Protocollo di Studio Principale per la Coorte della Rete Specialistica Ictus (SN Stroke) (Stroke-CORE)
La coorte Stroke-CORE mira a fornire una comprensione completa e aggiornata dell'assistenza all'ictus in Germania. A tale scopo, i pazienti vengono seguiti lungo l'intero continuum assistenziale: dalla gestione iniziale prima del ricovero ospedaliero, attraverso il trattamento acuto, la riabilitazione e l'assistenza successiva, fino alla prevenzione di ictus ricorrenti. Gli ictus che si verificano in età pediatrica sono inclusi in questo studio di coorte.
Inoltre, lo studio mira a stabilire strutture efficaci per l'identificazione precoce e l'inclusione dei pazienti nella ricerca clinica (screening). In questo contesto, vengono implementati processi di screening strutturati a tutti i livelli di assistenza presso i siti partecipanti all'interno della Rete di Medicina Universitaria (NUM), una collaborazione tra ospedali universitari tedeschi. Contemporaneamente, la coorte funge da piattaforma all'interno di questa rete per affrontare sistematicamente domande di ricerca aperte in varie aree dell'assistenza all'ictus e supportare la pianificazione e la conduzione mirate di futuri studi clinici.
Lo studio include pazienti con ictus ischemico (causato da un'ostruzione di un vaso sanguigno) o emorragia intracerebrale (sanguinamento all'interno del cervello). La partecipazione si svolge in diversi moduli tematici, ciascuno incentrato su aspetti specifici del decorso della malattia e della sua gestione. Questi moduli affrontano, tra gli altri argomenti, il trattamento ospedaliero acuto, le misure per prevenire ictus ricorrenti (prevenzione secondaria), le possibili complicanze, l'assistenza pre-ospedaliera, la riabilitazione, l'assistenza successiva e gli esiti a lungo termine come il recupero fisico, l'indipendenza nella vita quotidiana e le funzioni cognitive tra cui memoria e concentrazione.
Nell'ambito dello studio viene raccolta una serie di dati. Questi includono informazioni sociodemografiche (come età e situazione abitativa) e dati medici, ad esempio su condizioni preesistenti, diagnosi, trattamenti e comorbidità. Inoltre, la condizione di salute dei pazienti viene valutata in più momenti al fine di comprendere meglio il decorso della malattia. A seconda del livello di partecipazione, la raccolta dei dati può essere integrata dalla raccolta di campioni biologici, come il sangue. Questi vengono ottenuti sia durante le procedure cliniche di routine sia attraverso metodi semplici e minimamente invasivi (non invasivi).
Lo studio è strutturato in diversi livelli che differiscono per ampiezza e approfondimento della raccolta dati. Il livello base include informazioni essenziali, mentre i livelli superiori prevedono valutazioni più dettagliate. Inoltre, alcuni livelli includono la raccolta di campioni biologici, e ci sono moduli supplementari opzionali a cui i pazienti possono partecipare volontariamente. Questo approccio a livelli consente di adattare flessibilmente la partecipazione alla situazione individuale, contribuendo al contempo a una comprensione sfumata e completa dell'assistenza all'ictus.

Popolazione di studio
Pazienti con ictus secondo i criteri definiti in ogni singolo Modulo (IS, ICH, ictus pediatrico) ricoverati in uno degli ospedali universitari partecipanti in Germania entro 24 ore dall'inizio dei sintomi.
Criteri generali di inclusione ed esclusione:
- Soddisfare tutti i criteri di inclusione e nessun criterio di esclusione per almeno un Modulo
- Consenso informato firmato da pazienti capaci di esprimere il consenso e di comprendere tutto il contenuto del consenso informato o consenso informato di un appropriato rappresentante del paziente, se i pazienti non sono in grado di fornire il consenso da soli.
Descrizione
Criteri di inclusione:<\/p>
- Diagnosi di AIS o ICH; la diagnosi di AIS può essere posta clinicamente o radiologicamente (ad es., compromissione neurologica focale ad esordio improvviso, di durata superiore a 24 ore e di presunta origine vascolare, o con evidenza di ischemia cerebrale corrispondente ai sintomi clinici alla neuroimaging); per la diagnosi di ICH sono necessari sintomi clinici e diagnosi radiologica di ICH<\/li>
- Ricovero ospedaliero <24 ore dall'esordio dei sintomi<\/li>
- Età ≥18 anni per i Moduli base AIS e ICH; età <18 anni per il Modulo base pazienti pediatrici con ictus<\/li>
- Consenso informato: Consenso informato firmato da pazienti in grado di acconsentire e comprendere tutto il contenuto del CI o CI di un appropriato rappresentante del paziente o consenso differito, se i pazienti non sono in grado di acconsentire da soli<\/li>
- Criteri di inclusione aggiuntivi sono definiti dai singoli Moduli<\/li><\/ul>

Ischemic Stroke
Questo modulo mira a raccogliere dati longitudinali completi su pazienti con IS acuto per caratterizzare i predittori clinici e di imaging dell'esito.
Il modulo è integrato nel quadro di coorte più ampio (Livello 1) e si concentra specificamente sulle cause dell'IS, sul trattamento durante la fase acuta, nonché sulle complicanze e sulla prevenzione secondaria.
La raccolta dati e le misure di follow-up sono in linea con la raccolta di routine dei dati per la garanzia della qualità del Gruppo di Studio dei Registri Tedeschi per l'Ictus ('Arbeitsgemeinschaft Deutschsprachiger Schlaganfall Register' [ADSR], https://www.schlaganfallregister.org)
e con il Set Standard ICHOM per l'Ictus (https://www.ichom.org/patient-centered-outcome-measure/stroke/).

Emorragia Intracerebrale
Questo modulo mira a raccogliere dati su pazienti adulti con emorragia intracerebrale spontanea (ICH).
I partecipanti saranno caratterizzati in base a dati demografici, fattori clinici, trattamento e caratteristiche radiologiche, tra cui dimensioni dell'ematoma, localizzazione, dinamiche di espansione e presenza del «spot sign».
Lo studio mirerà a identificare predittori clinici e di imaging basali della mortalità intraospedaliera e a 3 mesi e dell'indipendenza funzionale, a esplorare sottogruppi con traiettorie prognostiche distinte e a indagare le associazioni tra presentazione clinica, parametri di imaging e complicanze post-ictus.
Queste analisi informeranno la stratificazione del rischio, guideranno le strategie neuroprotettive e di gestione acuta e supporteranno lo sviluppo di approcci interventistici personalizzati nell'ICH.

Gli esiti includono la gravità della malattia, lo standard di cura, le complicanze, l'ictus ischemico o emorragico ricorrente, il carico e l'impatto della malattia e del trattamento sulla qualità di vita, l'esito funzionale e la sopravvivenza in diversi momenti temporali.
Poiché SN Stroke è una piattaforma di ricerca adattiva che affronta varie domande di ricerca, ulteriori esiti e diversi determinanti che influenzano questi esiti possono essere aggiunti in ciascun modulo.
