirkninger kan føre til at pasienter mister krefter, og tvinger dem til å avbryte behandlingen, og dermed mister muligheten til å få annen behandling som opprinnelig var forventet å ha terapeutisk effekt.
Denne studien tar sikte på å forutsi perifer nevropati i bivirkninger av annenlinjebehandling (paclitaxel pluss ramucirumab) hos pasienter med magekreft ved bruk av flytende biopsier (liten RNA).
Hvis pasienter med høy risiko for bivirkninger kan forutsies før behandling, kan høyrisikopasienter tilbys medikamentreduksjoner eller andre kjemoterapialternativer.
Målet med denne studien er å forutsi perifer nevropati av paklitaksel i andrelinjekjemoterapi for magekreft ved flytende biopsi.

Pasienter trenger annenlinjekjemoterapi med ikke-opererbar eller tilbakevendende magekreft.
Beskrivelse
Inklusjonskriterier:
- uopererbar eller tilbakevendende gastrisk kreft (GC) histologisk bekreftet å være primært adenokarsinom i magen.
- alder over 20 år.
- Eastern Cooperative Oncology Group resultatstatusscore på 0-2.
- skriftlig informert samtykke etter at fullstendig studieinformasjon er gitt til pasienten.
- progresjon eller intoleranse for førstelinjekjemoterapi som omfatter fluorert pyrimidin og platina-antikreftmedisiner (cisplatin eller oxaliplatin) for avansert GC.
- tilstedeværelse av evaluerbare lesjoner som bekreftet ved hjelp av computertomografi (CT) eller magnetisk resonansavbildning.
Ekskluderingskriterier:
- Pasienter med forventet levetid kortere enn 3 måneder
- Pasienter med alvorlige komplikasjoner (angina pectoris, hjerteinfarkt eller arytmi) eller ukontrollerbar diabetes mellitus, blodhypertensjon eller blødningstendens.
- Pasienter med en historie med alvorlige allergiske reaksjoner eller alvorlig legemiddelallergi.
- Pasienter med en klinisk relevant psykisk lidelse som forbyr svar på spørreskjemaer.
- Pasienter som den behandlende legen mente at innmelding i studien er upassende for.
