Het voorspellen van perifere neuropathie van paclitaxel bij tweedelijns chemotherapie voor maagkanker
Hoewel de vooruitgang op het gebied van chemotherapie de prognose van maagkankerpatiënten heeft verbeterd, hebben veel patiënten nog steeds last van bijwerkingen.
Daarom is het noodzakelijk om een gepersonaliseerde behandeling tot stand te brengen door patiënten met een hoog risico op bijwerkingen te identificeren.
Hoewel op paclitaxel gebaseerde therapie de standaard tweedelijnsbehandeling is, is perifere neuropathie een lastige bijwerking.
Het doel van deze studie is het opzetten van een vloeibare biopsietest om door paclitaxel geïnduceerde perifere neuropathie bij maagkankerpatiënten te voorspellen.

Op paclitaxel gebaseerde therapie is de standaard tweedelijnsbehandeling voor maagkankerpatiënten, maar ongeveer 30-40% van de patiënten ontwikkelt perifere neuropathie die het dagelijks leven verstoort, en ongeveer 10% ontwikkelt ernstige graad 3 perifere neuropathie.
Bijzonder ernstige bijwerkingen kunnen ertoe leiden dat patiënten kracht verliezen, waardoor ze gedwongen worden de behandeling te staken en zo de kans verliezen om andere behandelingen te krijgen waarvan oorspronkelijk werd verwacht dat ze een therapeutisch effect zouden hebben.
Deze studie heeft tot doel perifere neuropathie te voorspellen in de bijwerkingen van tweedelijnsbehandeling (paclitaxel plus ramucirumab) bij patiënten met maagkanker met behulp van vloeibare biopsieën (klein RNA).
Als patiënten met een hoog risico op bijwerkingen vóór de behandeling kunnen worden voorspeld, kunnen patiënten met een hoog risico medicatievermindering of andere opties voor chemotherapie worden aangeboden.
Het doel van deze studie is om perifere neuropathie van paclitaxel bij tweedelijnschemotherapie voor maagkanker te voorspellen door middel van vloeibare biopsie.

Studie Bevolking
Patiënten hebben tweedelijnschemotherapie nodig met inoperabele of recidiverende maagkanker.
Beschrijving
Inclusiecriteria:
- niet-reseceerbare of recidiverende maagkanker (GC), waarvan histologisch is bevestigd dat het een primair adenocarcinoom van de maag is.
- leeftijd ouder dan 20 jaar.
- Prestatiestatusscore van Eastern Cooperative Oncology Group van 0-2.
- schriftelijke geïnformeerde toestemming nadat de volledige onderzoeksinformatie aan de patiënt is verstrekt.
- progressie of intolerantie voor eerstelijnschemotherapie bestaande uit gefluoreerde pyrimidine en platina antikankermedicijnen (cisplatine of oxaliplatine) voor gevorderde GC.
- aanwezigheid van evalueerbare laesies zoals bevestigd met behulp van computertomografie (CT) of magnetische resonantiebeeldvorming.
Uitsluitingscriteria:
- Patiënten met een levensverwachting korter dan 3 maanden
- Patiënten met ernstige complicaties (angina pectoris, myocardinfarct of aritmie) of oncontroleerbare diabetes mellitus, bloedhypertensie of neiging tot bloeden.
- Patiënten met een voorgeschiedenis van ernstige allergische reacties of ernstige geneesmiddelenallergie.
- Patiënten met een klinisch relevante psychische stoornis die het beantwoorden van vragenlijsten verbiedt.
- Patiënten voor wie de behandelende arts van mening was dat deelname aan het onderzoek niet passend was.
