Questo è uno studio osservazionale retrospettivo progettato come emulazione di un trial target utilizzando i dati delle cartelle cliniche elettroniche raccolti di routine dalla TriNetX US Collaborative Network. Lo studio mira a confrontare i rischi a 36 mesi di mortalità per tutte le cause e degli esiti cardiorenale maggiori tra gli adulti con obesità, diabete di tipo 2 e stadio cardiorenale-metabolico 2-3 che iniziano un agonista del recettore GLP-1 o un inibitore SGLT2 e successivamente seguono diverse strategie di intensificazione precoce del trattamento. Gli stimatori primari sono i rischi relativi e le differenze di rischio assoluto a 36 mesi.
I partecipanti eleggibili sono adulti di età pari o superiore a 20 anni con obesità e diabete di tipo 2 che soddisfano i criteri dello stadio cardiorenale-metabolico 2-3. L'eleggibilità, le esposizioni e gli esiti sono operazionalizzati utilizzando i dati delle cartelle cliniche elettroniche TriNetX, inclusi i codici di diagnosi ICD-10-CM, un indice di massa corporea di almeno 27 kg/m², un'emoglobina A1c di almeno 6,5% e i registri di dispensazione dei farmaci. I criteri di esclusione includono l'uso precedente di classi di farmaci rilevanti entro 6 mesi, malattie cardiovascolari maggiori o malattie renali avanzate entro 12 mesi, eventi cardiovascolari o renali maggiori entro 6 mesi e qualsiasi anamnesi di diabete non di tipo 2, HIV, chirurgia bariatrica o trapianto di organi solidi.
Vengono confrontate tre strategie di intensificazione precoce del trattamento mutualmente esclusive: terapia aggiuntiva precoce con inibitore SGLT2, terapia aggiuntiva precoce con inibitore DPP-4 o sulfonilurea e nessuna terapia aggiuntiva precoce. Per le strategie con aggiunta, la data indice è la data di inizio della terapia aggiuntiva entro 90 giorni dall'inizio della terapia di base. Per la strategia senza aggiunta precoce, la data indice è il punto di riferimento dei 90 giorni dopo l'inizio della terapia di base.
Poiché l'assegnazione del trattamento non è randomizzata, la selezione non casuale del trattamento viene affrontata utilizzando l'abbinamento del punteggio di propensione multinomiale con abbinamento del vicino più prossimo 1:1 senza sostituzione e un calibro di 0,2 volte la deviazione standard del punteggio di propensione logit. L'equilibrio delle covariate post-abbinamento viene valutato utilizzando le differenze medie standardizzate, e qualsiasi squilibrio residuo viene ulteriormente corretto nei modelli di esito. Il follow-up inizia alla data indice e continua fino al primo tra l'esito di interesse, la morte, l'ultimo incontro registrato o il 31 gennaio 2026, con analisi per intenzione di trattare e per protocollo eseguite.
L'esito primario è la mortalità per tutte le cause entro 36 mesi. Gli esiti secondari includono eventi cardiovascolari avversi maggiori, definiti come arresto cardiovascolare, infarto miocardico o ictus, ed eventi renali avversi maggiori, definiti come malattia renale allo stadio terminale, dipendenza o inizio della dialisi, insufficienza renale o morte. Gli effetti comparativi sono stimati su 36 mesi per confronti a coppie di aggiunte precoci, con analisi di sottogruppo pre-specificate per stadio cardiorenale-metabolico, gravità dell'obesità, funzione renale basale e comorbidità cardiovascolare basale.

Criteri di inclusione:
- Adulti di età pari o superiore a 20 anni.
- Obesità, definita da un indice di massa corporea (IMC) pari o superiore a 27 kg/m².
- Diabete mellito di tipo 2, definito utilizzando i dati delle cartelle cliniche elettroniche, inclusi codici di diagnosi e/o emoglobina A1c pari o superiore al 6,5%.
- Soddisfare i criteri di stadio 2-3 cardiovascolare-renale-metabolico (CKM) al basale.
- Inizio di un agonista del recettore GLP-1 o di un inibitore SGLT2 come terapia di base.
- Classificazione della strategia di trattamento basata sull'inizio precoce dell'aggiunta entro 90 giorni dall'inizio della terapia di base, o nessuna aggiunta precoce con indice al punto di riferimento dei 90 giorni.
Criteri di esclusione:
- Uso precedente di agonisti del recettore GLP-1, inibitori SGLT2, inibitori DPP-4 o sulfoniluree nei 6 mesi precedenti l'ingresso nella coorte.
- Malattia cardiovascolare maggiore o rivascolarizzazione nei 12 mesi precedenti l'ingresso nella coorte.
- Malattia renale avanzata nei 12 mesi precedenti l'ingresso nella coorte, inclusa malattia renale allo stadio terminale, dialisi o velocità di filtrazione glomerulare stimata inferiore a 15 mL/min/1,73 m².
- Eventi cardiovascolari o renali maggiori nei 6 mesi precedenti l'indice.
- Qualsiasi anamnesi di diabete non di tipo 2, infezione da HIV, chirurgia bariatrica o trapianto di organi solidi.
- Assenza di covariate basali critiche.
